The best "horror map" ive ever played is one that is supposed to be a haunted version of a default map. There was something around it that actually made me uncomfortable. I used to get the strangest feeling of fear when playing games that are usually multiplayer Like the feeling of being watched, except there's just another player on the map without you knowing I almost crapped myself after seeing a Terminator SNPC after a war setup that I had just wandering in the mirror room The guy just ran off by himself and managed to survive for HOURS
There's this one cool Fallout 4 mod called "South of the Sea" which adds in these cool underground tunnels that kinda remind me of Gm_theunderground. For context, the Glowing Sea from Fallout 4 is a highly irradiated area that takes up almost a quarter of the map. It is ground 0 for the nuke that destroyed Boston. The explosion caused the ground to turn into mud and sunk many of the buildings underground. What this mod does is allow you to explore these sunken buildings, connected by tunnels dug out by either the Children of Atom, or some wasteland creature. It is very scary as most of the enemies on the map are very tough and one mistake can easily lead to your death. On Survival mode, food and water are scarce with your only break being this large supermarket which is inhabited by a very powerful Deathclaw. Edit: I totally forgot to mention that there are actually multiple Deathclaws in the supermarket and they are special unique variants exclusive to this mod.
I know a series of maps called necrophobia. It’s divided into three parts. And there actually pretty good and reminded me of those early half life and half life 2 mods.
I've played tons of horror maps and no other thing gives me that same feeling as seeing either a jpeg with a loud noise or a t-posing stalker or charred corpse. Gmod horror maps are really an experience and probably the peak of human existence.

I like how every horror map is, use scariest models/enemies from good to Tpose jumpscare or just fighting/running from them. That or they just make a bunch of original assets that sometimes ruin the gameplay.

I'll try to explain why the dark room and mirror room and scary for those of you who don't understand it. Introduction: First the map itself adds to this feeling, not the 2 rooms. It's some kind of unfinished city. The best parallel I can draw is the Sunkin City from Wet Dry World in Super Mario 64. In fact, seeing a place like a city or town that usually has people and other life and alot of noise, now empty and dead silent is creepy. You see this sunken town wondering what happened here, where did they all go? Yet...the city is flooded too when you find it, which suggests it's residents died of drowning. How does this connect to GMOD? It's the same scenario except instead of a flooded town, it's an empty city Then there is the game itself, GMOD, the best parallel I can draw to this feeling is Single-Player Minecraft Worlds. You feel...alone, isolated, alienated, you're the only one of your kind, the only one with your abilites. Sure there are villagers...but, they're not like you, they don't mine, they don't build. The only thing that even resembles you are zombies, which only adds to the feeling that you feel like there were once more people like you. You find their structures, you find their work. Yet you don't find their builders. Which means one thing that (most likely) happened to them, they're all dead. (Finishing later)
16:20 The map doesn't give you directions on what to do... well, sounds appropriate to G-man to me, like when G-man drops you off at the start of HL2 and doesn't bother telling you anything about the combine or even where to go or do. lol
