      What a kind, kind comment. Thank you so much, my dear. I miss practicing terribly. But, you know, I've discovered that I can actually help MORE women this way than I could in active practice.
      Plus, I would find it very, very difficult to put my patient in the hands of another surgeon. I loved doing surgery!

    You are wonderful. I have been a nurse for 30 years and yet knew very little about menopause. I would love to have access to a provider like you. It would be life changing for me.

    Dr . Barbie, this video is full of "light bulb" moments. I literally got it, the meat of the menopause is how if affects our bodies. I am one that thought, its these darn hot flashes that are plaguing me, yet it's SO much more! The substance of your delivery amazes me. Thank you for your props, for those of us that are visual learners, they help me! I love you!

    Very cute story about the meetings at your former hospital.
    Very informative video. Before I found you I never heard of managing your menopause though I was already doing it.
    It took me 4 years to find out through a lot of research and consultations that I needed to take Estradiol.
    But I cant believe how I was treated by my primary care doctor and others. He said it was too "radical" to take Estradiol.
    It took me 4 years to get to the point where I realized Estradiol was going to help after doctors basically telling me I was crazy to want it.
    Out of all of the diseases to me Alzheimers is the scary enough!
    I used to think those cancer risks stopped at menopause.
    My GYN only prescribes bio-identical hormones. I did not know there were any GPs left until I saw your videos.
    My primary care doctor is an internist, Rheumatologist. But he is the chairman of 3 hospitals and has no time at all and when I had a fracture he hooked me up with a orthopedic surgeon.
    But now with HRT it scares the mess out of him.
    But it is so hard to find one these days at least in NYC. NYC have a doctor shortage. So my GYN is more of a primary care provider since he does everything for me.
    My GYN does acupuncture and only prescribes -Bio- identical hormones for menopause. I want to stick with him as long as I can. I have been seeing him for 25 years. But he is older and doesn't use a computer. But I will stick with him as long as I can while living in NYC.
    We are hoping to move to Hawaii and if and when I would like to find a menopause specialist.
    Hawaii has a large older community. They have these menopause GYNs that have plastic surgeons and do all of this plastic surgery for menopausal women.
    They are not listed with that organization you mentioned.
    They get good ratings but I don't know who s doing the ratings. I assume you cant make an assessment without knowing exactly who I am talking about.
    From your description I think my GYN is a hybrid. He has won many awards for research and stuff and is an MD but also does acupuncture for fertility and menopause.
    Your video was very informative as usual and I am looking forward to seeing my GYN in December and I will give him your book. (I still have to buy it)

    I have my hormones tested every year. As soon as they start slipping I wan't to be juiced up with every deficient hormone that shows up on the panel. I shudder at the prospect of bone fractures, heart disease, dementia, dry and wrinkly skin, thinning hair and the plethora of all the other maladies and annoyances. Thanks so much for your informative and fun videos!

      You are so welcome. Remember, though, don't ignore your own intuition just because a lab report says your hormones are normal. When they first start changing in peri-menopause, the lab values may be normal even though you feel that something is definitely "off." Believe your own intuition more than you believe a lab test.

    Fabulous info thank you. How sad that in Ontario Canada their seems to be only 1 specialist listed on that website and she is 2 hours away. If only hot flashes were the problem…..it’s depressed mood, anxiety, irritability, lack of sex drive, not sleeping without taking melatonin and generally feeling like I’m no longer me that has brought me to your channel. Will be contacting my family Dr to see who she might be able to refer me too as 3 years of feeling like this so far is enough. Thanks to you I will be more informed. Also just ordered your book. 😊

      Yes, the problem with these TH-cam videos is that you're only getting a tiny little sliver of information with each video. The whole education will take about a thousand videos!
      That's why the seminar is so great. In just two days you get the entire eduction to use for the rest of your life. And it's so fun and hands-on that you never forget it.
      I worry about women who watch a video or two, or even 50, and think they have the whole education. Nothing could be farther from the truth.
      I always encourage women to use a GYNECOLOGIST for menopause. They are the only physicians who have more than the hour of menopause education we get in medical school. It's the "tinker of all trades, master of none" phenomenon if you try to manage your menopause with a doctor who doesn't know much about menopause.

