    Thank you (again) Dr Chung for such a comprehensive and understandable discussion! I identify with 5 out of 10 of these reasons, and before meeting with you, I automatically expected….the worst. Once I understood this, I started to relax and not be overly worried, so therefore the spasms started to relent. I can still experience them, but not to the degree I once did and when they do happen, I know what to do. 🎉 If more and more medical personnel would actually truly do education and prevention, we would all be so much healthier!! You really are doing quite a wonderful service educating us all. 🥳🤗🥳 please, don’t stop!!

    Yes! I dealt with generalized anxiety for over a year then I started having these issues with my anus. Thank you for explaining Dr Chung. I am interested in you talking about how to fix it!.

    Belly breathing or diaphragmatic breathing is most effective to relax the muscles from spasm. Doing pranayam correctly can relieve all these symptoms
    This is from my own experience. I had severe spasm whole day which was not letting fissure heal. Only this deep breathing has helped me alot.

    Yes! This is my issue. I had to figure it out on my own. After years of tests, 3 different gastroenterologists, 2 GYN’s and a uroGYN, all who could not figure out my chronic constipation. Who all just kept giving me different types of laxatives. I knew it wasn’t a consistency problem, just by my symptoms. At first I thought I had some sort of pelvic prolapse and the gyn told me to do kegels. Come to find out (from my own research) I should have been doing reverse kegels, which I could not do them, until I learned to breathe correctly, by doing biological breathing. This has helped a lot. Although I still have a lot of the symptoms you talk about, they are so much more mild and the best thing is, I have pretty normal poops and feeling so much better. Thank you for such informative content.

    I just had to come on here and let you know that you saved me big time. Tried everything and even medications from local docs and they didn’t help. Was dealing with a fissure for a year. Came across your videos and listened to everything. Tried the panicura 5 and it worked. You have no idea how much that means to me.       I'm nearly 5 weeks after the doctor put 3 bands. I've this pain constantly it's making me mad. I practice yoga and can't even relax my anus with breathing. It's making me very miserable.

    Not once was stress and anxiety mentioned! Spasms and when you sit down to go - and it doesn't 'want' to come out are signs of anxiety and IBS - The correlation between your brain and bowel are very close, so, if you're in a very stressed state your bowel is going to be really effected.

      You mean pre-BM pain in the groin area? This happens to me when I’m holding it in too long because I don’t want to stop what I’m working on for a bathroom break. It can be one of those “pray to God make it stop” kinds of discomfort, but I’ve never had it last more than a few minutes thankfully. I can’t imagine that feeling going on longer than a few minutes. I’d be calling 911.

    I really like your content. I call mine the ring of fire the pain after a BM, but because of your video and a pelvic physiotherapist (Michelle Kenway on YT) im gonna say that I look at it different. My pain management and my view on it helps a lot!
    I am still in pain but I am not that anxious to go anymore. That really really really helped me.

    Gotta share your itching solution n all videos OMFG ! I think I love u 😆 Thank you so much - god awful burning w spasms starts after itching can last for rest of day, took a month after injury to get to take a BM 2 hours later it’s on for rest of the day. That lasted for weeks - Since learning of & using your simple remedy, NOTHING 🤯 … I’m shocked !! 100% shocked !! Can’t thank you enough 🙏🙏🙏

    I find sitting in a warm bath up to my waist mostly helps relieve the pain from my occasional proctalgia.

      The prep is the worst part. You go to sleep and wake up. Cleaning out with prep is the #1 compliant. I have plenty of them at age 74. You will be fine.

    Mine hurts so bad it effects my bladder. I just finally got relief with intestinal spam medication! It took 4 years!

    I have exactly this for many years after fissure operation because fissure would not self heal. Spasm now seems permanent. Have also had piles injected. Also internal camera all is healthy. This spasm gives me big problems. I have to take a lot of stool softeners fybogel. This also gives me problems with accidents. I hope so much your future video on this can help me and change my life. As I almost cannot leave the house.

      Apply some oil down there and massage gently but do not insert your finger inside. Massage it on the outside and notice the change, I have done it myself.

    Hi Dr Chung, this is by far the most informative discussion I've come across about something like this, a very helpful presentation. I was wondering, and I don't know if this has been asked or discussed, but do anal spasms ever get confused for something nerve related like pudendal neuralgia?

      Hey .... I have been dealing with what I was told was constipation. Now I am here trying to learn about this to help myself. I am also thinking pudendal nerve. Have you found out anything? I am taking Magnesium glycinate for the stress of this whole deal. It helps with muscle relaxation. But I would still have days that it wasn't enough. So I added vitamin B1.... oh man it really helped for a while... now it comes and goes on the helping that. So I am so very tired of dealing with this.

