Proximal Humerus Fracture Weeks 1-3 | Starting the Shoulder Rehab Process | Phase I

  • Welcome! You are here because you had quite a fall and need some guidance on your shoulder injury. I'm excited to be a part of your shoulder physical therapy process.
About 80% of proximal humeral fractures are called non-displaced. A non-displaced fracture is where the broken bones remain in their correct anatomical position. Non-displaced and minimally displaced fractures can usually be treated successfully without surgery. Surgery to align and possibly bind together with plates and/or pins may be necessary when bones are displaced from their normal positions. Treatment of displaced fractures in the senior population is controversial due to poor bone healing. Screwing into these bones can be like screwing into Balsa wood.
Why would I make a video series addressing this rare condition knowing I wouldn't get enough views to pay for the production of the series? Well, my story began when my mom broke her humeral head while running in the woods. Physical therapy and the orthopedic surgeon were good for knowing milestones, making sure she wasn't going to hurt herself at each stage but felt incomplete as far as a structured daily regiment that was sustainable. The weight of her rehab fell on me and the whole process made me realize that there were people, like you, out there suffering and feeling bewildered with the shoulder rehab process.
So, between your physical therapist, orthopedist and my videos, you are going to have great success. After the 13 proximal shoulder fracture rehab videos, there are no-weight and weighted shoulder strengthening videos on my site to guide you pack to lifting, throwing, swimming and hugging again. Don't give up. This is a long process that can take as long as a year to get back to pre-injury status.

Hi Dr Fields. I fractured my proximal humerus whilst cycling on Saturday and was diagnosed after a visit to A&E on Sunday. It is the first time I've fractured a bone, so all new pain game and swelling. As a cyclist training for endurance events these past few days since the accident have been a challenge. I have had an assessment at the orthopaedics yesterday and they informed me no surgery required which is good news and told me that physio rehab will start in 2 weeks. So whilst waiting it was wonderful to find this video and to begin some gentle exercises and have that feeling I'm getting better and starting on the road to recovery. Very grateful for this wonderful resource, and so well explained and demonstrated. Hoping for a full recovery and to get back cycling at the later part of the summer - here's hoping for a proper late summer here in England 😎🚴

Thanks so much Dr Fields, I broke both my humerus bones skiing a week and a half ago. 72 year old female, but fortunately no surgery and one arm has the clamshell brace. Pain hasn’t been too bad and it was so cool to find your videos. Just lovely with the music and inspiration. Unfortunately can’t do the self massage etc, but everything else. Hoping to heal quickly since my husband has to do everything, 🥰 with appreciation and gratitude, Sally

Broke my humerus bone while restoring my house, an old Irish cottage friday 02/02/24. I fell off my stairs missing my side step and fell all my weight on my shoulder. Clean fracture at the top of my humerus. Week 1 I felt better very quickmy - I’m making bone broth from ox tail, drinking a cup morning and evening, taking oyster shell pills, omega, calcium, d vit, zinc
My scan after ten days shows that’s it is healing in place and Dr was impressed that I had no pain when he pressed his fingers on my shoulder. Today it’s two weeks since accident and I’m feeling a bit down, wondering if I’ll ever recover fully when last week I was like yes and I’ll be back on handstands in no time :) thank you so much for your videos

I’m a 56yo female. I tripped and fell walking to work in a hurry yesterday. Had new multi-focal glass on. I’m amazed at how quick and easy it was to break my arm. 😂.. So anyway, thanks so much for being here. All of a sudden strength training and bone heath post menopause has become VERY important to me. I’ll be spending a lot of time on this channel no doubt 😁👍 Blessings to all on this journey of recovery and to ongoing better health! ❤

Thank you, Thank you. I fractured my humerus on Feb. 15th. my horse bolted as I was getting off. ER sent me home for a week with a sling and pain pills until I could see Orthopedic Dr. the very next day I was researching on TH-cam and ran across your video. You have helped me so much. I watch 3 times per day. To have something to follow along with is so helpful.