      I'm not as familiar with the resources for finding a menopausal specialist in Europe. But I know you have them. There's an organization called International Menopause Society (imsociety.org) that might have information on this. Go to the website and contact them. Just ask, "Are there gynecologists who specialize in menopause in Europe? If so, how can I find one?"

    Hello Dr. Taylor! Thank you for the education you're providing. It's all starting to click. I wrote you also under the last video. I will be going to my gyn Monday to discuss my options. I was hoping, though, that you could clear something up for me. Is oral prometrium both bio-identical and synthetic? If so, how does it do for protecting the uterus, since the synthetics work better for this than the bio-identicals? It sounds like it might be the option for me, in addition to estrogen, since I have a uterus but I have had endometriosis. The idea of it helping me sleep sounds terrific. Thank you! P.S. My husband now knows who you are as he is constantly hearing you as I watch and re-watch your videos! I love it!

      Yes, oral Prometrium is both bioidentical and synthetic. It consists of Progesterone USP (bioidentical) and it's produced by a pharmaceutical company (synthetic), and is FDA approved. It's really the best of both worlds.
      In terms of protecting your uterus, you should have nothing to worry about. Because it's synthetic, the dosage will always be consistent.
      You sure are doing an impressive job of understanding. I'm so proud of you!

      Thank you! It's your doing, you know! Before these videos, I was clueless. At my GYN appt today, after discussion, my dr. prescribed Minivelle .1 mg dose and Prometrium 200 mg daily. I figured it would be 100 mg not 200 mg but maybe because the estrogen is the highest dose? I didn't want to question my dr. about the dose because I thought it might be overstepping my bounds. If it is too high a dose, do you have any suggestions for what I should look for? BTW, my dad was an OB/GYN; unfortunately he passed away many years ago. It takes a special person to practice OB/GYN!

    This got me thinking to when I had an appt with my gynecologist I was in my early 50s I asked her what she thought of bioidentical she threw her hands up and said oh I don’t believe in it it’s just a money making scam. Well I let it go but I’ve always been interested in it. So I went thru menopause without taking any meds be it over the counter or prescription. I wish you were around back when I was peri menopausal. Oh well....thank you for educating us.

    Wow, I'm so glad I found your channel. I have the typical story of suffering for the past 5 years with perimenopause and then menopause symptoms, and I'm 53 now. I have spent thousands of dollars on integrative docs and chiropractors that sell supplements with not much relief. Keto diet worked a little for my symptoms, my family doc just wanted me to go on regular hrt, I tried biodenticals, but didn't have an ob gyn that knew much, but she tried to help me work with a compounding pharmacist, but I felt lost.
    This video is so correct and so was the last one I just watched about not using a full dose or recipe, that video fits me exactly because I just found a new M.D who doesn't deliver babies anymore, and now specializes in menopausal issues. I have a new biodentical RX for estrogen, testosterone, progesterone, and I'm afraid to take it, and I just stare at it while I suffer with severe hot flashes, night sweats, racing heart, etc....

      There's a number after each video title. If you start with video 1 and watch my videos in order. I can assure you that you will understand absolutely everything. Without an education , you're shooting in the dark.
      The problem with trying to get a quick answer from anyone is that you don't know the alphabet. Yet, you're trying to read.
      I start this series assuming you know nothing. And guess what: You'll be shocked at what you don't know. (You don't know what you don't know!)
      I can tell by your comment that you are, quite literally, all over the place. I can help you. You do NOT need to suffer. But, you do deserve a pro[er education. You will be a menopausal woman for the rest of your life. Why would you cheat yourself out of an education on the rest of your life?
      Jumping around in no specific order will definitely confuse you.

