Using the tester is similar to open loop circuit used by the railways to run electric trains. If you wear completely insulated footwear you will see the tester will not light up. The household neutral is grounded either at the transformers near your house or at the nearest distribution station completing the circuit.
The amount of Current shock and one dying out of it is dependent both on the potential difference (Volts) and the current (Amps) and also which part of the body it passes. If it is thru heart death is definite unless someone use a defibrillator to start the heart immediately after the accident if it is household current. Lightening (minnal and idi)strikes are fatal because the voltage is several million. The American voltage supply is 110V and is less lethal than ours which is 230 V. Farmers get heavy shock and die because their high power pumps are 440V. Performing electrical work alone is never suggested for safety reasons.
