It's the worst when I play games with QTEs. Resident Evil, TellTale games, etc. I'm always getting crushed, beheaded and julliened by half orc bards every 5 minutes.
Lets just say that of my in-game deaths are experiments in the name of science: "Does this game have fall damage?" "Will this campfire burn me?" "Am I able to swim without getting exhausted?" "Is that a NPC or an enemy?" ....I'm just checking so you don't have to.

I died *before* the tutorial in The Witcher 3. Walked out onto the balcony after getting out of the bath in Kaer Morhen and was so impressed with how gorgeous the view was, I tried to step out *onto* the balcony ledge for a better look. Slightly overstepped and fell to my death. Classic Geralt.
Shooting and/or blowing up an atomic bomb with conventional explosives is genuinely one of the safest ways to disarm it. Since the internal triggering mechanism requires a very carefully timed sequence of trips and triggers to cause the nuclear reaction, blasting the whole thing to pieces is the simplest way to ensure it doesn’t detonate the nuclear way. It’s still considered a method of last resort by Explosive Ordinance Disposal guys since it leaves a bunch of radioactive particles in the immediate vicinity, but it IS totally an option on their flowchart.

The most epically embarrassing way I've ever died was in Pac-Man 2. I defeated the final boss, went to triumphantly swing across the room to the exit... and the ceiling tile I'd hooked fell and crushed me.
So this is embarrassing, but in every Assassin's creed game I've played (and I've played most of them) I always manage to screw up at least one leap of faith and simply jump to my death. I guess that's what happens when you have "faith" that the hay cart is hidden somewhere beneath you and not on the other side of the building you're jumping off...
Simply free running is a hazard in any AC game. Undoubtedly the game will think you will be pointing mildly off course at some point and you'll just jump to your doom, rather than scale a tower or hop over a low wall.
Never really happened to me. I just lined up my character where the birds were. There is shit left behind if they fly away so you know where the haystack is. It’s pretty hard to miss it on a synchronisation point.

Moved a little too close to the mine while trying to get zombie's attention. BOOM! Should have just fired off a pistol round then legged it.
Also there is Vampire the Masquerade: Bloodlines Ever since the start of the game, every sensible character you meet tells you not to open the sarcophagus Then come the game's climax...you choose to open the sarcophagus
Or the Kuei-Jin who everyone tells you are trouble and even tell you themselves they consider kindred little more than abominations. Cue you, a kindred joining them clearing out their enemies (all the kindred) followed by them explaining there's one kindred left... ...and then they give you your reward. Just from Skyrim. Riding your horse over a cliff you thought was a lot smaller than it was. Killing the chicken in Riverwood right out of Helgen and then all of Skyrim shows up to kill you. Not seeing a super obvious tripwire or pressure plate and dying from the trap. Whirlwind sprinting off a cliff on accident. Not timing the Become Ethereal shout when jumping off a cliff. Moral of the story avoid cliffs in Skyrim, be nice to chickens, and watch out for traps.
The only ones I haven't died to that you listed was the horse one and the turn ethereal one, probably because I haven't gotten far enough into the game to get either of those.

+Overlord I only tried to about two or three times before I realized u couldnt do any sort of guard break like u could with some of the other bosses. so just jump over him a few seconds before he fires it at u and he will be wide open and defenseless for a few strikes....btw make it a double jump to ensure it misses and doesnt send u flying thru the air and into the ground like he has done to me SEVERAL times...also ACTUALLY block Vader since he will steamroll right over u if u dont.

I once had my most powerful Pokemon on 1hp, fighting Cynthia's Garchomp, I wasn't confident he could kill it in one hit so I decided to heal up and battle on from there. So I load up a full restore and use it, and then watch in abject horror as one of my other Pokemon, whom was only missing about 2hp itself gets fully restored and my main guy remains on 1. I can imagine the look of disgust he must have given me as he got obliterated by the Garchomp.
My favorite my own damn fault Dark Souls deaths are carefully setting up for a jump... and then accidentally back-stepping off _the other_ side of the abyss. I've done it in all three games.