    Thanks to you and your videos I was educated and prepared to speak to my healthcare provider. I have now been on .375 estradiol patch and 100mg prometrium for almost 4 months. I have noticed a big reduction of hot flashes and over just feel better overall. Some nights there is a slight difference in my sleep but for the most part I still wake during the night and have trouble falling back asleep. I was wondering if you feel my dosage is sufficient. I remember hearing you mention that there was a dosage to reap all the health benefits of HRT. Can you please let me know. Your information and what you are doing for women is spectacular.

      You are on half the estrogen (Estradiol patch) dose necessary to prevent heart attack, osteoporosis, and Alzheimer's Disease. Those things are way more important than hot flashes, night sweats, or insomnia. But it does explain why you aren't sleeping well, too.
      As for the Prometrium, it depends on how you're taking it.
      If you take it in a "cyclic" fashion (only a few days per month), you are on half the regular dose.
      If you take it in a "continuous" fashion (regularly throughout the month), you are on the proper dosage.
      Remember also that progesterone helps with sleep. If you take it at night, it will help your sleep more than if you take it in the morning.

    The best cure for hot flashes is called portable fan. I never had it but would treat it that way. I’m more worried about other more serious issues of that phase.

    This video was perfect for me. It all just clicked for me. 😁 I am thinking about going to a specialist in menopause for sure after my vaginal vault prolapse. I see an oobgyn but she also delivers babies. She started me off on HRT but was not able to explain why I was still having other menopausal issues. It makes so much sense now as to why. Thank you.     Found a doctor on NAMS & she says lowest dose of hrt for the shortest time😳 CRAZY - I'm 55 postmenopause, have every symptom😢 Given 0.025 estradiol patch, asked for 0.075 turned down... So sad, we need more doctors like you❣️

      1) They joined a club.
      2) They took an exam based on the goofy guidelines.
      Just because they're a NAMS provider does not mean they will help you manage your menopause your way. It doesn't even mean they know a lot about menopause. And, since menopause is thee most neglected topic on Earth, they don't have to know much to be a NAMS provider.

    Dr., I have a quick question. Please help me. I'm on 0.075mg patch, but I'm still having terrible insomnia, but I don't want to increase the dosage. Can I use phytoestrogen OTC cream as a supplementary to the patch? Will it boost or cancel out the effect of etrodiol?

    I don't have one....gyno oncology did my ripping out in my girl parts. No regular gyno for me yet. There is a menopause clinic at our women's hospital. Sadly, there is an 18 month wait list to get an appointment....It's not like I went into it slowly, no, surgical menopause...