    Hey Doc, I am from Pakistan and I have been wathcing your videos since my hemmorried surgery. You are doing a great job. I really like the way you answer all the questions.
    I just want to discuss my case.
    I had my surgery 4 weeks ago. I am doing fine. My foctor left my wound open. I am doing fine but not 100% as I am still having a drainage on the guaze I place at my surgery place. It hase a small amount of yellow and green fluid on it. I am not feeling any pain or any blood in stool. I thought to discuss it with you. I am using Mycitracin cream at the wounds.
    Can you give your opinion on this one please.

    Thank you, Dr. Chung. This is very relevant to me after our consultation last week. I'm really looking forward to the video on the remedies!
    #1 - Pooping takes a while to get started - 15+ mins average
    #2 - Your farts are really loud! - Usually, control it by concentrating (external sphincter control?) or separating the butt cheeks
    #3 - You have to push to get the POOP out - I try to manage it by sitting longer duration (45 mins average)
    #4 - Poop looks like a thin spaghetti noodle - Generally slightly wider than an ink pen.
    #5 - Your anus shuts down the POOP pipeline - Only a problem during diarrhea or fissure. Long duration and relaxation techniques (Google maps scrolling) help
    #6 - Your anus feels like a grape - Rare but does happen (like right now). I always thought it was trapped gas.
    #7 - Anal pain can last for hours and hours! - I have been suffering from this for about a month after a fissure and subsequent 5 hours of back-to-back seated meetings
    #8 - The pain wakes you up at night! No Mercy! - Happened a couple of times during the last month. It was not excruciating but uncomfortable to wake me up
    #9 - "Burning" anal pain - Typically happens after diarrhea or a lot of poop. This was constant for about 3-4 days in the last month. I wonder if it is again the friction that causes this
    #10 - Bleeding + "Pain with a SOFT poop!!" - This happened quite a few times last year as streaks of thin lines on the poop. I thought it was undigested tomato!

    I had mistaken fissures & anal spasms for piles. & the doctor I went to suggested surgery right away. he didn't even inform correctly what is the problem I have. I didn't get the surgery & kept thinking I had piles.. for 2 years, I suffered from fissures, bleeding , pain & burning, without even knowing that I had fissures, not piles. I only researched about piles & thought I need to get surgery, & went back to same doc & got the surgery last week. he didn't even tell which surgery he performed, why & how will it help. Its my big mistake to not get a second opinion and get consultation from more places..T^T .. now I feel so sad & violated that I had only fissures that could be healed without a surgery, but was made to undergo surgery...

    you talked me out of it, im gonna die with my bleeding stage 4 prolapsed hemorrhoids... thanks for the honesty, my local doctors are god awful and very deceptive...
    taking Miralax is how i got the hemorrhoids in the first place... i didnt even take the 14g recommended, i only took 2 grams.... i spend the next couple days on the toilet with explosive diarrrhea and ended up with terrible hemorrhoids by the ned of it theyve been with me ever since on and off, well any time im standing...

    I have all of them except bleeding! The pain is burning, snakes biting or razor blades, it can be hell for a couple days. Still recovering from ileostomy reversal so that does not help!

    Place electric heating pad or warm towel in your lap and the sphincter muscle will relax in no time. This helped me big time and I take it with me wherever I go.

    I get no. 8's. They feel like a muscle cramp to me and it will wake me up from sleeping and I just have to wait it out, about 5-10 minutes.
    I also have bleeding fishures when I am tired, and sometimes they last for a looonng time (several minutes).
    I have started using a bidet and that has helped ALOT!

    I go to the bathroom and the pain last 6 hrs every time. I try immediately sitting in a tub and then putting on my cardizem with lidocaine cream but it doesn’t work. It kills me.

      @@user-ni1he8qv6u thank you so much for asking. I ended up having a fissurectomy which I guess is technically a type of sphincterotomy. I’m now going on 13 weeks after the surgery come this Friday. I was able to go back to work 4 weeks after. It’s still something I pay attention to every day. I had pruritis after the surgery, had to take 2 antibiotics. All in all after the surgery wasn’t as terrible as before. Before the surgery was the worst pain I’ve ever gone through. It was definitely manageable and got better every week. Now I’m pretty much back to my normal life except because of this I changed
      My diet a little. Mostly cutting out my lovely vodka, soda, and most dairy. Also don’t have as much fast food. Again thank you for asking. I wish you good luck. I hope you’re not going through this.

    My spasms are present the majority of the time, albeit painless, no blood unless straining strongly on internal hemorrhoids. I've been watching Michelle Kenway's vids...very helpful with Belly Breathing to relax. I have virtually every symptom you describe except pain, and have had the condition now for around 5 years, interestingly beginning after consuming some bad, rancid Trader Joe's peanut butter. Prior to that my movements were relatively normal, but since I discovered thru trial and error that peanut and nut butters lock up my colon like concrete. It takes a few weeks for that advanced constipation to pass. One particular "mechanical trigger" I've noticed is when laying down in bed, if any pressure of the mattress is upon the anus directly, or even just beneath the tailbone....seems to trigger an extra tightness. Therefore I most often sleep on my side. But my sphincter seems to now be in continual spasm. Your videos are THE most informative videos on this important subject. THANK YOU!

