Since we're all sharing our stories.
My first PC death was in Tyranny of Dragons, specifically the early parts of Rise of Tiamat (incoming minor ToD *SPOILERS* in case you haven't played that storyline)
The entire campaign had a rough start with lots of players leaving and joining. By the time we had some consistency, our DM proposed we skipped most of Hoard of the Dragon Queen (levels 1-7) and jumped straight to Rise of Tiamat (levels 8-15). We didn't have full consistency until we went to the Sea of Moving Ice where, long story short, my cleric of Bahamut got killed by an adult white dragon inside an ice cave.
The longer (and hopefully entertaining) story: It was a lair action of intense cold that reduced me to to 0 hit points and on the round I rolled my first death save, my party managed to slay the dragon. I, however, rolled a 1 on my death save and even my +2 to all saves doesn't stop a natural 1 from causing two failed death saves. Without any healing left, the party were rushing towards me try to stabilize my cleric. No armor to slow him down, Erebas, the Dwarven barbarian, was the first to arrive. Sadly he did not manage to stabilize me and nobody else managed to get to me before I needed to make my next and possibly last death safe. With a +2, I only need to roll an 8 to buy me one more round... and I roll a 7... and the DM ends the session...
I did the math for that one. With a +2, you have a 1.75% chance of failing your three death saves in just two rounds.
After-match: The dutiful human cleric of Bahamut had died to one of Tiamat's chromatic offspring. The cleric could no longer aid in foiling the plans to bring back Tiamat, nor could he continue his own quest of resurrecting his master, a silver dragon who found our Cleric in an avalanche and whose ashes were partially stored within his holy symbol: a hollowed dragon tooth sealed with silver scale. No longer could he champion the ideals of Bahamut. No longer could he be a protector of the Sword Coast and his friends.
However, his party were eager to bring him back. JUST prior to venturing out to the Sea of Moving Ice, the cleric had taught his wizard friend and fellow party member some divine rituals, including the Gentle Repose spell. Despite a long journey back to Waterdeep, the power of magic and two copper coins covering his eyelids, the cleric was brought back to Waterdeep no worse than when he died. The party quickly sought the aid of Onthar Frume, their loyal friend and an esteemed Paladin of Torm. He was quick to offer his help in resurrecting the slain cleric. Still, resurrection magic is harder to use than other types of magic and would require the contributions of the party as well (Mercer style resurrection shill check). The party tried everything. The barbarian threatened him to return. The bard sung a lovely, yet melancholic tune about his heroic deeds. The wizard begged for the return of the most welcoming friend he never expected to meet after being transported to this world. The wizard also offered up a black dragon egg. This egg and product of chromatic kind, our dear cleric of Bahamut had long wanted to destroy. Still, the wizard convinced him to let it have a chance to be a dragon of good and proof that not all dragons are evil. The opposite belief being the reason why his village killed his master, the cleric agreed and tried to cleanse any evilness within the egg with a prayer. Whether the prayer ever worked, nobody would know as it disappeared during the resurrection ritual in a beam of light. When the light vanished, the human cleric was nowhere to be seen as well. His armor and other worn possessions, however, were still there. Furthermore, they were worn by somebody else. Not a human, but a scaly humanoid creature with horns, and an elongated snout. The creature looked like a dragonborn, but something was unique about his scales. Instead of being the usual metallic breeds, these scales glistened with beauty of platinum. The resurrection ritual had failed, but the just Bahamut had intervened and brought his cleric as his chosen champion and a platinum dragonborn, possibly the first platinum dragon to be seen by anyone currently alive on the Sword Coast.
I don't know if our DM decided to intervene just because she knew everyone wanted my character back and things would probably be easier that way as well OR if it was because my character had genuinely earned the respect of Bahamut. I can however tell you, to me me, it at least felt like the latter and it was honestly a super cool thing to have happen to the kind of character I was playing. It felt earned and it made me ecstatic.
I know it was a lengthy story and I know it wasn't a perma death. Still it was my first character death and a super memorable one largely because of the after-match. I thank anyone who bothered to read through all of this. Unfortunately, the tale of our dear cleric ended like most do... with the players and DM having to discontinue the campaign 2 episodes before the final confrontation with Tiamat xD

