      Hi there, yes you need a consistent progressive plan in place. It is very easy to do too much sometimes when we feel "good" and we end up as you say going backwards.     I did best rest for 2 weeks and was feeling much better, couldnt wait to deadlift heavy again..so i did and back to square 1..ugh. Guess I can't do what I love anymore..but you gotta do what you still can. Hopefuly one day

      Hi there, unfortunately this injury takes a lot longer than 2 weeks to heal. You need to treat it seriously and adapt your training to ensure a full recovery.

    Great video. A lot of people think reabsorb means it goes back into the disc. What actually happens is that your body's macrophages erode the part of the disc (nucleas pulposas) that escapes past the wall (annulus). You don't get it back into the disc.
    In the 2 to 4 months that you waited for the sciatica to stop, it's interesting that you could do planks. They are causing my sciatica symptoms to inflame. I'm a month from the chiropractor lighting me up and really injuring me. I had minor numbness before that.

      Hi there, there are actually 3 theories that are hypothosised. One is the inflammatory response by the body does bring about phagocytosis by the macrophages, the second is dehydration of the extruded disc material and the third is retraction back into the parent disc. It could be that the type of disc bulge/herniation present will dictate the preference of one of the suggested theories. It could also be that they do not all work in isolation but each have a part to play. One must also remember that some disc herniations/bulges do not reabsorb fully but the patient will become pain free and will have no more symptoms. Regaining a pain free active life should be the ultimate goal regardless of what the disc is doing :)

    Hi @BashBackPain - I already had surgery on l5s1 herniation on Nov 2023. Same l5s1 has reherniated and am in a lot of pain. Bedrest has helped over the last 2 weeks but the minute I walk around it hurts. Am on a lot of pain meds and pregabalin- can I start the exercises now when I’m in pain?

      Hi there, yes you can start with basic movements such as abdominal bracing, dead bug, arms only bird dog, hip extension and others that do not cause pain when you do them - you can find those and others here th-cam.com/video/I1RqzZqfh6g/w-d-xo.html and here th-cam.com/video/2IO3v6LAqUg/w-d-xo.html obviously you do not want to overdo it but movement will encourage blood flow to the area which is necesssary for healing. I know walking is painful for you but even if you can mange 1 to 2 mins at a time it is a start and if you can do maybe 10 of those mini sets a day that is a good starting point.       @@24jesusperez feeling better because i stopped lifting weights , i limited sitting time and i m walking 10k steps a day , it took me 2 years to heal

    Great video! So inspiring! I’ve been there also…. I’m 47, first surgery in 2003, ‘cause I tought it was the fastest solution and in a certain way works, but after only three years I had second surgery…. Then I recovered quite good with good lifestyle, exercises and workouts. In 2015 I was again there! But at that time I listened to my chiropractor and follow more conservative approach: a couple of cortisone injection in the spine to get ride of the pain and a lot of exercises. In these last 8 years I was gable to get back in a very good shape, squatting, deadlifting heavy and having a normal life. This summer for some reasons that I can’t understand I started feel gradually sciatica pain till terrible level…. I’m now struggling to recover again and this time I’m trying to have an even smarter approach and hopefully it will be the last time in my life!!!

      The one thing we have to maintain is spine health and very often we forget about the exercises we did during rehab and go back to our old ways. I still do my decompression stretches after exercise, especially if it has been leg day. It is difficult to say but we could infer that once you have injured a disc it may be more prone to issues in the future so it is best to try and maintain good disc height and health as we age and continue to exercise.     This was the most helpful video I've watched. I find it fascinating how you can see doctors and physical therapists and still not get the insights and information you need for a full healing journey. You really need to learn from someone eho has been through it, who also has a strong grasp of how the body and spine function who has also done their own research. Thanks for sharing your insights and inspiring people going through it!

You would not believe how many times i hear the story of patients not being given the right information and having no idea what is actually wrong with them when they get diagnosed with a herniated disc.       @@bashbackpain I am. L5 S1 here as well. It's been 6-7 weeks since the aggravation (improper kettlebell swings, too much weight). Still limping and dealing with pain. Been diligent with core strengthening. But only started your decompression exercises yesterday. Helped a bit paired with stability work. The area I feel lost in is I am extremely tight around my hips and psoas, due to inactivity and lack of mobility. But my impression is stretching the hamstring while laying aggravates the nerve causing more pain. Doing some psoas stretches that feel comfortable but the kneeling hurts even when bracing correctly. I'm not sure how much stretching or what tissue work I should be doing at this time. Any thoughts?

