The wall is supposed to be the opposite side to the hand you want to initiate the behind the back dribble. So at 3:58, the wall would be on coach's right.
No specific grade level. I'd base it more off if they're able to a regular crossover well. Once they're able to do that, I'd immediately teach them this. They'll get it down quicker than you think if they put in the time and have good guidance.
I think it's just to get your hands closer to the ball. It gives you a reference point (your butt isn't going anywhere) on where to place your hands. My problem was exactly how he said, my hands were too high and too wide apart.

When I do the flat behind the back I find myself bending my knees inward, is that okay or should I work on having my knees facing opposite directions?
