- Dr. Simmons discusses the critical need for earlier detection of dental problems to mitigate future health challenges. He explains why people's mouths hold the secret to good sleep. He connects the importance of good sleep to better health later in life.
The lower jaw, or mandible, has a massive influence on our health and well-being. The mandible holds the tongue, and is part of the upper airway. When the jaw is small, it reduces the space for the tongue, crowds the back of the throat while asleep, and frequently leads to Obstructive Sleep Apnea. This seemingly minor situation of a small jaw increases our risk of heart disease, stroke, heartburn and a host of other health issues. When asleep, the tongue and muscles of the jaw typically relax and the back of the airway can become crowded. In many, the body attempts to prevent this blockage with actions such as, grinding-and-clenching teeth while asleep, keeping the airway open. This leads to tempormandibular joint dysfunction, pain and destruction of teeth. As a neurologist, I exam the jaw and teeth of patients, looking for signs of sleep apnea. Physicians working with dentists can screen patients with this jaw problem and initiate treatment early-on, even in kids, before years of suffering takes place. Jerald H. Simmons, M.D. is the Founding-Director of Comprehensive Sleep Medicine Associates, PA (CSMA) and the Sleep Education Consortium (SEC). He is Triple Board-Certified in Neurology, Epilepsy and Sleep Medicine. A graduate of Ohio State University, he trained in Neurology at Washington University, Sleep Medicine at Stanford University and Epilepsy at University of California. He began his professional career as Assistant Professor of Neurology at UCLA while co-directing the UCLA Sleep Disorders Center and directing the Clinical Neuro-physiology lab at Olive View UCLA. In 1999 he moved to Houston establishing his neurology / sleep disorders / epilepsy practice. He continues to conduct research studies relating to sleep disorders and developing methods to enhance the assessment and treatment of patients with sleep disturbances. His most recent research involves the relationship between Clenching / Sleep Bruxism / TMJ Disorder with Obstructive Sleep Apnea, as well as the relationship between ADHD and disturbances of sleep. He is currently extending his practice to provide assessment and treatment of patients who sustained concussions. Education has been an important aspect of his career, having trained medical students, residents and fellows in sleep medicine and then establishing the SEC as an outreach to educate the healthcare community. He has been instrumental in advancing collaborative care amongst different types of heath care professionals and is now leading a national effort to enhance the screening of children for sleep related breathing disorders so that early intervention can transform the lives of future generations. I work with clients for TMJ pain that have gone to him or his practice for treatment. We are grateful for the work he is doing!
Thank you for the great talk Dr. Simmons! As an orthodontist, I can say from personal experience having treated thousands of adults with Obstructive Sleep Apnea and TMJ Dysfunction, the problem starts very early; you are spot on. Numerous patients we see are retreatments needing implanted teeth to replace previous four bicuspid extractions after we complete our rehabilitation treatment. Orthodontists and pediatric dentists need to recognize this. In my book, The NEO Way, we recommend observation of the newborn baby verifying silent breathing while sleeping and treatment to increase maxilla to ideal can begin as early as age 3 or 4. Airway is King! and is our #1 Key to Facial Harmony. The traditional approach of waiting till later to straightening teeth can be very short sighted and cause many future health problems including early death. Some of us as orthodontists have been professing this for decades, but even today the mainstream traditional orthodontist is slow to accept responsibility for the result of the accepted traditional treatment. Just straightening teeth and not paying attention to the effect on the rest of the body is not in the best interest of our patients. The effect of not correcting a deficient maxilla can result in teeth removal with a traditional orthodontic treatment approach which can lead to a very bad future for the patient. Bravo Kent. I am a dentist in LA who focuses on this and I see very few orthodontists (only 1 that I know of) who is understanding the points you highlight.

I am 30 years old with obstructive sleep apnea and tmj disorder, GERD symptoms and I do have a small jaw
I plan to do mandibular advancement surgery to treat my sleep apnea and finally stop clenching my teeth at night and hopefully completely resolve my tmj disorder
I agree that this should be shouted from the mountain tops. I am a dentist that has been very well trained to treat this very thing. I see children as young as 1 year old to evaluate if these growth, development and function problems are starting to occur. "1 year old?" you might ask? Yes. I want to monitor these children and their growth. Statistically, 4 out of 4 children have these problems.

@@drdougortho because his peers are afraid of their entire profession being exposed for damaging millions of faces and children's health in the pursuit of straight teeth. Did you even listen to this talk? This man breezes over it but he mentions orthodontics pushes our Jaws back.
Patrick Zwally sweet baby Jesus. Because he said it then it’s fact? His whole talk was a run-on sentence telling a story about a fictitious kid with zero evidence cited. My bad, he did say our jaws have changed over hundreds of thousands of years. Yes, things tend to change over time. It’s called evolution. Any graduate level growth and development book shows how jaw growth occurs. The orthotropic crew are bastardizing given knowledge and making convenient, unfounded “truths” and feeding them to dentists who many have never learned this data to begin with.

@@omega373 No he really is not. Myofunctional therapy is only a thing in children where the jaws are still developing. In adults it can, at best, make the tongue less likely to slide backwards into your throat, which... is something, but far from actual treatment.

How would you get a child to comply with a sleep study and/or wear a CPAP at night? It seems impossible to get a toddler to do a sleep study.