      Ok, from my point of view you should never directly stretch the hamstring when suffering from sciatica as it simply aggravates the nerve and we don't want to do that. Try this routine and see how you get on - th-cam.com/video/t18SLywdzNE/w-d-xo.html - you may want to avoid the piriformis stretch at 5:24 but try the rest. These are myofascial stretches and not directly muscular so a little bit different. The idea is you stretch around the hamstring area and the hips to encourage good movement patters to reduce the stress on the lower back/compression on the nerve. As for release you can use a la crosse ball/tennis ball on the glutes but you need to be very careful you don't actually hit the nerve itself, you will know if you do! I have a video on self tissue release coming out this sunday. As you are only 6-7 weeks out from the initial injury it is normal to still have this level of pain. Are you able to sleep with no issues?

      Hi John, normally yes age is a factor, the older we are the longer things take to heal. However we are usually less likley to push too hard too soon so generall don't relapse like some younger eager beavers might. The main thing though is healing is possible no matter the age.     Writing this from my bed, I was walking with pain week a go I was able to sit, drive and do simple things, after one session with chiropractor I became a handicap can't walk now it's beed a 10 days in the bed thinking of surgery but I'm a fighter and this video gave me hop that giving enough time body will heal it self because human body is design to repair it's self to survive, Thanks brother as soon as I start walking I'll make a similar video.

    you give me hope bro, i have a herniated disc in greece and after six months have still not found a solution. although the pain has lessened, i am still not 100%, this is the worst pain ive ever had. even worse than wisdom teeth removal on a 24 hour plane and having covid twice - will look at getting into ur course

      Hey fella, a few of things you should start doing. Decompression stretches daily and as many times as you can during the day but not first thing in the morning.       @@smith2186 hey mate , i found out that I actually had piriformis syndrome and not a herniated disc . i finally fully recovered and no longer have this fake sciatica . the stretches u recommended helped a whole lot in general

    Hey, I really appreciate this video. I have two herniations the worst one that’s causing problems is the L5 S1 and I got my first lumbar epidural shot about 11 days ago. I’m feeling better but I have been dealing with this for about five months. I haven’t done as much core exercises as I should have. I’ve been doing the McKenzie more often and walking more often, I have been careful with flexing the spine forward and I usually brace my core and drop to one knee when picking something up or squat down with a straight back when picking something up. Still struggling with pain and I can’t really extend my leg, all the way out, both legs while I’m laying flat on my back. Any suggestions that you have let me know please I would really appreciate it.

Before you do these i would also follow these stretches first - th-cam.com/video/2BQJyIYwssM/w-d-xo.html BUT minus the nerve glide and do it after the stretches. Don't force the nerve glides, try them twice a day preferably starting after lunch and every second day to start. Let me know how you get on :)

    Stepped off a high pavement 3 years ago, felt a pop in my back and that was the end of my active life. The pain has been comparable to a gunshot wound at times, worse than anything I've ever experienced. Managed to just about survive by sleeping on a very hard bed, but I'm very limited in what I can do.
    I decided I was tired of waiting for this to get better and that I'd adopt a fighting mindset, I'm not going to become disabled without a fight.
    I've found some benefit in planks and cobra stretches, would appreciate any links you could toss my way.

    I have 3 heirnated discs myself been like this over 4 yrs recently got an inversion table hope it helps i alao hsve hip pain and right side glute pain i was told prp injections are my fix so wirh me its a waiting process but ill take any advice tips or videos im looking for relief at least for flare ups and spasms thanks i already dropped over 30 pounds still in lain feel like i tried every stretch in the book as well but im willing to try new ones

Hey i didnt get to try any of the exercise u outlined, unfortunate for me, i rehurt myself a month ago by sitting for almost an hr in a car by the hospital, i didnt know sitting can b so harmful. So its 2 months now and i still with back pain, i even feel weakness in my legs too, and my left feels painy as well. Kindly say if bulg disc heal? My mri show 2 bulg disc, its not hernated. Plz share ur knowledge

      The body is very good at healing itself if you provide the right conditions and for more than 90% of people with disc problems they heal with conservative treatement. This video goes over the sort of things you should be doing in your day - th-cam.com/video/wOIG6UtO0vk/w-d-xo.html and these are some uppper body exercises to try - th-cam.com/video/nCwPBrt1ZU0/w-d-xo.html and some lower - th-cam.com/video/3idof-42Hq8/w-d-xo.html along with some core work - th-cam.com/video/I1RqzZqfh6g/w-d-xo.html - i would pick 2 exercises from each video and do them 3 days a week along with the daily routine and then on the non-workout days i would be doing this stretching routine - th-cam.com/video/t18SLywdzNE/w-d-xo.html - wait at least 4 hrs before doing any stretching or exercises, do not do them when you first get out of bed in the morning.       @@bashbackpain Thank you for answering my message. I am 31-year-old, and I got a protrusion disc L5-S1 paracentral I think this was when without realizing it I lifted more weight in the gym. I have followed all their recommendations and I think I am improving. but I still have pain in my lower back but not exactly in the protruded disc. My work requires physical demand and sometimes dragging heavy things. I would like to know: how long the disc could heal? How should I train to protect the nerves? (my physiotherapist gave me a test and it was successful) that I should eat to heal quickly and when could i return to my work? I really appreciate your help. i don't smoke also i don't drink alcohol.

