Dr. You really know Your Stuff!! THX for these Videos on Cervical Issues - these will Become very prominent in Society thanks to Forward head Posture !! Despite my many Spinal maladies of multiple sites of Mild to moderate stenosis throughout my spine with Spondyliosis , disc bulges, DDD (advanced) , and Dextroscoliosis at 20 degrees. Much of this is a result of a 1" shorter right leg since 1966 at age 15 when I fractured my femur playing HS Football. It was left unadressed by the Ortho Doctor at the time. Afteer more than 15 years, the Lower back aches appeared with stiffness. The LLD caused the hips to be misaligned and curving the spine. Chronic Sacroilitis (Inflammed SI Joints) is considered Ground Zero for many Back and neck problems according to my Rheumatologist. Most, if Not all Back Issues go Up and Down form the SI joints. Driving to commute to and from my work for over 40 years - at least 2.5- 3 houirs a day (and more driving for the Job as a Real estate Loan officer) with Forward head Posture (as more than 1 Chirpractor have told me) created and contributed to the deterioration of the Cervical spine with Cervical Instabilities {CI}. CI can make you Dizzy, lightheaded, Tinnitus, Sinusitis, ear and Eye pains, Temporal/Occiptal headaches, TMJ (is a Big one) , Costchrondritis, even IBS and Bladder/ Bowel issues of Incontinence (like me). Any Info like this is Valuable. 6 Surgeions I consulted in 2018-19 all said No Surgery for You.....Too Much Arrthritis on the vertebrae and facet Joints. The more frequently you become mindful of your posture, the more often it will be correct. Strengthening exercises will be a big help, especially those performed for endurance strength with high rep ranges and long isometric hold times.

Hi Hassan. The key to pillow position is to arrange the pillows to support your neutral neck spine position (which is the exact position of neck arch and any correction for side leaning or twist of the neck that provides the most relief). Typically, for most people with cervical DDD, this means finding a pillow height that allows a level head and neck (parallel with the bed) for side sleeping, and a flatter pillow (to avoid forward head posture) when sleeping face up. Most need a mirror to confirm the level side sleeping position as what we think is level is usually not. I hope this helps you. Dr. Remy

Hi Dr. I just found out last week I have ‘severe degenerative disc disease in my C5 and C6 discs. My doctor had a nurse call me to tell me the news. I asked what should I do and she said the doctor said I’ve to continue seeing my chiropractor that I started seeing in November because of the neck pain. I feel frightened that I’ll end up paralyzed. Hi Caroline. You are quite welcome, and please remember that the vast majority of degenerative disc disease cases never wind up "paralyzed", and with the right treatment and exercise/ posture/ movement approach often become pain free and allow the person to stay active for a lifetime.
Hi there. If you're open to holistic treatments for your issue, there is hope! Through the A.R.E. in Virginia, V.A., look into the book called "The Edgar Cayce Remedies." In the arthritis article it describes cases of people taking orally olive oil and using castor oil by massaging it on arthritic joints daily. The positive outcomes were incredible. With this, and exercises, may help you very much. Also, drink good quality water - up to 8 glasses per day as it lubricates the joints, including the eyes for that matter, and skin too. While I am not familiar with your exact condition and your treating doctor or therapist should be able to answer this specifically for you, most will be helped with getting up frequently to break the seated position, performing small range of motion neck mobilizations every 15-30 minutes when awake, and include light stretching of their targeted tight directions of movement every hour or so.

Hi Tatya. Degeneration of the discs in the neck is similar to the rest of the spine. While degeneration is permanent and discs can never fully heal ( because of a lack of blood supply to the majority of the disc), the pain associated with the degeneration can often be totally resolved and in many cases prevented from occurring again if the proper exercises are used to condition a neutral spine , the proper postures and movement patterns changed from old habits, and nutrition and movement/exercise supports the soft tissues around the region. The key is that the exercises and posture/movement changes must be tailored to the individual's needs, as no two cases of degeneration are ever exactly the same. I hope this info helps you. Dr. Remy

Hi. I have degenerative disk at c5 c6 and severe c5 c6 stenosis. My pain goes down to left arm and hand. I take 300 mg lyrica a day and 30 mg amitriptyline at night. Some diclofenac sodium when pain worse. What's the solution for this? Thanks