    Got every single symptoms. Came on after a fissure. Well I wonder if this was going on a little before the fissure but mainly got worse after. Even thought fissure healed it’s stuck like this.
    Tried the ointments but GTN eventually stopped working. pelvic physio kept aggravating it. Acupuncture didn’t work. Trying Botox now. Also tried amitriptyline and now trying duloxetine. At this point I feel like the doctors must think I’m making it up they are baffled it still hurts. At leas you understand.
    Look forward to the treatment video. Hopefully there’s something I’ve missed.
    Also they won’t prescribe me any sort of muscle relaxant so that’s frustrating as I would like to know how effective it would be.

    I’m female and recently was diagnosed & treated with anorectal fistula. MRI at first suggested there was fluid between muscles but surgeon changed his mind! At first I was treated with draining seton but I’m now a few days into recovery from fistulectomy & fistulotomy, with Botox and high internal haemorrhoid banding. I’ve seen no sign of the band or haemorrhoid yet but it’s very painful. The spasms I get seem to stem from the hemmerhoid I’ve been complaining about for years. My queries are could a hemmerhoid actually be a hidden thrombosed hemmerhoid under the tissue? Can spasms cause bladder/uterus spasm? Can piriformis syndrome cause rectal pain? I’m so scared I’m going to be left like this because I can’t live this way. I cried so much when I awoke from surgery! I’m in Scotland so don’t know how I’d get to see you!

    I read dicyclomine can help this, is that true? I was presrcribed another muscle relaxer Methocarbamol,doesn't
    seem to help much (I have a fissure) While having a BM doesn't hurt, it's after and for hours and hours. Doesn't bother me at night when I'm laying down and during the night and even when I get up,it's after the BM.

    🤣...farts, boulders and bricks ! lol......ok, I do get anal spasms and I have never even told my husband about these. I just get up and pace the floor for about half hour before it settles down and goes away....and it "always" happens at night. I recently learned that pressing a very warm, damp cloth against the area helps a LOT and relives the pain pretty quickly......"drop a duce" !! 🤣🤣 I love your descriptive and just plain out there references to real issues related to such a personal topic ! You're the man Dr. Chung ! 😂😂

    I have it 24/7 unless I quit lifting weights or if I'm on my feet to much and I only do upper body lifting for years because my pain and I thought it was Levator Ani Syndrome. I've been to 5 proctologist and 3 pelvic floor pts. I have all of these. The only reason I figure out anything is when I've been sick with covid and other illnesses that the pain is much better during that time. I post this because I hope it can help someone else. I spent so much time and money on hemorrhoid banding surgery pt etc to get no relief. I want to try a nerve block been chasing this for 5 years now. Good luck to everyone.

    Hello Doctor,
    I had grade III hemm. and have gone through the HAL RAR procedure with anolift, and now three months later and my hemorrhoids came back and is painful in the bathroom and when sitting. Can a hemorrhoidectomy be performed after HAL-RAR? Or are there other options in this case?

      So sorry this happened to you. This is exactly why I do not recommend the HAL RAR. I highly suggest you see a specialist in your area. I cannot comment because I have not examined you.

    Dr could you talk about manually dilating anus with fingers/anal plugs? I’ve read that it helps with relaxing and stretching the sphincter muscle to let fissure heal and that it doesn’t cause incontinence…and doing kegle along side the dilating. What do you think about manually dilating anus? Is it safe, cause infection, if I get the sphincter wider would it affect if I we to ever get surgery in the future?would it increase chances of incontinence because I manually dilated my butt if I underwent a butt procedure? Gracias

    Thank you for video. Question: is anal spasm same as Levator Ani Syndrome? I had LAS 10 years ago and with pelvic floor treatment … was gone. Reappeared 6 months ago. Am being treated for LAS from pelvic floor specialist regularly for 5 months now. Constant pain and can hardly sit. I do have several of the symptoms you mentioned. Also have IBS and currently investigating this as well. I sit with feet on a stool and never push but use the “Ssssss” breathing method so that the proper muscles are being used to send poop out. I am currently considering Botox on left side of rectum where I feel like I’m sitting on a small golf ball. Will only release for 24 hours after pelvic floor rectal therapy. I feel like I have a red hot poker up my butt constantly. Is Botox on that left side a good idea????? Please help. Thanks From Canada 😳

      I am desperate for help. I have the same concerns and every symptom listed plus many more! I live in a rural area with only 1 small hospital ,not many Dr.'s office.I started struggling with this 6 months ago and it's only getting worse, everyday it's pain and discomfort. I'm talking that pain that will have you in cold sweats and uncontroably shaking, it's gotten so bad in my experience that it is as bad if not worse than child birth. Ive seen 3 differe Dr.s in my town that dont take me serious. They prescribe me antibiotics (idk why) tell me to drink fluids, and use enemas. I am 26 years old and I am healthy, I do not want to live my life in constant pain like I've been.