Just found your channel the other day and I’m so grateful to have found your channel! I fractured my right humerus bone on March 9th. My husband and I were getting off the golf course in our cart and he tried to avoid hitting another cart and I got thrown out and fell hard on my right shoulder and side. I suffered a mild concussion as well. Grateful to have not to have surgery, but I ain’t going to lie, this has been challenging for sure. I’m thankful too that I’m left handed so I can least you that arm and hand. I’m 63 and I have always been active and exercise 6 days a week. I’m sure my being fit kept me from having a worse injury. I do enjoy your calming demeanor and your positive mode of walking us through each move. I do see an upper cervical chiropractor for a different issue which has resolved through perseverance and a positive attitude! Thank you again and I look forward to complete healing with your guidance! Blessings☺️

I want to completely have full use and power before I start up again. And from now on, I’m driving the golf cart😉 Keep up the good work and helping us folks to complete wellness 😊

Thank you, endlessly! I cried like a baby, doing the exercise, from releasing so much tension! I fell down a 2 meter high ladder and got operated a week ago, have wires in my humerus and a sling...there was still so much tension, from the trauma...and the exercises help a lot, as well as your positive compassion. Thank you soo much!

No family. Was widowed some years ago and have no children. My age plus various chronic pain issues for many years has resulted in no friends. Oddly my neighbours haven't noticed my sling very much. One told me how she had a nasty cold and after 5 mins said "oh, what have you done?" No offers of help followed. Not that I'd have taken them up, but it would have been nice if she had.
So somehow managing on my own with right arm in a pretty poor way and left hand with thumb arthritis! Some tissue damage around elbow slowing things up a bit. And painkillers that don't seem to do much.
Anyhow it's been 2 months. Slowly getting back mobility and strength. It's nice to have found you.

Ttipped Fell / rolled down a steep concrete driveway 3 weeks ago.
Doing so much better still in a sling, but removing the sling for periods during the day, letting my arms stretch and doing your therapy.
I've done outside therapy before for other areas on my body, And you're right. I became bored and disengaged in going to therapy 3 or 4 times a week. It was the same exercises with little supervision
Your online therapy is really helping very easy. And I can do it several times during the day, without leaving my home
    Thank you for your videos❤

Hello my name is Yoto, I’m 37 yo from Sofia, Bulgaria. I fractured my proximal humerus, going back home, I fell from an electric scooter. I underwent surgery three days ago and I’m starting my recovery journey. I am a very active person and I go hiking, cycling and rafting a lot, and l have been in a lot of extreme and life threatening situations, but dumb accidents happen when you least expect. Thank you for these videos I will be following them all.
To all you people, who have suffered injuries, my message is this: Never give up, never surrender and you will get through every obstacle in your life, be persistent. Listen to your doctors, because they know better. I wish you all full and speedy recovery.

I am very motivated to improve as I will have a new grand baby to hold in July! I cannot, thank you enough. I broke this bone about a week ago when I flew over the handlebars of my bicycle, going downhill at 30 mph (I am 64 years of age, male). Thank you so much for these videos I just found you yesterday and have already done the video 4x I felt a little more sore the 4th time around, not sure why but I also tried not taking my meds until I absolutely had to.
I was celebrating my friend’s bday and we were being silly together, I have this thing where I love giving piggyback rides & I was telling her how proud of myself I was for getting to 250 lbs on the leg press machine at the gym, next thing you know I’m giving her a piggyback ride and it was raining out, we were two steps away from my front door when I stepped on slippery ground lost my balance and fell forward towards a wall with her on top of me. I fell unconscious for a couple of seconds and when I came to I was in excruciating paid. My boyfriend rushed me to the hospital & we discovered I had a spiral fracture on my humerus, completely broken. The worst pain I have ever felt in my life, they admitted me into the hospital and I spent about 3-4 days there, had surgery the day after being admitted & now have a titanium rod in my arm. This has been a very difficult and painful time for me as everything has slowed down, I’ve had to cancel my boyfriend’s bday plans this week since traveling to an island just doesn’t seem like an option & I’m praying I’ll be healed enough to fully participate in my maid of honor duties for my best friend’s wedding in June. My soft cast was removed last Monday & I’ve been trying to get an appointment to start physical therapy, my wrist and hand can barely move from being in a cast, surgery was 2/26/23 now I’m just sporting a wrist brace & sling, so grateful I found your videos to help while I get my physical therapy situated, you are a blessing.
Also I wanted to ask about the bone strengthening vitamins you mentioned I didn’t see a link for them in the description. Thanks so much!