Very rarely does DDD lead to disability, and because you are so young, if you learn about your posture/movement, spine stability and stress tendencies, and make the appropriate corrections, including a neck fitness routine, your chances of problems in the future will greatly diminish. Osteophytes are the body's way of passively stabilizing joints that have too much laxity usually due to trauma or the degenerative process. If one stabilizes the joints with proper body mechanics, posture and stability exercises designed around a thorough analysis of stability faults, then the body has the best chances of reabsorbing the osteophyte as stability increases

After months of pain I finally got a new Doctor Who agreed to get me an MRI and I had some “Minor bulged disc’s in my cervical spine” also some up osteophyte minor. Along with that was minor stenosis which I’m crossing my fingers goes away once the disc completely heals! I’m doing everything in my power to strengthen my body. Physical therapy stretches and yoga on my own I’ve lost 20 pounds I’ve gone on a gluten-free diet I started drinking bone broth and as much anti-inflammatory foods as possible I’m committed!

While there are many surgical options for cervical DDD, remember that the most successful outcomes are those that address direct nerve compression, stenosis, etc, and not neck pain. Hi Mike. While traction (especially intermittent/pumping style) is generally helpful for degenerative disc problems, the problem with home traction units (and those found in therapy clinics) is that there is no way of localizing the traction to the specific spinal segment that requires it. The best type of traction is performed by a manual therapist who can more appropriately localize the stretch to the involved segment. If the entire neck is "generically" tractioned, you may be causing damage (over-stretch) to the segments that do not need traction. Great question, and I hope this helps. Dr. Remy
Braces will hold your shoulders back, but the negative is that they only lead to muscular weakness because they are passive. Reminding yourself to keep a better posture and actively using your muscles to maintain that correct posture is usually a much better choice in the long run. Pillows should hold a neutral ( and pain free) cervical spine position.
I can only speak for myself and have a severe degeneration in my neck which has progressed beyond the pain stage.I have zero lordosis and I use a really cheap flat lightly stuffed Walmart pillow.$5 here in Canada.The minimal stuffing can be squished around to fit exactly under the neck to provide support.I find it way better than the expensive neck pillows which aren't really adjustable.I use the regular pillows if I'm reading in bed or whatever.
Most people will need a pillow that is malleable so that the neck and head can be held parallel with the surface of the bed when side sleeping ( by puffing up the pillow height) , and then flattened so that when sleeping face up the head and neck are as flat as possible against the bed ( within comfort of course)
I'm 38 use to be heavy lifter had 3 shoulder surgeries first at 16yr last one few yrs ago. Lumbar fusion 2007. I have ddd in neck and I've been tearing my biceps and triceps partial tears and haven't been lifting weights. I tore my bicep lifting gallon of milk that's how easy. I was wondering if you would know if its caused by my neck or what you think. I've seen 5 different doctors nobody knows why. Thanks
Hi John, While I cannot diagnose you as you are not my patient, I can tell you that with DDD in the neck, there is often a low grade nerve interference from the nerves in the neck to the shoulder/arm region (including the bis and tris) which can cause tightness, weakness and increase susceptibility to injury/pain. I would recommend finding a good sports chiropractor in your area to properly diagnose your entire musculoskeletal ortho complex and advise you on a tailored treatment and custom rehab program. Hi Reihane. It all depends on your case. If bending your neck backwards is painful, you will likely need to work on bending more from the mid back rather than the neck. Learning your unique neutral spine position and RPI to stabilize your neck while bending back will be essential to decease further mechanical irritation. Sitting and sleeping positions require symmetry as explained in previous videos Noonshin. Any exercise that causes pain should be modified so it is pain free, including but not limited to pain relief exercises, strengthening exercises, coordination exercises, flexibility and balance exercises. These modifications may include a reduced range of motion, amount of resistance, time/ endurance and speed. Working with your doctor and a trained rehab and pain management specialist who is well versed in healing exercises is usually your best bet to make sure that the exercise is tailored to you and your unique body and issues. I got the following diagnosis (22 year old female). 160cm, 46kg. “degeneration of the intervertebral discs at the C3/4-C5/6 levels, and a slight decrease in the anterior curvature of the cervical vertebrae. Mild disc bulging is observed at each level of C3/4-C5/6. No significant abnormal signal is noted in the spinal cord. Summary: Cervical disc herniation at C3/4-C5/6. What exercises can I do? What I’m scared and confused about my future and my what may have caused this. I also developed pulse tinnitus in my left ear, occasional dizziness and nausea. I’m not sure what exercises are safe for home as my physiotherapist just used machinery and devices to elevate pain but I would like to strengthen my neck and shoulders without worsening symptoms. Are the exercises in this video the answer? Additional query: what is your professional opinion about the Islamic 5 daily prayers in which you prostrate, should I pray sitting down? Your exercises will be far less likely to injure and far more likely to assist your healing if they support your custom neutral spine position, correct muscle imbalances, use appropriate loads, and avoid the mechanical faults that stress your discs. I’ve heard peptide therapy is really coming along recently with peptides such as BPC-157 which promotes angiogenesis at injured sites. What do you think of such therapies combined with anabolic steroids (testosterone cycle) as a temporary “boost/stimulation” for healing of the discs? I’m thinking of combining these into my c5-c6 rehabilitation routine along with traction. I’m a 35 year old tennis player who gets debilitated by this every 3 months or so. I cannot figure out what triggers it. But it goes away after 2 weeks of agony each time it does occur.
I am never a fan of anabolic steroids due to the havoc it has caused many patients and training friends over the years. While peptides may have promise, the bottom line is addressing the mechanics and functional weaknesses that are causing the injury to recur. If you don't fix the root problem (mechanics and conditioning), you are trying to "catch a horse after he has already left the gate", and will never properly "heal" the issue