My Brother had two characters die really early on in Hoard of the dragon queen. He had one guy die in like the second encounter cuz he was stupid. And his second character Fent Duelian the fighter duelist died cause he ran into a sally port full of enemies (he had an AC of 18 so he was cocky but he got crit ). So far my normal monk is the only person who hasnt had a character die so we meme calling it the mumen rider anime cause its just a casual dude who hasnt died while all my buddies fancy characters like a bahamut worshipping dragon born warlock dies whilst my monk is doing stupid shit like mounting the flying blue dragon at the start. @@BirdCyclops but maximum fall damage is only 120 blud. And the average max height fall damage is 60
A fall from a great height is one of the most common hazards facing an adventurer. At the end of a fall, a creature takes 1d6 bludgeoningdamage for every 10 feet it fell, to a maximum of 20d6. The creature lands prone, unless it avoids taking damagefrom the fall.
With the average hit points of a giant wolf spider being 11 and a 4th level druid average hit points being around 30 you would have had a chance to survive if he roll the dice but if the DM rules you die then you die

Me, realising someone’s probably gonna die, decides that Tetris stands with his sword out, shouts to the party, “Get Out, I will hold him back” Classic Hero style and as our wizard tries to cast a spell, Fireball, rolls a Nat 1. The rest of the party runs as the cave collapses and my character holds his position. The Cave collapses killing me and the BBEG, and that was the end of the campaign.

In 5e, giant spiders usually have 26hp. Fourth level druids usually have 22hp (assuming average rolls and a +1 con modifier).
ALL creatures take fall damage up to a maximum of 20d6 (averaged to 70) when falling more than 200ft.
Makes things easier to run for the DM. That said, I would probably have allowed the web a chance to work. You can't cover the entire hole in web, but you can shoot a string of web at the wall and then have both of you smash into the wall taking fall damage appropriate to the number of feet you were from it + maybe the distance you had already fell. If you survive that damage and, if I felt really mean (no that likely), I would have both people make con save with a DC based on the damage taken and calculated the same way the DC for concentration checks are calculated, just to see if you could ignore the pain enough to hang in tight.
I hope that would make for a situation where it would be cool if it worked and understandable if it didn't. When you've already accepted the spider can catch up to a tiefling mid fall, I don't see how you would all of a sudden be a stickler for spiders not being able to use their web ability to at least somehow try and save themselves.
Then again, it's not my table and I don't know what style of play that table likes.

New guy to group made a chaotic evil character for his first dnd character and he had multiple occasions of murdering random npc's and doing nasty things to the body (ie hanging them with a noose made of their own intestines, WTF) we met a pack of wolves, I as a druid attempt to tame them and succeed, new guy kills all but one, I confront him and tell him to not kill animals (my character didn't know he killed people) then he murders me feeds my organs to the last wolf then kills the wolf. At first I thought that it was typical new player murder hobo behavior till this point cause the detail he went into with what exactly he did with my organs to feed it to the wolf and what he did with my tabaxis skin turning it into a coat and the process to do so. I never have and never will play with that psychopath again cause it felt like more than just murder hobo but him trying to live out his twisted fantasies, I mean I've made chaotic evil characters but they don't go as far as this guy.

My first character died because I rolled a one to loot a chest, I got a nat 1 so my dm rolled his dice to see what consequence would happen. He had gotten a nat 20 so he looks at his list to see what happens. The first thing he says is “oh no” he had explained that I had basically summoned the devil. Me and my group tried to run to the city to escape it. When we got their it had rolled an intimidate to control us. It had controlled me (lvl 1 ranger, so pretty much worthless) and someone else I can’t quite remember I just know it was one my teammates. Then someone else on my team rolled an intimidation to control the devil, he got a nat 20. We thought oh cool we r safe, we weren’t. The wizard tried to kill it with flaming hands, which broke the intimidation, but the devil was fire resistant. The whole town had been set on fire because of the devil repelled the fire everywhere. The dm trying to save us just said the devil disappears knowing it did its job. He however could not save us from the burning town, there are walls around the town so we couldn’t escape with going through the burning exit. Lucky for us there was a fountain that the fire wouldn’t Spread to. After we stood inside the fountain, one of my teammates was like “ high five we survived” and me naive I did so. He had flaming hand activated which exploded in my hand. He had also got a nat 20 which killed me. In short I died from a high five.

