  • Rescue crews returned to the Potomac River on Friday as they continued searching for victims of Wednesday night’s deadly midair collision between an American Airlines flight and a U.S. Army Black Hawk helicopter.
Officials said the operation is fully focused on recovery, with no survivors expected. All black boxes have been recovered, according to the NTSB. Officials said that based on the information they have gathered, they have not made any conclusion about the cause of the crash.
"There's been a lot of questions and discussion regarding manifest. Let me make this very clear. The NTSB will not, and has not released a manifest. We have not in our history, not in our past. We will not be in this accident," said National Transportation Safety Board (NTSB) Board Member Todd Inman during a Friday evening news conference.

@ads6-1.0 lol no. Now if you want information in the field I'm an expert on that's different... This guy may be good at other parts of his job, but holding press conferences is not a strength. He is too technical. He awkwardly requests affirmations from the guy on stage who clearly isn't prepared and acts solely as a yes man. He thanked everyone on the planet first as if it was most important. He doesn't have the temperament to handle a press room. They need someone confident, clear and concise.

Todd Inman is highly experienced, shows empathy for families and victims, and shows a level of technical sophistication and nuance required for such a challenging and charged investigation. The NTSB's final report, which we can expect in 10-20 months, will identify corrections to the many causes of this accident.

Watching from afar. Terrible tragedy. However, is it not insane that military helicopters are allowed to weave in and out of the air corridors commercial passenger planes? It's like allowing private cars on a racing circuit while the F1 race is on. Either the base is moved or they build a new airport somewhere else.

@Langward09 that is all static pressures. If you hadn't noticed they hit the water at 150mph that causes a lot of dynamic pressures and forces that can't be simulated. Same reason why watches that say water resistant to 100m don't always keep water out when subjected to breaking waves. Dynamic pressures aren't even over the surface they twist and distort in a chaotic way.

It’s challenging to reconcile the claim of the safest form of travel, against 100,000 NTSB reports, to say nothing of the “proto accidents” that have led up to this truly preventable event.
Based on the definition of a “system accident”, the level of predictably precludes this event from being an accident - it was purely a predictable surprise.
If we are going to quote James Reason’s Swiss Cheese Model, we need to accept what those holes in the cheese represent, and I submit to you that normalization of deviance and a fundamental shortcoming in the deconflcition practices will appear as all too reasonable to have identified and patched up months - nay, YEARS ago.
To that end, it’s comforting to call this an accident. But, just as Challenger and Columbia were organizationally-based, so, too, will this tragedy.