I have been maintaining good posture, targeting the muscles that need strengthening, doing overall core conditioning. And I thought the problem would not trigger again. but then I was sitting down after a work out.... and I think something as simple as an airconditioner blowing cool air triggered it. After the MRI confirmed bulging disc between C5-C6-C7 I talked to the specialist and he said this is something that will go away with time because after more disc fluid bulges out.... gradually there wont be much left to bulge out and irritate the nerves. But I am trying to preserve my disc.... not get rid of it! to be honest... he seemed disinterested and not in the mood to be there on a saturday. Is it possible to strengthen the surrounding tissues so much that the discs will not further degrade? I feel like this is a downward spiral.

About two months ago I noticed a hump at the back of my neck. I instantly thought I was developing a dowager hump and begun exercise to reduce it. I then realized i was getting worse and developed severe neck and chest pains. I then went for a full body scan and was told I have cervical calcification, low vascular elasticity and calcium loss. Now am terrified coz I have no idea whats causing it or how to stop the calcification. Doctor, can this help
Humps in the back of the neck can be due to a variety of problems, including calcification, fatty deposits (lipomas), tumors and mechanical/posture abnormalities. Corrective exercises will only be helpful in correcting those that are caused by the latter.

Dr remi, I was traumtic event in 20 year, and I have mri 6 year after injury (xr 2 day after) U can give me your opinion (I know I can help to slow the porogres, but I was have also trauma (hit head on wall in pool on swimming backstroke (I can share image from my case. Not have good advice after trauma (and just in 2016 I was more idea, but I steal some practice, but not with men give advice In the pool the was big crash in swimming. It depends on the stresses and rest applied to the spine, the nutrition and the effect of the correct exercise stimulus/fitness to the injured tissues. Because degenerative discs are part of a complex of mechanical stress, pain and the body's adaptation process, sometimes exercise and postural/movement corrections and healing exercise programs will slow down degeneration, while at other times it can halt or even slightly reverse degenerative tissues. To think that a degenerative condition must worsen as time progresses is simply not considering all the appropriate variables, and for cases with the correct tailored approach is untrue.

Hi Hassan.
The key to pillow position is to arrange the pillows to support your neutral neck spine position (which is the exact position of neck arch and any correction for side leaning or twist of the neck that provides the most relief). Typically, for most people with cervical DDD, this means finding a pillow height that allows a level head and neck (parallel with the bed) for side sleeping, and a flatter pillow (to avoid forward head posture) when sleeping face up. Most need a mirror to confirm the level side sleeping position as what we think is level is usually not. I hope this helps you.

I asked what should I do and she said the doctor said I’ve to continue seeing my chiropractor that I started seeing in November because of the neck pain. I feel frightened that I’ll end up paralyzed. You are quite welcome, and please remember that the vast majority of degenerative disc disease cases never wind up "paralyzed", and with the right treatment and exercise/ posture/ movement approach often become pain free and allow the person to stay active for a lifetime.

