    Dr. Gersch, in a future video could you please talk about any undesirable side effects (that you know of or that your patients may have mentioned to you) of HRT (estradiol in any form/progesterone in any form) and also who should NOT be using it and/or who is not a candidate for it? I guess I'm asking about _lesser known_ side effects that not very many of us might know about. I love this channel. Thank you!

      You need to more specific. The perservatives in the gels are not great. Estradiol will not harm you. Progesterone taking orally isn't recommended either. You insert it. I know in the States they use GMO peanut oil. Compound pharmacies over there would help with individual formulations.

      I've been on bioidentical HRT (estradiol patch & oral prometrium) for almost a year, and I feel better than I have in 5 years! To me, there are a lot more unknown side effects of menopause than HRT. Perimenopause brought on the usual hot flashes, weight gain, and sexual dysfunction, but also heart palpitations, joint pain, extreme fatigue, brain fog, complete lack of motivation, depression, decreased immune function (I was sick constantly!) Plus, my gums started receding, and my teeth became more brittle. I took synthetic HRT for a year and only got relief from the hot flashes but still had all the other symptoms. In fact, the fatigue and sexual side effects were worse on synthetic HRT. But with my current treatment, I feel young, strong and healthy again!

    I used to use patches but even on the highest patch wasn’t absorbing enough to even attain the minimal levels needs for the health benefits. I’ve been using Estradiol Cypionate injections 2x/week and it has been so much better. I use a tiny insulin needle and there is no pain at all. My levels have been brought up to nice healthy levels and I feel very consistent all the time, no surges or dips at all. I encourage everyone I know on HRT to try injections, unfortunately there are not many providers willing to prescribe them. I hope in the future injections will be more easily attainable as it has made life on HRT so much more pleasant.

      I don't think she realizes that many of us either absorb at first, then stop absorbing or just cannot get to high enough levels on the patch or the gels. I tried everything - bi-est, patches (highest dose), gels and oral - which shot my estrone up. I have finally switched to injections and, for me, it's the best option and I can barely feel the needle prick.

      I do e Val injections and ❤ them because they provide healthy levels. The patches never got me high enough. So sad that a renowned Dr doesn’t even speak on injections. The only way to get them is telemed because most doctors are decades behind

      46yo, full meno since 42yo. And I came to say the same thing. Patches never got anywhere close to E levels that were helpful. I'm struggling to find the right dose/frequency for my Estradiol cypionate inj. Can anyone share what is working for them and the E levels you maintain? Thank you!

      @ thanks for posting. The creams are not consistent for me. I'm now trying the patch and waiting for blood level results. I will move to injections next.

    Dr. Gersh, many of us do not absorb well transdermally. I've tried patches, gel, and spray and I couldn't get my estrogen levels up. I still was not sleeping thru the night, had brain fog + other symptoms. I now use Estradiol Cypionate injections 2x/week at home and I can sleep thru the night! I use a tiny insulin needle without pain. I have to use telehealth from a clinic in FL because no one in NYC will prescribe. It would be helpful if well-known menopause experts like you would become familiar with at home injections so that we could get more drs to prescribe them. There are many women like me that transdermal methods do not work well for.

      The dosing for the patches and gels was just too low. Only way is to get compounded creams with higher dosages or like you are doing with injections.

    This was a fabulous Utube & please please do more. I’m young surgical menopause. I do best on estradiol patches however blood tests confirmed I have to change my patch every other day as no matter what dose/brand patch by day 3 I have almost zero estradiol. I wish manufacturers would realize how fast patches drop & start making an every other day patch change so insurances would cover it. Please talk about progesterone & especially with no intact lady parts as I use it now because it helps me sleep good but many drs tell me I shouldn’t be using it. I also use testosterone cream.

      You need to try the gel... high doses of it... Don't take no for answer. Remember its a cyclical process..You then need Progesterone too. If you have no Estradiol that's a big problem. Your body will go into massive decline. Take VitD3 and VitK2 daily. Magnesium Glycinate at night. Omega 3. Creatine powder too. I'd recommend Pilates as excercises.

