It took 8 mins from a report of an under sight to it getting noted as an actionable item!! Imagine if it took a plumber 8 minutes to observe a leaking hose and deciding to rectifying it. Or an electrician 8 minutes to observe a short circuit and deciding to switch off the mains This is why govt sucks the big ones...they are not capable of a simple acknowledgement that there's a problem and make an immediate and final decision to fix it

Our government is too big, although I’ve no doubt many public servants do an excellent job. One of the respondents baulked at the idea of doing an audit, despite giving assurances of ensuring up-to-date information. This harks back to your question: who is responsible, and what processes are in place to ensure that information is up-to-date? The conclusion was somewhat wish-washy, but at least they were, for the most part, reasonably affable. It will be interesting to see what information is provided to you, in notice.