While I am not familiar with your exact condition and your treating doctor or therapist should be able to answer this specifically for you, most will be helped with getting up frequently to break the seated position, performing small range of motion neck mobilizations every 15-30 minutes when awake, and include light stretching of their targeted tight directions of movement every hour or so.

Degeneration of the discs in the neck is similar to the rest of the spine. While degeneration is permanent and discs can never fully heal ( because of a lack of blood supply to the majority of the disc), the pain associated with the degeneration can often be totally resolved and in many cases prevented from occurring again if the proper exercises are used to condition a neutral spine , the proper postures and movement patterns changed from old habits, and nutrition and movement/exercise supports the soft tissues around the region. The key is that the exercises and posture/movement changes must be tailored to the individual's needs, as no two cases of degeneration are ever exactly the same. I hope this info helps you.

Good day, Doctor. I would like to ask if the main symptom of DDD is tge pain in the neck that occurs every now and then. I used to feel this kind of pain in my neck for a month now, but I have never tried to consult it to a doctor. You need to begin by seeing a doctor who specializes in neck conditions for a proper diagnosis and treatment plan. This should be followed by an in depth analysis of your body mechanics and spine stability issues as they relate to your symptoms for proper rehab design and fitness considerations
From my experience pain from dd in the neck usually begins in the shoulders first as time goes on the neck and head takes the brunt.
Hi. I have degenerative disk at c5 c6 and severe c5 c6 stenosis. My pain goes down to left arm and hand. I take 300 mg lyrica a day and 30 mg amitriptyline at night. Some diclofenac sodium when pain worse. What's the solution for this? Thanks

Thank you so much for taking the time to answer my question. I have been trying to figure all of this out on my own. After months of pain I finally got a new Doctor Who agreed to get me an MRI and I had some “Minor bulged disc’s in my cervical spine” also some up osteophyte minor. Along with that was minor stenosis which I’m crossing my fingers goes away once the disc completely heals! I’m doing everything in my power to strengthen my body. Physical therapy stretches and yoga on my own I’ve lost 20 pounds I’ve gone on a gluten-free diet I started drinking bone broth and as much anti-inflammatory foods as possible I’m committed!

While traction (especially intermittent/pumping style) is generally helpful for degenerative disc problems, the problem with home traction units (and those found in therapy clinics) is that there is no way of localizing the traction to the specific spinal segment that requires it. The best type of traction is performed by a manual therapist who can more appropriately localize the stretch to the involved segment. If the entire neck is "generically" tractioned, you may be causing damage (over-stretch) to the segments that do not need traction. Great question, and I hope this helps.

Braces will hold your shoulders back, but the negative is that they only lead to muscular weakness because they are passive. Reminding yourself to keep a better posture and actively using your muscles to maintain that correct posture is usually a much better choice in the long run. Pillows should hold a neutral ( and pain free) cervical spine position.

What would be the best pillow height or sleeping position for this
I can only speak for myself and have a severe degeneration in my neck which has progressed beyond the pain stage.I have zero lordosis and I use a really cheap flat lightly stuffed Walmart pillow.$5 here in Canada.The minimal stuffing can be squished around to fit exactly under the neck to provide support.I find it way better than the expensive neck pillows which aren't really adjustable.I use the regular pillows if I'm reading in bed or whatever.
Most people will need a pillow that is malleable so that the neck and head can be held parallel with the surface of the bed when side sleeping ( by puffing up the pillow height) , and then flattened so that when sleeping face up the head and neck are as flat as possible against the bed ( within comfort of course)
I'm 38 use to be heavy lifter had 3 shoulder surgeries first at 16yr last one few yrs ago. Lumbar fusion 2007. I have ddd in neck and I've been tearing my biceps and triceps partial tears and haven't been lifting weights. I tore my bicep lifting gallon of milk that's how easy. I was wondering if you would know if its caused by my neck or what you think. I've seen 5 different doctors nobody knows why. Thanks