    When doctors deny women estrogen and other HRT, women like myself ( 54 and in perimenopause)have no other option but to seek relief and help from pellets. Do i love pellets? No, but they work better than the 3 different antidepressants that i was on all at one time. This all has to change! I know im not alone.

      Yea I don't understand how you are denied these hormones?? You are probably in menopause at 54. Just because you get a bleed doesn't mean you have any eggs left. You can bleed on low levels of Estradiol. Loads of practitioners give Estradiol and Progesterone. Tell them no you don't want SSRI's.

    i am on .05 patch and 100progesterone 14 days a month since last November. asking my doc to test my hormones next month. No bleeding (I would not mind)
    feeling definitely better than before, gained 8-10lb(I don't excersise, I know, I should) joint pain and overall aching is gone(inflammation, I assume)so far I am happy about HRT. More energy- what a gift! 57yo

      Clean up your gut... you need higher levels of hormones too. Magnesium Glycinate at night. VitD3 and VitK2 daily. Pilates.. strength training. Walking.. fasting. Pre biotics and Pro biotics. Creatine. Stretches..lymphatic drainage massage. Magnesium Threonate in the morning for cognitive issues.

    I watched the whole video with the hopes of learning more about injections. They are from my understanding, easier to adjust dose, can be done in shorter amounts throughout the week, every other day even if need be, allowing women the best control of dose. from everything i have heard they are not very painful at all.
    i think we neeed to hear much more about this. Not about ooh ouch a shot! but knowledge about metabolism, dosing, formulations, etc
    i really hope you can update this area

      It's much easier to control. I have a few clients doing injections and the levels are stable. But it's not common at all.

    Hi Dr. Gersch, I take Estradiol injections (Estradiol Valerate) 3 x's a week. I do rhythmic bio hacking myself so my doses vary from .16 ml to .30 ml depending on the week to mimic a fertile woman's cycle. I find it easy, painless and consistent labs. Please share that option for women....and believe me I am needle adverse :-).

    Hello Dr Gersh. I am a new patient and I will be meeting you in the beginning of February. I have been watching your videos for several months, and I am learning so much. During perimenipause, my PCP only wanted to give me antidepressants and he was against BHRT altogether. I am glad I found you on TH-cam. II am hoping you will do a follow-up video to this one with info on postmenopausal women, specificall who is/isn't a good candidate for physiological BHRT, the dosing for super-aging and any possible side effects. I have so many questions, probably too many for a first visit, so I am asking now for myself and others who may wish to know.

      No. That isn't Progesterone. That's a synthetic Progestin. Its acts as a hormone disrupter. I don't recommend it. You want Progesterone. And you need high levels of Estradiol = Estrogen 2. You will not get uterine cancer. Look after your gut health first and foremost. Be active and cut alcohol out for good.

      The IUD is synthetic progesterone called prgestin. It caused irregular cells in my breast and that is now a known side effect

    I live in France , and my doctor said 2 pumps of your gel is the maximum you can get for menaupose. If I have understood, we may sometimes need more ? I already have osteoposis.

    Dear Doctor, Thank you so much for all the precious information! Could you please tell with respect to the cream that can be used on face. All the cream products in UK are with estriol, tiny amount , there should be estradiol inside to use on face ?? And is it possible to combine small amount of gel where estradiol is with the face cream ? 🤔Thank you

      You can get 80:20 (Estriol/Estradiol)online. Do not put Estradiol gel on your face.. there is Ethanol in the formula. It won't do much..Estriol is weak..its called E3. Plumps and feeds tissues but wears off quickly. Its recommended for the vaginal area. You need Retinol high grade and L ascorbic acid. Also a high strength Glycolic and Pyric acid cleanser. Micro needling as well.