The thing is, with _Portal 2,_ for the first half, falling is rally, really harmless. Chell even has a conversation with GlaDOS while they are both falling down a bottomless pit. So it's always a temptation in _Portal 2_ to see what's down there.
Funnily enough, logic says that Shepard would have survived... since you know, over 70% cyborg and those neurons being replaced with wiring long ago:D That's why the poison fails to kill you once your Paragon/Renegade score is high enough ie Sheppard is adjusted to their bionics. And why she failed to mind control us earlier if implants are working too.
An extremely old puzzle/platform game: "The Lost Vikings"; see, in it, each of the main protagonists had different set of skills and only one could actually jump. Ah, I've lost many a Baelog and Olaf by forgetting that, for them, space didn't mean they fly over a spike or a pretty deep pit :D Also, in older Fallout games - when you plan your turn in combat, decide to move a bit closer to the enemy to get a better chance of hitting him and, once you moved, realizing that you calculated wrong and you're EXACTLY one "action point" short to perform an attack ^^' This, naturally, gives your opponent a better chance at hitting you and the bastards don't even have the decency to thank you for jumping into their line of fire :D
@Stephen Older: well, both "The Lost Vikings" and "Fallout" are relatively hard to finish... in both cases patience is a BIG virtue, since if you mess up, you commonly need to start over from the last "checkpoint" ^^'
Fun fact: shooting a nuclear bomb probably wouldn't actually detonate it. Those things are (unsurprisingly) very complicated and any damage would probably just make it inoperable.
While wandering about in Serious Sam, I came across what looked like a movie set with a tiny frozen battle. Using the scope on my gun, I zoomed in on the tiny Serious Sam and wondered what would happen if I shot him. So I did. And I died. Don't know what I expected. I wish I could remember exactly where, but it's been years since I played.
2 years old comment, I know. But yea, I know that level, it's a secret on the "Tower of Babel" level from Serious Sam: The Second Encounter. (And on a side note just because, the secret itself represents the "The Great Pyramid" level, which is the final level of The First Encounter).

In Arkham city when you first enter the church and are held at gunpoint by guards, those guards slowly discuss whether or not to shoot me dead. I had a decision, listen to the button prompt to grapple to the ceiling... or play a game of chicken. They won't actually shoot the Batman right? It went not well.
I got one. When Fallout 4 first came out, I was pretty low level and decided to try my hand at taking on a Brotherhood of Steel patrol. I laid landmines, lured them into ghouls, I kept my distance and used grenades and pot shots. And I won. As I was celebrating and looting, I realized I was standing next to one of my landmines, and one of the ghouls had actually survived, and decided to tackle into me, and jumped right over the landmine that was right next to me. That is my one way I died that was totally my fault XD
Sleeping with morninth takes a special kind of stupid, as it requires you axing off samara, a comically powerful biotic who introduced herself by carving a bloody canyon through eclipse mercs like a complete badass, something even the most renegade Shepard would appreciate
Whatever happened to Halo 2. In the final level, Johnson literally tells you to get out of the way as he fires the Scarabs excavation beam to gain access to the control room. If you chose to ignore him, death comes quickly.
when playing Pokemon Mystery Dungeon: Blue Rescue Team and making it to the post-game mission that explains the story (in other words: the escort mission in Murky Cave) for the first time, I had no idea Murky Cave did not have any items except for money, so I went in there with only three apples and five Oran Berries in my inventory. died from hunger not even halfway through. to be fair, the game never made clear I needed to bring a full inventory of berries and apples (unless I failed at reading English and it did tell me), but it was still dumb.
I usually went into dungeons with my inventory one-third filled up with Big Apples, Oran Berries and Reviver Seeds, and I was expecting to find projectiles and more food items in the dungeon. Murky Cave has nothing, only money. game caught me off guard. you're completely right though, I should've been better prepared.

Dark Souls isn't even THAT hard, it's just that before it, gamers had gotten used to hand-holding and fake difficulty with artificial increases to damage instead of real challenge. I died *before* the tutorial in The Witcher 3. Walked out onto the balcony after getting out of the bath in Kaer Morhen and was so impressed with how gorgeous the view was, I tried to step out *onto* the balcony ledge for a better look. Slightly overstepped and fell to my death. Classic Geralt.

Shooting and/or blowing up an atomic bomb with conventional explosives is genuinely one of the safest ways to disarm it. Since the internal triggering mechanism requires a very carefully timed sequence of trips and triggers to cause the nuclear reaction, blasting the whole thing to pieces is the simplest way to ensure it doesn’t detonate the nuclear way. It’s still considered a method of last resort by Explosive Ordinance Disposal guys since it leaves a bunch of radioactive particles in the immediate vicinity, but it IS totally an option on their flowchart.
That is both useful and terrifying to know.

The most epically embarrassing way I've ever died was in Pac-Man 2. I defeated the final boss, went to triumphantly swing across the room to the exit... and the ceiling tile I'd hooked fell and crushed me.
So this is embarrassing, but in every Assassin's creed game I've played (and I've played most of them) I always manage to screw up at least one leap of faith and simply jump to my death. I guess that's what happens when you have "faith" that the hay cart is hidden somewhere beneath you and not on the other side of the building you're jumping off...

Simply free running is a hazard in any AC game. Undoubtedly the game will think you will be pointing mildly off course at some point and you'll just jump to your doom, rather than scale a tower or hop over a low wall.

