I think you'd never be able to see a tiny pixel if they didn't have a larger more visible area like the green dot to work off of. As long as the player knows the center of it is where they gotta worry about, then they're good. But without anything around it to highlight that, it might be hard to notice otherwise

It's fairly intuitive once you realize that about 90% of bullets on screen are either for intimidation or for assisting shape recognition. In reality there are usually only about 5-10 entities on screen you actually care about and need to pay attention to at any given time.
The true final boss looks intense, but not CRAZY. It's almost as if the player is performing a really slow dance with it: not much movement is involved, but one misstep can mean death. I think the music also helps this.
The design of this boss is so meticulous. The music choice and decision to go slow for the final two phases was brave, but it works. It tests pretty much all skills needed for this game. Panic/fast evasion, slow and focused evasion with broad vision, and exploitation of bullet patterns in the last phase to realize where you need to be to survive (rotating along the outside edge of the boss's barrier on the opposite side of the large purple bullets). The music and art only work for it and provoke the intended reaction. The color choice for the bullets and player character allow you to avoid getting lost. It's one of THE bosses in video games of all time.

Queen Larsa makes Omega Flowey look like a joke! She is literally a billion times harder than Omega Flowey, she may not have the power to alter timelines, but she can unleash an insane bullet hell, anyone who can legitimately beat her is a god of top-down shooters, that itself is an accomplishment.
Well, actually, pretty much every REAL bullethell boss makes omega flowey look like a walk in the park. Undertale bosses tend to be difficult only because you have a larger hitbox than in most other bullethells

Reaction speed, pattern reading, memorization, patience, serenity while dealing with so many bullets... All of these things must come together and reach a whole new level to be able to take on this sort of challenge.
The amusing part is how the first.. 'phase' we'll say of this boss is a colossal pain in the ass, and the second form is, comparatively, far easier, as a reward of sorts.. then the second half of that fight starts and it immediately becomes moderately more oh-shit worthy. (Comparatively.) Still though fuck that noise, not chancing doing THAT in my life. There's 'good chance I might not pull this off' and then there's 'not a hope in fucking hell in the slightest'.
The 'true' boss has, on an immediate analysis anyways, far more predictable patterns. As a result, this would indeed make her easier. If you take note, her attacks in the gigantic super-bug form thing are mostly diagonal, and flying at varying degrees, while form 2 generally stays more horizontal and vertical, while flying at generally the same speed.. This makes form 2's attacks far more predictable and easier to gauge, which as a result, makes it easier to find safespots faster, as you can judge each axis individually instead of at the same time.