    Please do a video on progesterone! In the US bioidentical progesterone is only available in capsule. The only way to get bioidentical progesterone approved by the FDA is in a capsule form. It's very hard to find a provider in Massachusetts who would write a prescription for a progesterone cream to be made in a compounding pharmacy.😢

      What's the issue?? You insert 200mg vaginally every 2nd night for 2 weeks or thereabouts. If you need to up the dose to 300mg you do so. If you are able to take Progesterone nightly for two weeks do so. A lot of women can't do that.

    I love the Evamist spray! With the patch I wasn’t sleeping on the 3rd day. I hope I’m getting enough to prevent bone loss and heart disease. I definitely feel better using it. Please update any new info on Evamist if you can.

      Er, you hafta see an out-of-system provider who doesn't use insurance to achieve higher doses. But that gets too expensive with the patches. I'm going to switch to the gel she recommends and then check my levels at my obgyn without confessing where my levels come from. The system forces us to do such maneuvering! It will mean paying someone out-of-pocket for the first visit at least. Here in the States we are at a disadvantage compared with some other countries, in the menopause realm at least.

      My doctor prescribed .5 MG estradiol pill vaginally 2x/ week for one year for vaginal atrophy. Says that's the same effect as transdermal at lowest cost. I was so grateful I got him to up my dosage (from 1x/wk for 3 months) that I was not going to argue with him. He's by the book "lowest... shortest...." especially at my age of 68. I was forced to quit HRT 10 years ago when I was "bleeding like a teenager"... but happier and far healthier than I am now.

      She can't unless you are her patient. Every woman is so different. And of course the age and the womans health overall. You need high doses of Estradiol especially the younger you are. Progesterone needs to be tweaked to the woman. You don't want Progesterone down regulating Estradiol. Some women take it every second night and you insert it. You don't ever take it orally. And remember its cyclical not static. Again the womans age comed into it.

      Jessica, do you know that Dr. Gersh does in fact prescribe progesterone? I've taken her course and am following her lectures on TH-cam very closely-- whenever I am doing routine tasks that allow for that. I'm even doing a small log of my own experiences with her Advanced Protocol, so this lecture of hers is very helpful! We need to stick together in the menopause realm.​@jessicahitchens6926

      Your not post menopausal. Your menses have ceased. So your moon cycle is paused. No post that is your state until you die. Its essentially a deficiency in crucial life hormones. I'd recommend supplementing. It won't get better as you get into the 60s. Usually a decline after 61/62. Do your research. She has a back catalogue.

      What is that,please? I missed the boat on HRT. My primary care physician just wanted to put me on antidepressants when I was in perimenipause. That wasn't for me. No bio identical HRT either. He was against that. I don't think he kept up with the latest research. I got no info from him. Very old school. I would appreciate knowing about this.

    I wish you could talk about progestrone levels, I take 1mg estradiol patch and oral 10mg progestrone. I do not sleep and have weight gain. progestrone

      Protection of bone mineral density (or increasing it) depends on the serum estradiol levels achieved, not on what form you used to raise it. Suppression of bone loss is thought to occur when estradiol reaches 50-60 pg/ml. However there are studies (using patches, pellets, or gel) that showed significant gains in bone mineral density where subjects average estradiols were 100-120 pg/ml. There is also a wide range of individual differences (always some women still losing bone when most are gaining) in these studies. Which is why tracking bone turnover markers (in addition to estradiol levels) is really necessary if (like me) you are working to increase bone mass.

      @@cheriyost2780 Do you know what your serum estradiol is? Most doctors will tell you you’re in the normal (postmenopausal) range but they don’t say, “and that means you’re losing bone!” Unfortunately they are trained NOT to check blood levels and just do HRT for menopausal symptoms. I wish I was still just in the osteopenia range. If you do a patch ask for not less than 0.075 and better a 0.1 mg), then test your levels. I started this bHRT process 10 months ago with BiEst 50:50. After 6 months my E was only 27 pg/ml (I was started super low). I finally just started straight estradiol cream (compounded, 3 mg/d) and will test E next month.

