I think the game did pretty well with sticking to how the characters were historically. They didnt just analyze the novel. But they also used the Han book of Historical Records and other ancient historical documents to see how the people really were. DW Just exaggerated the characters a bit to make them stand out more.
Zhuge Liang was a masterful administrator, but he sucked at picking people for the job (Ma Su, lol) and without Liu Bei appointing people and running shit while Zhuge focuses on domestics and internal affairs, Shu fell apart. Zhuge Liang has also had very little military accomplishments, and Shu-Han's biggest achievements were spearheaded by the likes of Liu Bei, Fa Zheng, and Zhang Fei - among others. Zhuge Liang and Guan Yu earned their godly status due to their loyalty and Luo Guanzhong's rampant fangirlism. His military successes were either temporary, or a "successful" retreat. He antagonized people (Li Yan, Wei Yan), and over-estimated others, leading to disastrous defeat (Ma Su) - in military terms, Liu Bei was better, Fa Zheng was better, Zhang Fei was better, Ma Chao was better - and that's only counting the people in Shu-Han.
According to novel : Cao-Cao bad-bad-bad ! evil-evil-evil ! Liu Bei Super good-good-good like angel ! Fact is, Cao-Cao also respected by Wei people because his kindness towards his general, that's why Wei generals really loyal to him.
If their deaths weren't recorded how do they know when they died? Also Cao Cao died of a brain tumor. He could've been been cured of it but due to his suspicion he refused treatment...

Most likely to old age or they were slain in battle (which I doubt). Another example Gan Ning, we don't know how he died in real life, but we know the year. Unlike the game or Romance of the Three Kingdoms novel, Gan Ning was slain by Sha Moke at the battle of Yi Ling. And yes, Cao Cao died of a brain tumor.