I am sooooooo thankful I found your videos! I tripped over my dog’s crate on Jan 14 and my arm was outstretched so when I hit the wall, it bent up and backwards. I knew immediately that I needed to go to the ER. They said I had a slightly displaced proximal humerus fracture. I saw an ortho who put me in a sling and said the only thing I could do was bend at the waist and let my arm dangle a few times a day, until I saw him 3 weeks later. My boyfriend basically had to move in with me because I can’t dress myself alone (as a larger chested woman, a bra is not an option to me if I go anywhere, and obviously I can’t take one off or put it on by myself).
THEN a week later (on the 21st) I was taking my dogs out (one at a time) because my bf was staying at his house for the night. My precious pittie must’ve seen something (it was pitch black) and she bolted. It happened so fast and so sudden that it caught me off guard and I didn’t even have time to let go of her leash. I got pulled up against the corner of my house and then ping ponged hard on the ground - directly on my broken shoulder. I was literally writhing in pain and the tears started flowing (I didn’t even cry when I broke it the week before, nor did I cry 10 years ago when I broke my ankle in 4 places).
I called a friend who picked me up and took me to the ER. They re-xrayed it again and I had managed to further displace the fracture (you can see on the xray how it is now almost detached from the bone), AND I fractured my elbow (my arm was in the sling so I have no clue how I fractured the elbow).
I’ve been feeling more and more defeated because normally I’m super independent. I hate relying on anyone, let alone not being able to do “simple” things.
Not only did these exercises help relieve some of the pain, but your calming words almost made me cry as if I’m understood and not crazy or exaggerating the pain and inability to do certain things.
I’m definitely here with you for a while. Thank you again.

I fell the day after Mother’s Day and broke my humerus at 48 years old. I am a mother of 4 and I’d rather have 4 more babies at 49. The drs said it was a clean break; yet I sat in the soft sling for 5 weeks and now a long cast. I am week 5 again ( in long cast) and praying for total healing. I pray for us all because I have never broken a bone and I started roller street skating to loose the Covid weight and I would rather stay fat. I’m back at the orthopedic next week. I’m taking every bone mending Asian herb- including Acupuncture, I’m on Calcium, Bone Marrow, Vitamin D-K and Geritol. I feel better, but I am scared of everything; I don’t want to ever fall again. As soon as I get out of this cast, I’m buying knee, elbow, neck pads and probably bubble wrap. I just purchased the vitamin you mentioned and if you know any specialist in Atlanta Georgia please refer for therapy. I’m not entertaining surgery because I spoke to another lady 46 who have the same injury with surgery and is 46( younger than I) her healing is delayed so I am not forcing or rushing mine with surgery. Her bone shattered, mine didn’t and it’s lined up and healing in Gods timing ❤

You're absolutely amazing for making these videos 🙌🏼 I'm so grateful for having found these so early in my recovery. I found out Monday I fractured my proximal humerus (non-dislocated thankfully) on Friday in a ski accident in the High Tatras of Slovakia. I moved here from the US on a temporary work-visa and am an avid rock climber who just happens to like to ski sometimes. So much for climbing plans this summer..eek. I caught a bit too much speed going downhill and veered right to avoid other skiers/snowboarders because I knew I was going down. Hard hit. Bad bad, not good. I got up to assess the damage, heard a slight crackle/pop, and knew it was over... adrenaline was coursing through me and knew I needed to take several minutes to gather myself. Also, a friend I was skiing with almost had a heart attack seeing my *glorious* wipeout and I wanted to make sure they were good too. I got back up on my skis and when I felt the pain shoot in my arm when I grabbed my poles I knew then there was no way I could ski down from that point...so I trekked (ego was hit hard in that moment too). Ended up taking a lift back up (only way out was through) and then skied down a blue (no poles) to get to base, get out of my ski gear and take Tylenol immediately. Ended up using my doggie bags to fill with snow to ice the damage.
Anyways, it's been an adventure to say the least and going through Slovak healthcare system has actually been...good! X-rayed, ultrasound, and referred to a specialized sports center by Orthopedic badass I found for MRI to ensure no tendon tears and begin PT in a couple weeks.
Not sure what to actually expect for recovery and if/when I'll be able to get back to climbing/surfing in the next 12 months but hoping for the best and staying positive.
Thanks again for making these videos and your outlook -- it's genuinely so refreshing and nice (especially for someone who's so far from home).