      Progesterone packs the bone in. Estradiol helps with bone density. VitD3 and VitK2 daily. Strength training too.

    My gynecologist says the .5 mg estradiol pill inserted vaginally = transdermal delivery most cost effective with no clotting issues.

    can oestrogel be used on the face? i've heard you say no in other videos, but that is what is easiest for me to get. i want to use it to supplement a low level patch. the oestrogel is not the pump kind. It's 0.6 mg per gram and you apply 2.5 g gel per dose to the thighs or upper arms for 1.5 mg of estradiol. I would use a smaller amt than that since my main route would be the transdermal patch. I'm on a .05 patch. The gel has trolamine (looks safe) and a carbomer (safe according to the EWG) website. Thank you. Anyone else using the gel on their face?

      No... You only use Estriol and a smaller amount of Estradiol in a formulation. It plumps and feeds the tissues especially the vaginal area. It will not stop aging. It's not strong enough. Estriol is only 8% the strength of Estradiol. You need to have it internally and to incorporate a strict skin regimen. Invest in your products. The fibre blasts don't work as well without Estradiol. So collagen and elastin //hylauronic acid production falls off a cliff.

      @dancerjan46 it depends on your levels. The doctor took blood work to determine how much I needed. The cream also has testosterone in it and I am taking progesterone via a capsule at night. I know my estrogen level was low, but at almost 81 years old, I knew it was.

    My doc insists on saliva testing did hormones and insists I stay in the post menopause “ ranges” what can I show her to get her past the estrogen stroke “ risk” and her an optimal dose ?? She refuses to accept serum lab results ?? Please help i need data she don’t believe me !

    My Lenzetto spray is a 1.53 mg/dose, it feels strong because it does more than 2 1mg Sandrena packets. I feel it in my breasts more.

    Please more info on levels and rhythm or cyclic hormone therapy. Changed my doctor and it took a lot of research to find someone who will work with me to use hrt for health optimization.

    Thank you for your videos. I am on disability in Canada and do not have coverage for anything but oral hrt. I am on bijuva. 1 mg estradiol and 100 mg progesterone. Can I take this medication rectally to increase absorption and to avoid the high estrone levels I have. I would really appreciate a reply as no one has helped me regarding this question! Thank you!

      Are you sure you don't have coverage for patches or the gels? Even here in the US they are covered, on Medicare, at least.

    Okay I'm confused I you were against compounded creams??? I watched one of your videos and you were against Bi-Est creams from compounding pharmacies! Please explain? I use a Bi-Est 50/50 from my local compounding pharmacies.

      She wasn't against compounded creams. She just felt many women were getting bi est, and the docs were using just a small amount of estradiol with their estriol.

      The doctor’s issue with BiEst is its ESTRIOL component, not that it’s compounded. I just switched from BiEst 50:50 (which is half estriol and half estradiol) to straight compounded estradiol for exactly that reason (estriol competes with estradiol for the hormone receptors which diminishes the outcomes that you’re taking HRT for (at least in the case of bone).

      Progesterone and VitD3 for bones as well. Magnesium Glycinate and Malatate too. Strength training. A clean gut too.. Progesterone manufacturers the bones it packs it up. It also helps with skin too. Estradiol is the Queen bee though.

      I personally don't use injections anymore more as I found that really unpleasant and expensive. Plus non-physiologic doses. But I felt good tho was getting too low a level and the providers refused to let me raise it or take charge of my own dosing.

    I tried Bhrt, my hopes were high... all menopausal symptoms were gone in a day, but it worsened my asthma so badly and it made me so constipated I sadly had to stop! Nobody talks about the effects of hormone replacement on asthma 😞

      That's not the hormones. That's your GUT.. you clean out your gut first and foremost. Then slowly introduce hormonal supplementation. You tweak it to your needs. Look into why you suffer with Asthma too? Its all related... those inhalers have Acetone in them. Be aware of ingredients in anything including the hormones.