In my second-ever campaign the DM had us all hanging out in a tavern (as one does) and soon after we started, a cannonball came through the wall.
Lo and behold, the bard with -2 constitution (it was her first time, and this was a level 1 campaign) is the only one on the side of the tavern that imposed disadvantage on the check everyone was about to make, rather than advantage.
As we later found out, while this did knock her unconscious immediately before the first combat even started, the damage technically should have killed her outright. Our DM decided she didn't deserve to die for that, though, and she didn't make any death saves since we had a cleric with Spare the Dying.

I didn't handle my first dnd death very well.
My trash druid died in a ooze pit that the rest of the didn't see and jumped over said pit. My new character (another druid who was looking for her and drag her ass back to the druid enforcer organisation they were from) in that game would bully and physically threaten the party to bring trash druid back to life.
    The party did
I felt bad after and apologized to my group and explained that this was my first death and didn't know how to handle it. They understood. But 2 weeks after, (I missed due to work) the boss battle that killed half the party (the only healers in the group) and derailed the story .....
On the bright side, I'm not bothered by my characters dying

I remember how my first character died.
In that session, we found an old man in a cave, waiting for a letter. One of our clerics knew from a rumor that the one that would give him the letter would be rewarded. So instead of doing the entire quest, I hit him with a FIREBALL for a total of 36 damage. Instantly killing him. It only got crazier, my entire group, except for the Bard, turned on me. That same Bard was also attracted to my character and was gay. My character didn’t feel that way about him. So the Fighter/Rogue charged at me and I attacked him. The Elf Wizard tried to bring me to the authorities, I declined. The other Cleric (who also Multiclassed into Monk) was furious about this betrayal (his and my character where bros up until this point), which made it kinda sad. My character tried to justify his actions, but the group didn’t agree with his methods at all. So without warning, the Demon Lord Graz’zt showed up and asked me to be his servant. I agreed, as did the Bard (Graz’zt offered me as a reward). The group ran off except for the Cleric/Monk who told the others that he would never forgive himself if he didn’t fight till the bitter end. This turned into an utterly badass moment, because he was only Level 6 at this point. Graz’zt tried to use Crown of Madness on the Cleric/Monk and the Cleric/Monk BARELY SUCCEEDED his saving throw. Then he made an attack, almost killing the Cleric/Monk. My character’s turn was now. I missed with my cantrip. And described how frustrated he was at this point. So I then made my damage roll for Scorching Ray, and I rolled a *NATURAL 1.* I took 36 damage. With my arm blown to pieces and with only 6 health left, Graz’zt teleported to my character and stabbed him through the head, killing him. The Cleric/Monk multiclasser summoned his God, the God charged at the Demon Lord and both vanished. Now with only the Bard and the Cleric/Monk left, he wanted to kill him. He would’ve done so too if it wasn’t for his God telling him that redemption was possible. And so just knocked the Bard unconcious. Now I play as my Sorcerer’s sister Eldritch Knight. Who’s not at all like her brother. Now, I spoke out loud how awesome it would be if my Sorcerer was revived as one of Graz’zt’s minions, and the DM seemed to have liked that idea, as he told the others that the Demon Lord had taken my body with him. Which probably means that he’ll be a Miniboss of sone kind. Needless to say, it was the best session yet!

