The first version was more than 6 hours long (never released). The original Leone version was 4.5 hours long (also never released). The European version was about 3.5 hours long. And the US cinema version was something like 1.5 hours and a disaster (damn those producers to hell). Leone's daughters have been working for years to restore the 4.5 hours version, which premiered last year - yet, there are still 24 minutes missing due to problems with rights and ownership. Martin Scorcese has been supporting the process and is continuing to do so.
Actually, they had something like 8 to 10 hours of footage when they finished filming, and edited that down to about 6 hours and were thinking of cutting that into two 3 hour parts... But the production studio refused because of the failure of "1900" which failed commercially mainly due to the fact that it was in two parts, so they settled on one 4.5 hour long film which was sadly cut down to just short of 4 hours long for its showing at the Cannes film festival in 1984, where it was widely acclaimed. This 3.9 hour cut was Leone's decision, which was done to better appease the distributers. The movie became a disaster when these distributers had it cut at 2 hours and 19 minutes for theatres! Leone's children tried to screen a remastered version of the film at Cannes in its original run time (4.5 hours), but due to some legal complications only managed to show a 256 minute version, missing about 13 minutes, but is still the closest it has ever been to the original runtime that Leone wanted.
Well, 10 hours was the entire material, as I understand it. A bit much, maybe. I watched all the "deleted" scenes there are on youtube and they would not have made it into a better movie - I think. Some of the relations between the characters would have maybe been more obvious sooner, true, but I always liked the fact that there was a mystery about much of what was going on for much of the film. The "original" European version is a masterpiece which never ceases to amaze me. Best film ever.

And if the studio's aren't willing to do this, why don't fans reach out to some of these guys?
@Brendan Milburn WW1 footage in better condition? I highly doubt it. If you watch the behind the scenes of that documentary you can see to what extend they had to restore the footage, to the point that they had to create artificial frames. In the case of Once Upon a Time in America it's a matter of restoring the colours. We have set photo's and also different scenes from the film as reference material. Sure I'm not a huge fan of tampering with art. But if the movie was meant to be seen in full colour, you might as well try to replicate what it was like originally. It will most likely not be perfect, but it can be much better than what we have now. Sergio Leone didn't mean for the film to be seen in such a bad state. This may have been expensive in the past. But nowadays there is open source software, and some really talented non profit artists who can add colour to a black and white pucture. So I don't see a reason not to try. I'd do it myself if I had the time and skills to do so.
