premium processing fee doesn't cover I-140 & biometric fee, you need to pay $700 for I-140, premium fee is separate $2500

You can only pay the fee from the local bank account from the US. So find someone there who can pay the fee on your behalf.

I think no successful applicant has yet got the visa stamped. However, several petitions have been approved and successful applicants have received a welcome letter from USCIS. I think when the priority date becomes current, then the successful applicants will be called up for an interview at the Visa Center (US Consulate/Embassy) in their respective cities. The visa officer, nonetheless, can waive the interview if his/she deems it pertinent but it is less likely to happen in Pakistan.