The Atheist Bear I remember something similar to that happening for one of my first characters, as in the character coming back as an enemy of the players part
I made Joseph Stalin as a Totem warrior barbarian for some reason and while other characters are describing what they do at the tavern, I describe him going to a street corner and trying to start a communist uprising in what is a town the size of Waterdeep with the inspiring leader feat (not how the perk works but it still sort of seemed reasonable enough for the DM). A few high rolls later and this has somehow worked, a large army of peasants stand behind me as I a march them towards the castle. The guards of course stop the large horde of peasants and crazy barbarian man and immediately ask them to leave or they will use force. Peasants back away but my character won’t back down and begins screaming about how he will slaughter all tyrants (bear in mind this was a normal monarchy that was in no way corrupt but this was a joke character after all.) Guards begin trying to stop him but he actually holds his own for a while, sweeping guards with his Greataxe off into the moat and throws one down the stairs and he takes quite a bit of damage. My character then dives backwards with his Greataxe behind him to move away from the guards while killing the guy behind him, which thanks to some really high rolls works somehow. Now that one of the men has just been skewered, dozens of guards surround my character and he quickly gets knocked down. He ends up in a sort of hospital recovering from his injuries. Other player character (supposed to be important noble in this town but is also a warlock) walks in and punches my character in the face, but it doesn’t really do a lot of damage because he’s a barbarian so it doesn’t knock my character out as intended. So you’ve just got this noble just beating up Joseph Stalin in hospital while guards have to remove him. My character then ends up in prison but after my army of 500 goats (yes, my army of 500 goats) storms the prison, they’re easily able to smash down the wall to my cell and I escape. I made a new character after this as he wasn’t seen for a while. Then, while the party are at another town, the door gets kicked open and he shows up (I got to control him which was a cool idea on the DM’s part.) Chaos insues as a level 20 barbarian fights a party of level 5s. Character charges the warlock and does a fair bit of damage, but my character starts to take damage so he has to call in his goat army which downs the wizard and eventually the two monks of the party eventually just punch him to death

My first character died by toxic dirt mold from a small villages well. They weren't even in the well anymore, they were in a field by themselves and wanted to look around in their backpack, and some of the mold got in the bag. When they opened the bag the mold shot out into their throat and slowly choked them out while the rest of the party was celebrating in a tavern.

I only have one character who "died" and it was because my friend who was DMing at the time really liked the deck of many things, but we were all new to tabletop roleplaying so we had no idea what that was, also we were playing Numenera but he put it in anyway by making this diety guy offer us potato chips that when you took a chip it would trigger in the same way as the deck of many things. Well my character, not knowing anything was up with the bag, took a chip and was transported to the elemental plane of air, which according to our GM "Is just air, so your character just falls endlessly until he dies,". I argued that that probably wasn't the case and now I know I was right, but he was the GM so I didn't push it.
It felt pretty stupid and I don't really play D&D with him anymore because he only plays in a way where he has fun and pays no mind to anyone else's fun, which usually results in him being a dick to everyone.
Either way, that's the only tabletop roleplaying death I've ever had.

So, my first ever d&d character has not died yet, but I want my character to die in an epic way. So, here’s the setting. It’s a big cliff and the bad guy is holding me at knifepoint threatening the other players.
“GIVE ME THE JOURNAL OR I KILL YOUR FRIEND!”
That sort of stuff. Knowing my party members, they would save me no matter how many crimes I commit (I’m a rogue). So before they save me, I would actually not let the bad guy have the journal because the bad guy would use it for bad things. So I would throw my valuable belongings to my party members, stab myself through the stomach, which would stab the bad guy too, then utter a final prayer to Mask, the god of thieves, then YEET MYSELF OFF THE CLIFF with the bad guy still skewered to me.
    That’s how I wanna go.

I've made that mistake in the past when I wanted to change I to a bat for a stealth mission, I was lvl 5. Changed into a mouse thinking I would just go unnoticed until I ran into a wolf that tried to eat me lol. I killed it with my mighty stick. Getting back on topic the cr1 Giant spider is the one I use to annoy my party members more than anything. They need to climb anything, I web 'em and carry them up it or just shoot flying party members out of the sky lol. I'm my defense they were asking for it.

Failing saving throws etc. Obviously you can also make other connections such as character classes and setting but that goes for a lot of fantasy media.

@@tmaxim2651 sure, but there's no set party cooperation or magic. I know its fun to say that D&D encompasses all stories, but not all forms of media fit D&D.

Elf-lord's Friar of the Meadowlands I would never say that D&D encompasses “all forms of stories” and GoT is the only thing I’ve watched that’s given me a D&D *feel* . No form of traditional media is anything actually like a D&D campaign, but specific parts of Game of Thrones (eg deaths) remind me very specifically of making a bad roll and getting myself into an unfortunate situation.

I believe my first character who met their end was my LE Drow Fighter in the Out of the Abyss Module. He died keeping the banshee (?) in the Dawnbringer dungeon busy while the rest of the party handled the ghosts. One of the party members picked-up my PC's skull and is hoping to have him revived at some point as thanks for buying the time needed to save everyone else.

