I'd say probably a year from very start, is when you'll be walking normal, lifting weights etc etc. I've seen some videos where the person is 8 months in from start, already lifting weights etc

He most likely wanted to retain his athleticism as much as possible. I probably would have gone for 12cm personally, that would still be pretty low-risk.