Oh my, Talia! You remind me of myself in my 20s. Charging the mountains on my mountain bike, skiing double blacks, surfing double and triple overhead waves, rock climbing in Yosemite, etc.. Now I have 6 year old twin girls and we're skiing in Tahoe next week and jumping in the kelp forests in Santa Cruz in springtime. I know we speak the same language on that.
Well, the gifts of slowing down and patience are yours to be had with this one. Time to embrace a different perspective.

I took a tumble at the weekly 5k parkrun at our local country park. Tripped on a treeroot and faceplanted. There's quite a few staff from our local Hospital that regularly do this run, and that's how I ended up with two Consultants, a GP, two nurses, a Policewoman and a Physio ( all off duty ) fussing over me as they waited for the Ambulance. Impact fracture of the proximal humerus, gravel rash, bruised ribs etc. You don't bounce as well once you reach your 70's!.
These videos are excellent, can't wait to get out running again.

Dr. Fields, Thank you for you service providing self-help consult to those in need. I have successfully progressed through your entire Proximal Humerus Fracture Therapy series with excellent results. My 12 week Dr visit has indeed verified that my fracture has healed and my shoulder's range of motion has been preserved. I will need to continue ROM routine for I might have an acute rotator cuff injury that occurred when I initially fractured my humerus (minimally displaced greater tuberosity).
Post 12-weeks symptoms include shoulder pain when I reach behind back and attempt to lift arm away from back.
I am hopeful I will have good results treating this conservatively (without surgery). My Dr will assess progress 6 weeks from now.

Hello 👋 on 1/14/23 I was coming up from the basement with my arms full. When I got to the top my German Shepherd jumped up and pushed her paws on my chest, I flew down at least 6-7 steps, then slid the rest. I shattered my humerus, and fractured my ulna and radius. The surgeon was able to do the needed repairs with plates and screws. Things were moving along okay until I ended up with a horrible infection. Currently have a PICC line for daily Rocephin and Oral Rifampin twice a day. I have a wound vac on my incision to assist in the draining.
As of today 3/25, my labs are improving so much! I am on the mend, however the doc has to go back in, eventually, after bone growth happens, and remove the hardware. In the meantime I have home nursing and home OT.
I came across your videos this week. I am starting with week 1-3 and I love these exercises. Honestly my tightness has settled down a bit already.
I know I have 10-12 months of recovery ahead of me at least! Your videos are so helpful to me. Attainable exercises that I am comfortable with and definitely helps my mental health. Being home bound is HARD, especially for a 45 year old.
    Thank you so much!

Hi Dr. Fields! I’m 2.5 weeks into a mid-shaft humeral fracture (non-dominant limb 🙏🏼) & treating conservatively with a Sarmiento brace/sling. I’m a 61 yr old F, no h/o osteoporosis. Have been on D3 for a long time (last level was ~85 I think.) I was carrying a full laundry basket down the stairs and at the very bottom I must’ve thought I was on the last step but was on the 2nd. I stumbled across my kitchen & my humerus slammed against the edge of a granite countertop. The granite won. Thankfully I have no other injuries. My shoulder and wrist are good; radial & ulnar nerves intact. 🙏🏼 I’ve been rather surprised by how VERY painful this injury is. My husband is a big, strong guy and I’m little but the pain was such that he called EMS. I’m still needing a norco 5mg at bedtime and usually one more dose late afternoon. I want off asap but I’m also trying to be kind to myself.
Count me as being committed to your series here!
I do have one question: What about TENS? The med literature looks more positive for acute pain than I had expected. Thanks so much! Sorry for being verbose but I’m mindnumbingly bored. I’m retired and usually spend my days sewing & being artsy. I miss it.