      In addressing which “diagnostic tests” to get for menopause, the first thing to do is to put things into perspective. Menopause is, quite literally, puberty in reverse. Puberty is when you begin your reproductive life. Menopause is when you end your reproductive life.
      So, did you have to get a diagnostic test to prove to yourself or to the world that you had entered puberty? Did you have to get a diagnostic test to prove that you had finished the transition through puberty? No. Your bodily changes, your moods, your relationship stress, and your psychological state were all so obviously due to puberty that you didn’t need any labs to tell you what was happening.
      Menopause is no different. You, and everyone around you is going to know that something’s happening. Like puberty, you don’t transition through peri-menopause into post-menopause overnight. It’s a transition that takes time - 2 - 10 years for some women.
      And you don’t know the exact moment at which you finish the transition and become post-menopausal any more than you know the exact moment you finish puberty and become post-pubertal.
      Hormonal transitions are like a rollercoaster. Multiple hormones change gradually. But the process is not predictable or logical. It’s chaotic. That’s why your life is chaotic during these transitions.
      From an academic standpoint, we neatly divide menopause into phases: Peri-menopause and post-menopause.
      Pre-menopause is before any transition into menopause occurs. It’s the time when you are having regular menstrual cycles.
      Peri-menopause is the time during the transition. It is due solely to low progesterone. And it is characterized by wacky periods. Over time, they get father apart and you skip some. It can last for 2 - 10 years, during which time you can still get pregnant.
      Post-menopause is when you have gone for 12 consecutive months without a period. It is due to loss of estrogen. All the symptoms of menopause are indicative of an estrogen deficiency state. Although estrogen is the hormone your body has depended on for just about everything, you lose it. This happens because we are living long enough to outlive our ovaries.
      Two years after post-menopause begins, your testosterone drops. This is associated with a decrease in sex drive.
      And, if humans were textbooks or robots, the phases would be perfectly defined. But we aren’t textbooks or robots. Our lives are a combination of many things that no hormone can measure.
      So, although it may be interesting to “diagnose” your peri- or post-menopause, you certainly do not have to do so.
      If you do want to get lab testing for hormones, here are the standard diagnostic tests:
      A blood FSH (Follicle Stimulating Hormone) is the most accurate test to diagnose post-menopause. The normal pre-menopausal level is 2 - 15 mIU/ml. At the time of post-menopause it skyrockets to greater than 30 mIU/ml and remains high forever. This is the test used in the traditional medical community by M.D.s.
      The problem with FSH is that during peri-menopause, your FSH is a rollercoaster. One day it’s high. The next it’s low. Up and down, up and down it goes. It’s as unstable as everything else in your life.
      But, if you tell your doctor that you’re suffering from symptoms of menopause, and that you’re unable to cope, but your FSH is normal, you could put yourself in an awkward positon. What if the doctor says that your FSH is normal and shoves you out the door? This happens all the time.
      Your blood test is but a snapshot in time. It is not representative of your life. It does not show the rollercoaster that is creating havoc in your life.
      If you have a provider in the Alternative & Complementary community (Osteopath, Naturopath), they’re more likely to test your saliva for estrogen, progesterone and testosterone. It’s a more convenient, but much less accurate test. And all the same problems with believing you versus believing the test apply.
      So, there are diagnostic tests for menopause, but you do not have to rely on them.

    So here in the UK does my GP need to refer me to a menopause professional , or is there a council here in England that would know ? Unfortunately in this country you can't shop for the best , everything is controlled by your GP. unless you have mega cash to see someone private medically in Harley Street. Know what I mean ? Xxxxx

      If your GP controls everything, then you would need to get your GP to refer you to a menopause specialist. There's an association called "International Menopause Society." The website is imsociety.org. Contact them and ask if they have a way for you to find menopause specialists. They're based in the UK.

    There's only one actual provider in my town. Spokane, WA. She doesn't take my insurance. There are two listed as pharmicists though. What does that mean? Do they just do the meds or can I see them to help me figure out what meds I need? My insurance... through the Marketplace, doesn't help much. Many providers in my area have backed out. That's another issue though. I know.

      No pharmacist is a provider. Pharmacists only know how to dispense medications, warn of interactions between medications, and carry out the physician's orders. Be very wary of a pharmacist who tries to give you medical advice.
      You're in a bit of a pickle if there are no "providers." Tell me, is there an Internal Medicine doctor, a Family Practitioner, or a General Practitioner?
      Physicians are so miserable these days. They only want to help their patients, but they are under stiff constraints and regulations by both insurance companies (who are the ones calling all the shots) and governmental regulations. Most patients have no idea how difficult it is for physicians. Instead of a two-way relationship between you and your doctor, there's a three-way relationship between you, your doctor, and your insurance company. Patients tend to blame this on the physicians, but it is not their doing at all.
      Can you believe that insurance companies, with absolutely NO medical knowledge at all, are the ones telling your physician what you can and cannot have? It's terrible.
      Of course, if people were willing to pay physicians themselves rather than rely on the insurance company for payment, they could have that one-on-one relationship. But most people won't so that. It's odd because people buy Alternative & Complementary treatments with their own money all the time. Most insurance companies won't pay for them. And it's a multi-billion dollar business.

    I have a consult set with Dr. Taylor in January!!! Went to my PCP and she did progesterone and estradiol. I asked "do you need to know where I am in my cycle?" She said it doesn't matter. I'm day 12 (when she tested) and progesterone was 1.66 estrogen 123... She said all was fine and normal... Anyone know if this is correct?
