Honestly I thought about how confusing it is as someone who doesn't play the games and I don't blame you for having all these questions LMAO but to answer those questions yeah people do live outside and there's still radiation but enough for humans to survive, the big robot suit is Power Armour and he's a ghoul because the radiation made him that way, most were killed during the nuke but some turned into sentient ghouls which expands their life and deforms them. That's the short of it all!