      Hi Joan, apologies for the delay. Residual pain can last a while but it will dissipate as you recover. For someone with a manual job it is very important to try and manage how you move and to brace your core whenever you are moving something heavy. For core work this is a good start - th-cam.com/video/EOQlETW_KQE/w-d-xo.html - as for a daily spine hygiene routine this is one to watch - th-cam.com/video/wOIG6UtO0vk/w-d-xo.html and for helping the disc reabsorb this is one that should be done daily - th-cam.com/video/vgId5sj7QZI/w-d-xo.html - also when you get home allowing time for the lower back to relax is also very important check this one - th-cam.com/video/yTGojXMSAVU/w-d-xo.html - As for eating to help with healing you want to avoid inflammatory oils, namely corn oil, trans fats and canola oil. You want to ensure you are eating adequate protein and as unprocessed as possible. Getting strong and stable is vitally important to assist the body as if you put the body in the right position to heal it will heal, 99% of the time.     Hi, I am having L4-L5 (mild) and L5-S1 disc bulge and associated pain on right buttock, calf and ankle since 2017. The pain is not that intolerable, but restricting my activities, especially on the glutes and calf. The MRI also shows mild degeneration on L5-S1 disc. This happened after a heavy overhead lift and since then I have tried many treatment methods including medications, massages, physiotherapy, chiropractic, homeopathic etc. I was into games and workouts but after the injury am out of all my activities and stressed. There was a period of time when I felt better and as you mentioned in the video I get back to the games and boom, the pain is worsen. Now, I am seriously stressed and low in confidence. I wonder if I can get better as the injury is almost 5 years old now, especially with the degeneration on L5-S1. As you have already been through this agony, hope you can understand my situation. Please advise

      I have heard a lot of different opinions from different doctos / chiropractitioners that stretching hip flexors / glutes etc. is sometimes even counterproductive as it irritates the damaged disc even more as its pulling on the sciatic nerve. What's your take on that? @@bashbackpain

    What about PRP or stem cells?
    I’ve had a herniated C5 and tried everything 10 yrs.
    Maybe cleaning that bone spur or cutting out part of disc that’s pressing on nerve might be only option. You advised against it

      Hi there, both PRP and stems cells would appear to be beneficial for herniated discs. I am not aware of too much peer reviewed literature in that area specifically but the anecdotal evidence seems very positive. If suregry is your last hope and you have tried everything else and you are not able to live with the pain/disability and you have spoken to a number of doctors who do say this is the only way then suregry will not be considered. This is your choice though and i can't tell you yes or no. What i would suggest is trying some decompression stretches such as this El Doa for the cervical region to see if you get any relief - th-cam.com/video/3WsrIfljmp4/w-d-xo.html - try it daily, before bed for 5 days out of 7 then get back to me and let me know how you are doing :)

    Thanks for ur first reply. I want to share my present situation with u. So i suffering with back pain for a month now, the mri sho 2 bulg disc in the lower back, the doc sa i have 2 pinch nerve, he is not a helpful doc, he dont want to give a steriod shot, he is only recommending surgery. I frm Guyana SA. I on pregablin and pain tabs. My walking is very slow, cant go normal speed. My exercises is walking alone, we dont have chiropractor in our country to get guidance. I depend on ur informative videos alone. I avoid bending and tilting. I stay away from work cos of my pain an slow motion and jus to care myself. Plz advice how i can improve from here

    Any recommendations I been dealing with pain off and on it start about 1 year ago with lower back pain and sciatica at first I couldn’t walk or stand for no longer then 5 min and every time I walk it was hurting so I went for a mri and I got a L5S1 herniated disc then I got a epidural injection it help for maybe 2 weeks now I have some pain on my lower back and backside of my knee but only if I stand for long time should I keep waiting and have my body fight this

    After 6 months I've still got drop foot. How long can the nerve root be impinged before permanent damage?

