I did not understand the nose-septum slicing thing until you explained it. My surgeon (not Alfi) had mentioned it but I didn't get it. Now I do and i can confirm the breathing is just on a whole different level. Life changing! Would love to see more of this type of content.
I’m getting my surgery done with Alfi on august 15th to treat facial pain and TMJ issues primarily. I was just there to get my scans done for custom plates. His office is awesome and I’m really excited to get the procedure done with him.
How did you get your scans done? I was supposed to get them done last week and was canceled because of the hurricane and now my surgery was pushed back from August to November.
@@christinal434 I actually have pretty well aligned teeth with a good arch, so I didn’t need to get braces for surgery luckily. Only have had my wisdom teeth removed.

Hey Ron, love your work! I read your comment that you will be having more interviews with other surgeons in the coming months. I will likely be going for jaw surgery in the coming year but suffer from a pre-existing TMJ issue, namely anteriorly dislocated discs. This is actually a quite common thing in the general population. It would be interesting to know how other surgeons handle this issue as the surgeon that I consulted has surprisingly no worry about this, but I am not quite sure. I definitely do not want to worsen the TMJ as this is akin to playing with fire. Maybe you could collect some questions from the community that you ask at the interviews. Thanks so much for this content.. I really needed this :)
I did a consultation with his side kick, Dr. Carrillo, back in February. Switching to BCBS insurance for 2025, that way I won't need to spend 50k out of pocket to get my MMAG done. I live in Houston, kudos on missing the hurricane.

Hey Ron. I'd love to knw your thoughts on the "hygiene hypothesis" of allergies, especially since allergies are one of the most common causes of blocked airways. If you aren't aware of the hygiene hypothesis, it's essentially the idea that food and seasonal allergies are a product of the overly-sterile environments of industrialized societies. Seasonal/food allergies are less prevalent in pre-industrial societies. The hygiene hypothesis asserts that exposure to common "allergens" in the first year of life can prevent/lessen the develop of an allergy to those substances as a child grows up by getting the immune system "used to" those substances. Babies in indsutrailized societies are often kept inside in homes that have filtered air systems and very clean homes, so when they get older and are suddenly exposed to things like pollen, animals, peanuts, etc., and start going outside more often/ entering environments besides the home, their immune systems overreact, resulting in allergies. There is a good amount of research backing this theory, and also many practical examples. For instance, peanut allergies are super common in the U.S., which leads many parents to avoid exposing their babies and young children to peanuts like the plague, and then when those kids end up actually trying peanuts, they have a severe reaction to them. However, in other countries where peanuts are a major part of the cuisine and babies are exposed to peanuts early and regularly, peanut allergies are almost unheard of. It also applies to things like pets--babies who are exposed to a dog or cat in the home from birth are less likely to develop an allergy to them compared to babies who don't grow up with a pet in the home and then get one when they're older. Breastfeeding is also considered prevenatitve because it exposes babies to a variety of microbes through the mother's breast milk. Even some major medical organizations have updated their guidelines now to reccomend the safe introduction of allergens at the age of 4-6 months. Of course, there are conflicting opinions about this topic, and some individuals develop allergies anyway. But research like the LEAP study (Learning about peanuts) seem quite promising. Would love to hear your thoughts about this or watch a video about your thoughts.

I had surgery with Dr Alfi in 2023. Was fortunate enough to get referred to a good orthodontist for Invisalign and then on to Dr Alfi after asking my dentist about overbite correction. It’s something I’ve been thinking about for many years and glad I finally did it

Dude, I know you’re busy with being a dad and such, but you should really try getting into dental school again. Apply to state schools in a state with multiple options. California and Texas both have 3+ options. California is more expensive to live in and the schools are more competitive, so I’d go for Texas. You can establish in-state residency in only 1 year in most states. Just change your drivers license the second you move there and a year from that date is when you can establish residency. Or just pay out of state for the first year and establish residency in the subsequent years. It’s never too late to follow your passion. I’m going to grad school now after graduating college four years ago. For you, you will enjoy dental school. You won’t just be trying to “get it done and over with” to then start your “real life.” Going to school for something you’re truly interested in IS “real life,” and every day of learning new things is rewarding.
The thing about segmental lefort over MARPE, does it improve the condyle position?, with MARPE you can control the expansion for a healthy condyle position, eliminating TMJD can the same happen with segmental lefort or SARPE cause that's usually dentoalveolar

Is the healing time with a DJS with Dr Alfi cause less swelling and quicker recovery since the surgery is 60-90 min as apposed to 6hrs? What has Dr Alfi said about the recovery time? Also, for patients out of state, would you say the hotel you stayed at was comfortable to be in if you were recovering from surgery? What is the average hotel stay time post surgery for DJS? I’m sure there are a lot of factors involved such as pre existing conditions.
Yes that aspect sucks. However I hear Dr Julian Wilson at Kaiser can possibly do it without pulling any teeth. It just makes it easier to do it without them. So you’d need to be 100% on JS if you’re gona do it
I had an online consult with dr alfi last year. He is accomodating and patient. At that time he told me his record for MMA was 47mins. Im from australia so will be a logistical nightmare but if i were in the states wouldnt think twice about proceeding with him. I would assume the patient you mentioned that went straight to surgery and avoided marpe, as well as others that do similar, would not reap the benefits of a widened maxilla.
Indeed, it may be the case that skipping the marpe first might leave some patients yearning for a bit more tongue space and nasal breathing gains, but it could also be the case that surgery is sufficient for many patients too, sans marpe. Especially for patients that want to save time and get their "jaw hacking" over with.

Hi mate! What are your thought on invisalign? Grinding plastic for 2 years 24/7 sounds awful to me, when I think of hormonal health. Also, what are your thoughts on badly positioned 8th teeth on the lower jaw? I don't want to remove them as I'm afraid that will badly affect my face structure in the long run. I'm 29.
I hate Invisalign for some of the reasons you describe and more. It causes me to clench and grind like a son of a gun. Then again, regular braces, BRIUS and every other teeth-moving system sucks too.
@@darthnihilus777 it went really well. Pretty big movements: 10mm lefort 1 with CCW, 13mm bsso at pogonion with 7 deg CCW, and a genio which i need to ask how much movement it was. Recovery kind of sucks. I’m still very swollen. I’m not recessed from my profile anymore. I was secretly hoping to have a “chad” face afterwards but I don’t think that will happen, and is just down to one’s original bones. And I think any more advancement would have been too much, likely giving me that “dog” or “crocodile” look that some Alfaro patients have. I definitely recommend a consult.

Just had a consultation with him two months ago. I unfortunately have to get my wisdom teeth removed prior to the surgery.

How much maxilla advancement did he do ? I feel like the maxillary advancement is trickier to determine compared to the lower.
It's different for every patient but Dr. Alfi is able to do larger advancements than average for sure as the custom plates make that possible.