My first character (from 5e) died within the first 10 minutes of the campaign.
It was the 1st quarter of my freshman year in highschool. After a couple weeks, I heard an announcement about the schools gaming club, being held in one of the science classrooms after school on a friday. Hoping to make some friends and play some games, I went. When I arrived, there were a couple different groups. One played Smash 4 on a projected screen, others played computer gams, and some scattered kids played various card and board games. The last group, however, sat by some cabinets pouring over some books. They were setting up for a D&D campaign- Horde of the Dragon queen. Having not played a true D&D campaign since I was a little kid, I pondered whether to ask to join for a while. Building up the courage, I went over to them. Luckily, they accepted me. Over the next week or so, I worked on making a character, though it took a while to get used to the system. After that, we began.
The party was made up of 4 people, myself included. Most details of the rest of the party escape me, so forgive me for the lack of info on certain characters. There was a gnomish druid, played by a sophomore, a gnomish ranger, played by a junior, a half-orc barbarian, played by a senior, and my character- Jared Palmer, dragonborn cleric. His whole backstory was he was a human cleric that had come into contact with a cursed relic that turned him into a dragonborn. Seeking help, he turned to the god of dragons, Bahamut, for guidance. Eventually, he was told to do great deeds in the name of the god, and Jared would be reverted to his true form. This came to be somewhat of a problem in the party, as the ranger had a vendetta against dragons for destroying her home and family, and therefore disliked my being there. The backstory of the others never came up, or at least I wasn't around for it.
When the game began, we were in a cart, riding towards the town of Greenest, the first area of the game. The senior was late, so her character wasn't with the group at this point. As we grew closer, we spotted smoke rising from the city, along with a dragon circling over top of it. We stopped the cart and began discussing what to do. As we did this, the senior (and her character) arrived. The half-orc came running down the road. She jumped into the cart and took hold of the reins, making the horses move as fast as possible. Everyone had to make Dex saves. The barbarian and ranger save easily, while the druid barely saves, clinging onto a rope being dragged by the cart. I, however, rolled rather poorly. I apparently fall out of the cart from the front and get run over. Some dice are rolled, and the wheels do enough damage to knock me below 0 hp. As it happens, I had set my Con as one of my lowest stats, thinking it was less important because it didn't have any skills attached. Not understanding the whole 'death saves' thing, I thought I died immediately. And when we did roll for the save, I still died! Only around 5-10 minutes of game time had passed, and one player had already died. We all had a good laugh for a few minutes, then continued on. The DM spared my character, rerolling the damage dice and having my character run after the cart. Many misadventures were had from then on, including cultists, vampires, accordions, drugs, and so on.
After my freshman year, the group fell apart. The senior went onto college, the then junior began focusing on college and came less and less, and the then sophomore did other things. We still played from time to time, but by junior year, the group lacked a schedule, only playing the occasional game. Still, thanks to them, I found my love for D&D, and have played in many games since then. I hope you all have enjoyed the story.
TL;DR: In my first 5e campaign, I died within the first 10 minutes because another player was late and I was too ignorant for my own good.

My first barbarian “half-orc”(he was a human with a cursed orc arm) died fighting a simic hybrid poisoner in a gladiator match. I just had to out DPS her with frenzy, but near the end I started to roll badly and she took away my halberd. Then my DM rolled a critical that brought me to 1 hit point from endless endurance. On my last hit I fell unconscious from its toxic skin and the simic put a spear into my back.

My first (and only, so far) PC death happened because I had a berserker axe, and I finally failed the wisdom save. I had like a +9 to wisdom saves and the DC was 15, so I had had this thing for MONTHS and nothing bad happened, and this time I managed to roll like a natural 3. I lost control and attacked someone in the party and one of my fellow party members restrained me, then we got hit by a big attack by the boss and I auto-failed the dex save because I was restrained. I took more than my max HP below zero so I was insta-killed. The Paladin revived me via a magic item that let him cast revivify a few levels earlier than they usually can, but it was a total shock.

I haven't had a character die yet in the year I've joined my Pathfinder group but the longest living character Mo died tragically falling from a height, getting revived, only to be crushed by the falling bad guy who died to. And since Klaus and Mo were good friends Klaus knee that Mo wanted his body to be returned to where he was raised, which was a underground kobold city. In which they asked the party to join their feast celebrating Mo's life just for the party to find out that Kobolds eat the dead. Yeah, it was... Something.