      Hi there, do you have any ability to flex the foot towards the knee at all or have you completely lost all of that control? Does the foot drag toes down when you walk? As much info you can give me as possible would be helpful :)

      @@bashbackpain I can actually flex it all the way up but it feels like it takes a lot more effort than my good leg. I did trip the other day as presumably my toes were down more than usual. The biggest issue is running at speed which used to be no problem now I just make so much noise with foot slamming on treadmill in gym people start looking at me. I've had differences of opinion from clinicians, some saying it's just ligament damage and others a bulge. It was deadlifting with poor form that caused it. Not yet had x-ray or MRI. I was in quite a bit of pain for the first month, but after that it was just numbness and tingling and then finally just numb big toe, but the foot drop has just never resolved.

      Should add that the stand on your heel test is easy on good leg and a fight on poor leg.

      Can you manage nerve flossing/gliding? what has your primary care doctor told you in regards to the nerve?

      @@bashbackpain Yes I've done a fair bit of nerve flossing, but nothing has resolved the drop foot, which seems to be only a little better. I will get on a treadmill soon and have another high speed test, so will know more then. More generally the whole leg feels lazy. I'm in no pain or discomfort, that all cleared up a while ago. Primary care GP referred me to spinal assessment, which is nearly 2 month waiting list as I type. That would only be an x-ray, then another wait for months for MRI. Wondering if I just pay for an MRI next week.

    When you talk about what should stay in the body, you are referring to a buldge, if it’s a full herniation/extrusion either by surgery of you bodies immune system. That piece of disk is gone. It doesn’t go back in if you recover/heal your body removes it. I’m not advocating surgery I just thought it should be clarified

      Hi Mike, thanks for your input and yes i am generlising somewhat but there are actually 3 theories that are hypothosised. One is the inflammatory response by the body does bring about phagocytosis by the macrophages, the second is dehydration of the extruded disc material and the third is retraction back into the parent disc. It could be that the type of disc bulge/herniation present will dictate the preference of one of the suggested theories. It could also be that they do not all work in isolation but each have a part to play. One must also remember that some disc herniations/bulges do not reabsorb fully but the patient will become pain free and will have no more symptoms. Regaining a pain free active life should be the ultimate goal regardless of what the disc is doing :) I was paraphrasing what a surgeon said to me in that if you can avoid getting opened up be it for meniscus surgery, back surgery, shoulder surgergy etc then all's the better :) but of course there will be times when it is completely unavoidable and the best course of action.     Im pretty sure i herniated a disc in June and up until two weeks ago i was starting to see the light at the end of the tunnel and then bam! Pretty sure i reherniated it. Im starting to lose hope….

      Before i just laid down and eventually started to walk as tolerated and was doing so well until nov 14th. Im just laying down again as it’s just too painful. its improved slightly to where its not so agonizing to use the toilet.

      So you do not have any access to physiotherapy? Did you get a diagnosis this time around from a medical professional? You need to try and get some help so you can ascertain exactly what is going on

      @@bashbackpain at first i went to the er and they gave me meds but no mri and said to do physical therapy but i just wasn’t able to. Only reason i made it there was because my leg went numb after my boyfriend had to lift me up under my arms. After they got drugs in me the numbness went away except for my toes and the pain came back. eventually i became bedridden. I also haven’t seen a doctor in years so had no pcp. It sucks i was doing so well. All i did was stand up!

      What is your injury and when was it diagnosed? it is normal in the acute stages to be incapacitated until that initial inflammation calms down. It is also a time where you can have set backs as the body is in the acute stage so it is easier for you to hurt yourself again. I need more info before i can advise :)

      @bashbackpain injury was on 27th of May. Pain started the next day morning when I woke up. I could still walk with the pain. It got better every day but still pain. Then it was only when I stand up the pain was very intense. Then I can walk without pain. I kept going to work and I didn't even need pain killer. However, after those initial 2-3 weeks I started to get worse again Then did MRI it was herniated disc l5S1. It was big but it was going down not huge pressure on nerve. I didn't lose strength, no numbness. However walking causes so much pain and calf muscle cramps. I rested for 2 weeks now in bed with very little walking every day . I'm lost now as doctor said don't do anything that makes pain. Which means I stay in bed all the time? I see people talk about mckenzie but it starts to hurt just being on tummy in 5 seconds. Today is my birthday and I feel more miserable than ever

      @bashbackpain he said no point. Just rest. I went to physiotherapy anyway and the guy was super useless. He also said rest and showed me one trick is to push my legs with hands while laying on my back 🙃

Which part of the world are you located as this has to be the worst advice from a medical professional i have ever of before in my life. Please do share with us :) And how many sessions of physiotherapy did you have? What made you think he was useless? I must say your health system seems to be rather antiquated if this is the case. I am very curious now to hear where you are in the world??