Never really happened to me. I just lined up my character where the birds were. There is shit left behind if they fly away so you know where the haystack is. It’s pretty hard to miss it on a synchronisation point.

Just from Skyrim. Riding your horse over a cliff you thought was a lot smaller than it was. Killing the chicken in Riverwood right out of Helgen and then all of Skyrim shows up to kill you. Not seeing a super obvious tripwire or pressure plate and dying from the trap. Whirlwind sprinting off a cliff on accident. Not timing the Become Ethereal shout when jumping off a cliff. Moral of the story avoid cliffs in Skyrim, be nice to chickens, and watch out for traps.
"Be nice to chickens" also super applicable in Legend of Zelda. o_O

The only ones I haven't died to that you listed was the horse one and the turn ethereal one, probably because I haven't gotten far enough into the game to get either of those.
Traps do no damage, horses are invincible, cliffs are wimps, and that chicken had it coming.
Until dawn has some pretty good 'your own fault' deaths - like investigating the voice in the tunnels

Stupidly assuming that because I spam force push throughout the game as Starkiller that Darth Vader wouldn't likewise spam it against me.
+Overlord I only tried to about two or three times before I realized u couldnt do any sort of guard break like u could with some of the other bosses. so just jump over him a few seconds before he fires it at u and he will be wide open and defenseless for a few strikes....btw make it a double jump to ensure it misses and doesnt send u flying thru the air and into the ground like he has done to me SEVERAL times...also ACTUALLY block Vader since he will steamroll right over u if u dont.
You just have to get on that one pixel of raised ground and you get a god mode effect where he can't hurt you

I once had my most powerful Pokemon on 1hp, fighting Cynthia's Garchomp, I wasn't confident he could kill it in one hit so I decided to heal up and battle on from there. So I load up a full restore and use it, and then watch in abject horror as one of my other Pokemon, whom was only missing about 2hp itself gets fully restored and my main guy remains on 1. I can imagine the look of disgust he must have given me as he got obliterated by the Garchomp.
My favorite my own damn fault Dark Souls deaths are carefully setting up for a jump... and then accidentally back-stepping off _the other_ side of the abyss. I've done it in all three games.

The thing is, with _Portal 2,_ for the first half, falling is rally, really harmless. Chell even has a conversation with GlaDOS while they are both falling down a bottomless pit. So it's always a temptation in _Portal 2_ to see what's down there.

Ah, I've lost many a Baelog and Olaf by forgetting that, for them, space didn't mean they fly over a spike or a pretty deep pit :D
Also, in older Fallout games - when you plan your turn in combat, decide to move a bit closer to the enemy to get a better chance of hitting him and, once you moved, realizing that you calculated wrong and you're EXACTLY one "action point" short to perform an attack ^^' This, naturally, gives your opponent a better chance at hitting you and the bastards don't even have the decency to thank you for jumping into their line of fire :D

@Stephen Older: well, both "The Lost Vikings" and "Fallout" are relatively hard to finish... in both cases patience is a BIG virtue, since if you mess up, you commonly need to start over from the last "checkpoint" ^^'
Fun fact: shooting a nuclear bomb probably wouldn't actually detonate it. Those things are (unsurprisingly) very complicated and any damage would probably just make it inoperable.

I got one. When Fallout 4 first came out, I was pretty low level and decided to try my hand at taking on a Brotherhood of Steel patrol. I laid landmines, lured them into ghouls, I kept my distance and used grenades and pot shots. And I won. As I was celebrating and looting, I realized I was standing next to one of my landmines, and one of the ghouls had actually survived, and decided to tackle into me, and jumped right over the landmine that was right next to me. That is my one way I died that was totally my fault XD

when playing Pokemon Mystery Dungeon: Blue Rescue Team and making it to the post-game mission that explains the story (in other words: the escort mission in Murky Cave) for the first time, I had no idea Murky Cave did not have any items except for money, so I went in there with only three apples and five Oran Berries in my inventory. died from hunger not even halfway through. to be fair, the game never made clear I needed to bring a full inventory of berries and apples (unless I failed at reading English and it did tell me), but it was still dumb.

I was Bomb, don't remember what my "Helper" was. As I was bombarding Bonkers, my shit "Helper" hit the Bomb trap in the ground, destroying the floor. I was holding a bomb on my head when that happened.

How are people THIS bad at videogames? Dark Souls isn't even THAT hard, it's just that before it, gamers had gotten used to hand-holding and fake difficulty with artificial increases to damage instead of real challenge.
Games used to be hard to pad out game time, at least Dark Souls has a hell of a lot more going for it than just being hard.
