I'm very surprised you didn't got right away the Doctor was joking during that whole section in 20:53. Go rewatch the scene again and observe his tone and how Bill reacts to what he said... He was just trying to comfort her and probably himself as well
I thought the episode was pretty good, although the messaging about capitalism could've been less on the nose. But I hate the bit about the Doctor being blinded! That felt like very old-fashioned television, for want of a better way of putting it, and it's a very feeble idea for a science-fiction series in which the main character can do all sorts of technical wizardry when it suits the writers.

One of the biggest difference is there's no aliens/supernatural. The big bad are human beings. Like in episode 3 or episode 2 (in a way)
