you need a better scan tool, one that can show live data. the "code reader" is not very reliable. and it looks like you have vacuum leaks after all, which is allowing un-metered air into the engine which affects the O2 sensors making the ecm think the engine is running lean causing the injectors to open too much then flooding cylinders causing all kinds of issues.