While I cannot diagnose you as you are not my patient, I can tell you that with DDD in the neck, there is often a low grade nerve interference from the nerves in the neck to the shoulder/arm region (including the bis and tris) which can cause tightness, weakness and increase susceptibility to injury/pain. I would recommend finding a good sports chiropractor in your area to properly diagnose your entire musculoskeletal ortho complex and advise you on a tailored treatment and custom rehab program. It all depends on your case. If bending your neck backwards is painful, you will likely need to work on bending more from the mid back rather than the neck. Learning your unique neutral spine position and RPI to stabilize your neck while bending back will be essential to decease further mechanical irritation. Sitting and sleeping positions require symmetry as explained in previous videos Noonshin. Any exercise that causes pain should be modified so it is pain free, including but not limited to pain relief exercises, strengthening exercises, coordination exercises, flexibility and balance exercises. These modifications may include a reduced range of motion, amount of resistance, time/ endurance and speed. Working with your doctor and a trained rehab and pain management specialist who is well versed in healing exercises is usually your best bet to make sure that the exercise is tailored to you and your unique body and issues.

I got the following diagnosis (22 year old female). 160cm, 46kg.
“degeneration of the intervertebral discs at the C3/4-C5/6 levels, and a slight decrease in the anterior curvature of the cervical vertebrae. Mild disc bulging is observed at each level of C3/4-C5/6. No significant abnormal signal is noted in the spinal cord. Summary: Cervical disc herniation at C3/4-C5/6.
What exercises can I do? What I’m scared and confused about my future and my what may have caused this. I also developed pulse tinnitus in my left ear, occasional dizziness and nausea. I’m not sure what exercises are safe for home as my physiotherapist just used machinery and devices to elevate pain but I would like to strengthen my neck and shoulders without worsening symptoms. Are the exercises in this video the answer?
Additional query: what is your professional opinion about the Islamic 5 daily prayers in which you prostrate, should I pray sitting down?

Your exercises will be far less likely to injure and far more likely to assist your healing if they support your custom neutral spine position, correct muscle imbalances, use appropriate loads, and avoid the mechanical faults that stress your discs. This must be determined through an in depth analysis of your body mechanics and spinal stability/conditioning issues and level as they relate to your pain condition. I’ve heard peptide therapy is really coming along recently with peptides such as BPC-157 which promotes angiogenesis at injured sites. What do you think of such therapies combined with anabolic steroids (testosterone cycle) as a temporary “boost/stimulation” for healing of the discs? I’m thinking of combining these into my c5-c6 rehabilitation routine along with traction. I’m a 35 year old tennis player who gets debilitated by this every 3 months or so. I cannot figure out what triggers it. But it goes away after 2 weeks of agony each time it does occur.
I am never a fan of anabolic steroids due to the havoc it has caused many patients and training friends over the years. While peptides may have promise, the bottom line is addressing the mechanics and functional weaknesses that are causing the injury to recur. If you don't fix the root problem (mechanics and conditioning), you are trying to "catch a horse after he has already left the gate", and will never properly "heal" the issue

I have been maintaining good posture, targeting the muscles that need strengthening, doing overall core conditioning. And I thought the problem would not trigger again. but then I was sitting down after a work out.... and I think something as simple as an airconditioner blowing cool air triggered it. After the MRI confirmed bulging disc between C5-C6-C7 I talked to the specialist and he said this is something that will go away with time because after more disc fluid bulges out.... gradually there wont be much left to bulge out and irritate the nerves. But I am trying to preserve my disc.... not get rid of it! to be honest... he seemed disinterested and not in the mood to be there on a saturday. Is it possible to strengthen the surrounding tissues so much that the discs will not further degrade? I feel like this is a downward spiral.

About two months ago I noticed a hump at the back of my neck. I instantly thought I was developing a dowager hump and begun exercise to reduce it. I then realized i was getting worse and developed severe neck and chest pains. I then went for a full body scan and was told I have cervical calcification, low vascular elasticity and calcium loss. Now am terrified coz I have no idea whats causing it or how to stop the calcification. Doctor, can this help
Humps in the back of the neck can be due to a variety of problems, including calcification, fatty deposits (lipomas), tumors and mechanical/posture abnormalities. Corrective exercises will only be helpful in correcting those that are caused by the latter.

It depends on the stresses and rest applied to the spine, the nutrition and the effect of the correct exercise stimulus/fitness to the injured tissues. Because degenerative discs are part of a complex of mechanical stress, pain and the body's adaptation process, sometimes exercise and postural/movement corrections and healing exercise programs will slow down degeneration, while at other times it can halt or even slightly reverse degenerative tissues. To think that a degenerative condition must worsen as time progresses is simply not considering all the appropriate variables, and for cases with the correct tailored approach is untrue.