      @dradamfields  7 หลายเดือนก่อน

I bet you miss using that wonderful shoulder. Nice D levels. Get on K2-7 as well for calcium appropriation. I am not a user of tens but, if you have it, there could be some blood flow and pain relief their. Might be good after the rehab exercises as well. Hand in there, girl! Check with your doctor but, probably time to get off of the brace soon. I'm glad you found me ; )

Broke my humerus skiing too getting bumped off a chairlift getting on. I was given a sling like the doctor is wearing too. When it came time for physio I was given a heated towel to kill the pain but unfortunately it would fall off as I tried to bend my arm. I then thought of buying a thermal leg wrap designed for horses which covered the whole length of my arm as the whole arm was painful and this was an excellent painkiller despite the fact it dripped water which I did not care about as it was working so well. I don't remember how many weeks of physio it took but it worked and I needed no painkillers at all.

Sorry this is the first time I have commented so you can't even imagine my history but here goes: my mother is here, she has dementia, I got up to do my walk at 7 am and have been going 90 mph since. My head was about to explode, had to eat some Calm chocolates. Long story short I came in my office to do the first video just to be able to sit and
rest and realized it is my new meditation. I did it with my eyes closed cause I have it memorized. My mom came out of the bathroom then began lugging her clothes out to the closest door. I know she wants to go home cause this is where she belongs right now to complete a work in her heart. Anyway we had a long talk I was quite happy but she
went back to negative. So after exercising my arm I thought of a song I wrote called Is Your Slate Clean and that it was talking to her I went to the piano and began to play it I found out how much I needed the breathing exercise and my fingers all enjoyed more exercise playing the piano and thought I should share with you because of the peace I got from you and your video. Great video and I will be practising these exercises every day as I was given nothing, just time healing advice.
Three weeks ago I was dumped by a wave and somehow injured my arm in the process, the pain was excruciating! The following day I saw an ER doctor and the x-ray confirmed that I have a fractured shoulder, not to immobilise it, just rest and painkillers. However I know have massive bruising running down my arm, which is gradually changing colour from black and purple to yellow, so I know the body is doing its job healing. I’m getting a lot of unwanted attention though because it’s ghastly to look at. Anyway should I get another x-ray from my own doctor and seek physical therapy ? Thank you.

I fell in the parking lot of Wal-Mart on February 23, 2024. All I was doing was opening the back door of my assisted living provider's car to put my bags in the backseat when I fell over the metal strip that anchors the cart corral to the ground (I told a few people that I lost a fight with the cart corral at Wal-Mart). Since I have a mental disability and receive services from ,my county Board of Developmental Disabilities, my provider had to make out an incident report. (Since I'm 47, I'm not eligible for home health, since I have a husband.) I was able to get in to see my primary care doctor that afternoon (I fell in the morning). Since this happened, my husband has had to get up when I do to help me take my shower, get dressed, and get my breakfast--then at night, he helps me change into my nightgown. I have been to an orthopedic doctor twice since it happened, and I have another follow-up and X-ray in a couple of weeks to determine how soon I can take the sling off.

My story is quite stupid, on December 3rd I went skiing and had a wonderful time. Went really fast, first day of the season and we decided to go eat at 2pm, we arrived in a skilift where we'd removed our skis and had them in had, the restaurant was across from us after a tiny ~10m hill followed by a flat area. We decided not to put our skis on and just walk/slide down. My friends did a few steps a few slides but I decided to use my ski boots as mini skis and sled the whole slope XD When met with the flat area, my boots blocked and I fell head first at very high speed, only one arm free, left one took all the shock, shoulder pop out, had to bring my arm under me to pop it back in. Diagnostic : a little of everything, micro fractures, ruptured ligament and tendon.

