Treatment with Tocilizumab for COVID-19

  • In this lesson I take a look at a new promising medication being trialed in the US right now to potentially treat COVID-19. This medication is called Tocilizumab, which is an immunosuppressant interleukin-6 receptors inhibitor. I talk a little bit about this drug and its past, including past FDA approvals for other diseases, before going in to how exactly it is that this drug works.
    From there I talk about how these mechanisms of action might be beneficial in COVID-19. I reference a couple studies, one small, early on study from China, as well as a more recent much larger study from Italy, that one which showed a significant reduction in mortality.
    I finally finish out covering the dosing as well as the common side effects to be seen with this medication.

    Eddy, I am a certified professional coder. My job involves auditing and coding diagnoses for ED and Hospital in-patient encounters. I have just binge-watched all your covid related video and just want to say thank you. I know your videos are designed for critical care nurses but in my role validating documentation and assigning diagnosis codes to provider notes, I read HPIs, hospital course summaries, and progress notes. I review lab reports and imaging reports. It is incredibly useful to really understand the notes and not just depend on ICD 10 and CMS definitions of criteria for various diagnoses. It is not my job to question the medical decision making, just record it, but at a personal level, I want to understand the rational for various treatments. The Covid pandemic has delivered a kind of hospital course unlike any other acute disease I have followed. You have helped me to understand the evolution of the treatments applied to Covid as well as enlighten me in other areas that have caused me to wonder. I don't want to be a provider, but I think I would love medical school, just for the opportunity to consume the knowledge.

    ICU nurse here. I administered this for the first time the other night and it seemed to have an almost immediate benefit! My patient was maxed out on Levophed and we were considering a second pressor when I started the shift but by the end of the night, I'd titrated it down to 4 mcg/min. Core temp also came down from 104.1 to 98.4! This is very anecdotal, I know, but I'm still very excited to have seen such a sudden, positive change in just one shift.

      I have survived covid 19 because of this medication. Nurses said that I have improved after this injection. Now out of hospital to give a testimony 🙂

    My father is fighting currently. Hes on a bipap. Hes on his 5th day of remdesivir. Next step is this option. I'm praying it helps him. He is strong.

      My father passed away on Aug.28th. He didnt have covid any longer. He tested negative 5 days before he left us. He passed away due to a 2nd sepsis that his body contracted due to long term ventilator use. He was 21 days on a ventilator and 14 days on a bipap. He was such a strong fighter. I will miss and love him for all eternity.

      @@paug86 yeah, it helps calm inflammatory processes that cause RA damage. The hope was it could interrupt the runaway inflammatory processes of severe COVID. However, it just didn't show us improved patient outcomes and has been discontinued as a consideration for experimental treatment.

    It seems lower pf ratios, lengthier symptoms is very concerning. I guess it depends on what phase of the covid cascade the patients in when receiving a dose. Lowering inflammation vs lowering autoimmune system. If patient is already in ARDS, would it still be worth it? Or perhaps it could be more effective in those only needing nasal canula O2 with stronger immune system at the that time? Too many questions, but thats what research is for. I still think iv dexamethasone has played a major role in the treatment of covid19, possibly larger than remdesivir.

      Yeah a lot of great questions that I think we just don't know the answers to yet. I completely agree about dex, but even that, a drug we have used for so long, we don't know the perfect timing for it on when to give. So much of the treatment here is based on the timing I think and until we have more data to know for sure, its often going to be a judgement call based on experience and best guess based on condition.

      The million dollar question... from what I hear, if all goes perfectly, maybe late fall early winter? But even then at this point, I don't think it would be in enough quantity to be available for the general public at large. A LOT of factors and thinks we just don't know yet, so really this is just a best guess.

      Everyone has a covid story, friend positive within 3 days on full face mask 2 callopsed lungs, 40s fit non smoker close to ventilator coma then used this with double steroids lungs cleared up ,3 days later moved to normal ward oxygen levels normal.
      Now the slow recovery starts

    India just approved itolizumab which is a psoriasis drug and cd6 blocker. Even though the clinical trials had only 30 patients, the results were still very good that they approved this drug for moderate to late stage disease. Can you do a video on this but also on another drug that seems to be promising which is leronlimab. Thank you

      Any of the "mab" drugs are going to be IL-6R blockers. The basic premise is going to be the same as here with Tocilizumab. That said, as I mentioned in there, some only impact the soluble IL-6R while others only impact the membrane bound IL-6R. One of the nice things with Tocilizumab is that it blocks both.

    Moderna’s phase 1 vaccine trial results have been published in the New England journal of Medicine. Can you interpret the results for us lay people. What do the assay numbers mean? Particularly the PRNT assay data

    . Be on the lookout for fungal pneumonia if your patient takes us also potentially consider prophylactic treatment. Unfortunately I had to patients that took this medication and both followed the exact same clinical course developing fungal pneumonia.

    My father was on bipap for 12 days and from last 2 days they have intubated him and he on mechanical ventilation.. doctors are suggesting not to use actemra/tocilizumab.. please suggest something for his speedy recovery

    My father was given tocilizumab on friday. Today Monday. He showed litle improvement yesterday but today his oxygen requirement has gone up. Has tocilizumab worked or failed?

      @@ashmatharu159 We refused the 2nd dose, it didn’t really help. The cough came back, liver enzymes got worst. I talked to other doctors. Ivermectin is better. The tocilizumab is better given earlier stage. My brother continued on with ivermectin per the flccc protocol and is doing better. He is now out of ICU. There is no single medicine that will do the trick, it has to be multimodal approach. Please check the flccc website and their protocols.