9 weeks ago I missed a step and fell reaching out to catch the top step. I dislocated my shoulder right off the bat. The emt came to reset my shoulder . It took 7 tries and 2 shots of pain medication to reset my shoulder. In the er the X-rays (and another round of of pain meds) showed my humerus was broken in several spots. Cat scan (followed by pain med) showed sever damage that would require surgery.
I had to go to the orthopedic surgeon. 2 weeks later is when I had surgery. A 2 hour surgery end up being 5 hours . I ended up with a plate and 10 screws to put my humerus together. Of course that was not the only injuries - my rotator cuff was torn in several places and totally torn from the bone in two places and my bicep muscle was severely torn. I’m a 64 year old female in reality good health. I was told I may never regain full use of my arm again - mind you this is my right arm and I’m right arm dominant. I started pt 4 days after surgery. I came across these videos for extra exercises to do (with pt approval) at week 7
I’m over 100 prom flexion and 60 prom abduction. I’ve been told because of the total damage my pt will be months slow and steady and not to push myself because I’ll injure myself again and set my recovery back. I was just given authority by the doctor to drive, with the caveat that I have to have a co-pilot for the next two weeks to ensure I can comfortably handle the steering wheel and shifting
    - even go park to drive refugees strength. No lifting over 2 pounds. By 12 weeks I can do 5 pounds. So everyone is different a lot depends on the extent of injury. I’m still trying to find videos that include a humerus break and rotator cuff damage. Hopefully these videos will help getting a close to 100% as possible I’ve been told I could be 18 months before I reach goal.

Hi Dav, You have a realistic goal. 12 to 18 months sounds about right. Even then you will be doing shoulder exercises forever but have great looking muscles ; ). There are no-weight shoulder exercises and weighted exercises that come after this series that will do wonders for you. Slow, steady and light reps are your friend right now.
...And BTW, you are now left arm dominant. Enjoy the musculo-neurological journey. I'm sure you are celebrating each little victory.

I fell thru my porch landing on Tuesday evening. The ER said it’s a simple closed proximal fracture of my left humerus I find l can do these first exercises fairly easily. Should l do these for awhile or move to second phase? This shoulder was frozen and had bone spurs a few tears ago. I remember the pendulum exercises. I can’t see my orthopedic person until 5/25. I remember my pt for my shoulder surgery began a few days after. It was very painful. This is very painful. I forged a close healing relationship with my therapist and orthopedic doctor at the time but my insurance changed and l can’t have them for this go around. I was only given three days worth of pain medicine so l am interested in some natural pain management as well. The exercises are wonderful.

Sorry to hear about your mom’s fracture - sending well wishes for her recovery 🙏 Your video timing is (unfortunately) perfect for me as 6 weeks ago while visiting friends and family in Whistler, I was walking my friend’s dog on an snow / ice covered sidewalk going downhill. With my arm outstretched holding the leash, I went down hard and landed in excruciating pain. I was assuming soft tissue damage, but an orthopedic doctor visit and X-ray when I returned home showed a non-displaced fracture of the proximal humerus bone - diagonally and laterally from the greater turburcle where infraspinatus attaches. Your encouraging words and guidance in your video are so appreciated. Im trying to accept the healing timeline but it is challenging. Grateful for this - thank you!

    Hi Dr. Adam !
I have Proximal humerus fructure on my right shoulder and arm since 06/04/2024, fell down from a plastic chair,
at hospital they applied cast for me it was so heavy and painful for 10 days, removed and used Arm sling with belt to hold the arm from the middle to lower pain.
I can move or lift my arm within a limit, I can wash my face, but can't comb my hair, my friend Ortho told me that I'm not supposed to use shoulder cast, anyway I'm trying to follow up your classes to exercise.
What do you suggest so that I can lift my hand up freely 😞

i had an unfortunate bicycle accident 10/22/2023. CT findings: 1). Mildly comminuted humeral neck fracture with extension to the anterior aspect of the humeral head articular surface. 2). Minimally displaced fracture through the base of the greater tuberosity. I am in a shoulder immobilizer and awaiting an appointment with an orthopedic surgeon 10/27/2023. I suffered a horrible calcaneal fracture in 2018 and after 2 surgeries required a subtalar fusion on peroneal tendon repair. I did not let that devastating injury slow me down.
