  • Consultant Shoulder & Elbow Surgeon Mr. Nick Ferran exaplins ulnar nerve subluxation also know as snapping ulna nerve. This condition causes symptoms similar to cubital tunnel syndrome.
    Cubital tunnel syndrome occurs at the elbow causes shooting pain down to the little and ring fingers of the hand, numbness or pins and needles in the ring and little finger of the hand, and can cause weakness in the hand and weak grip. This is caused by pressure on the ulnar nerve at the elbow in the cubital tunnel.
    Patients with ulnar nerve subluxation get similar symptoms but also experience a clicking sensation on the outside of the elbow as the ulnar nerve slips in and out of its groove.

    I had a very successful ulnar nerve transposition on my right elbow one year ago. I had ulnar nerve decompression a year prior, but 5 months post surgery my nerve started subluxating (if that's a word) constantly. Decided to have the transposition done and it feels great. I had the same issue, not as bad, on my left arm so I decided to have that fixed before it got worse. I'm not 3 weeks post op. Still working on getting full ROM back. I have some weird nerve sensations in my upper arm, which I assume will go away eventually. In my opinion, this is a very simple and mostly pain free surgery. My pain was never more than a 1 post surgery, unless I moved it or tried to bend it too much to do my hair. :)

      I’m in a lot of pain. I’m wondering if this could be it. The VA is my health care system. Suffered the injury 5 months ago don’t have a diagnosis. Was the pain you were feeling 10/10?

      @@juancarlosflores7376 I'd say my pain was a 7 or 8, but I have a pretty high pain tolerance.

    Why can't the Surgeons place the nerve back into the cubical tunnel instead of creating a separate path?.. Thanks for the video

      It’s not really a true tunnel. Most times the bone is flat at the back or the triceps is large and pushing the nerve off the bone. So leaving the nerve in place usually just means it slips again.

    I have had an on-going issue with my ulnar nerve for almost 2yrs now. I had cubital tunnel release surgery at the elbow a year ago. It hasn't fixed my issue. Still have tingling and chronic dull ache in that arm. Now, i have a constant snapping over my elbow, especially when upper arm is pulled back or pressure applied to it. I haven't seen any comments anywhere where other people describe this kind of snapping. It snaps the most when arm is raised above my head, like when washing my hair, or pressure put on upper arm. Its quite debilitating and uncomfortable.

      Worth getting it examined and repeat nerve conduction studies to see the state of recovery of the nerve.
      They can take a long time to settle down if the symptoms were severe pre-op.
      If the nerve is subluxing it may need a transposition operation to settle it down.

      Maybe what you have is NTOS thorasic outlet syndrome nerve compression in the thorasic region.

    Hi Nick, thanks for making this video. My partner has this issue and has been struggling for years so I'm trying to become educated on it. 2 years ago, he underwent ulnar nerve transposition surgery, but unfortunately his nerve has not stayed in the appropriate place and is now still moving back and forth when he bends his elbow. His pain has consistently gotten worst over the years. He would need another surgery to correct it again but he's scared of the risks so he hasn't done it. He works a desk job and works out at the gym lifting weights. He's planning to quit his job to take a break but he still plans to work out. Is lifting weights ok? I'm quite concerned and it's hard to get him to go see a doctor. Also, is there any desk setup that you recommend that could help when he uses the computer? He feels best when his arms are straight. Thanks a lot.

      Usually exercise is the problem not posture for this condition but if concerned employers should usually arrange an ergonomic assessment as part of health and safety regulations in the UK.

    Hi Dr Nick, great video and very informative. I have a question. I have ulnar subluxation in my left arm. I find it very difficult to get bicep contraction in my left bicep doing exercises like bicep curls, where as I have no problem getting contraction in my right arm. Is it safe to assume this is because of the subluxation?

      I don’t think the biceps can be explained by the subluxation. Ulnar nerve doesn’t supply biceps. So this is unlikely to be the cause.

      Im not Dr nick but i have the same issue and can say yes, i had ulnar nerve subluxation on my right arm and every time i lifted to get a full contraction would be hard because the nerve would slip in out of place each curl, you could also be doing it perfectly but because the ulnar nerve is slipping in and out as you curl blood is building up around ur nerve instead of ur bicep which is why u dont feel the contraction as much, i recently had surgery done and am looking to make a full recovery

    I also have this issue, I hear a snapping sound whenever i take dumbbells but I don't have any pain
    Can i do weight lifting exercises or should I consider surgery

      I don’t think snapping should equal surgery if you have no other symptoms. But I would recommend at least getting it checked out to see what exactly is snapping.

    Can cervical degenerative disc disease and other muscle issues cause this?

      Cervical disc problems can sometimes affect the nerve roots in your neck but this is a specific nerve problem at the elbow.
      In some patients (usually male weight lifters) an abnormally large triceps can push the nerve out of it groove or squash it against the bone causing similar symptoms.

      @@ShoulderLondonGreetings, I have an extremely well developed triceps, especially the long and medial head, but I also had a small injury when i was about 13 when I tried to do a muscle up and I felt my ulnar nerve slip. And since that happened, when I do triceps focused exercises like triceps pull down or close "grip" push ups, I feel tingling/pinching in the nerve, but it's only localised in the elbow and it doesn't travel down my forearm at all, and I don't always feel this sensation only in certain angels of the movement or when I fo higher weight on the pull downs. In a few weeks I will go to my doctor to have an X-ray made, but since your field of expertise is elbows and shoulders, do you think ill need the surgery or is there something like physical therapy that could help.

      @@hadrian2505 I think you need nerve conduction studies or EMG so they can know the state of the nerve. If the problem is something mechanical like the muscle pushing or squeezing the nerve then therapy is unlikely to help and it’s more likely to need surgery.

      @Hadrian hey your symptoms are the same as mine but I never had any injury nor did it happen suddenly during an exercise. I wonder, have you gotten better now and what treatment did you have ?

    Hey man I have this from when I was grade 5 I was play fighting with a grade 8 actual boxer and he slammed me to ground and landed on my arm and dislocated it. I then went to hospital and I don’t remember to much about what they said but all I know is I now have the ulnar nerve flipping back and forth over medial epicondyle. I’m now 17. Ever since I was like grade 7-8 I got into weight lifting and it’s been a big part of my life and now I can’t because I’m scared to workout the triceps. I went to my doc in January and I believe she referred me to an sports performance orthopaedic or a elbow specialist I kinda forgot but it’s now halfway through April and I’m so depressed man. Is there any way I can have a procedure done that allows me to lift as if I never had the issue before? Meaning like I can lift weights and have no issue when I bend my elbow during excercise. If you can please get back to me that would probably solve most of my depression man I also liked and subscribed

      Hey sorry sounds like a tough time. We done treat under 18s so all we can do is give suggestions and that’s limited by not knowing the details.
      I assume your orthopaedic surgeon will need to asses your arm and the nerve and work out why it is subluxing. Ok most cases subluxing can be solved by transposition as explained in the video. Most patients make an excellent recovery so you should be able to resume training once healed.

I been doing 1 arm laying down tricep extensions with 10lbs but I do feel the nerve flipping over the medial 3/4 of the time during excercise specifically the tricep and I been trying to figure out if there’s a way I can do a certain exercise for the triceps or a bug in the form to reduce tension on the nerve.

      @@monaa1989 every time the nerve skips you’re doing micro damage to it. It may be trial and error to find a triceps exercise that doesn’t cause it but that may be unlikely so you may have to lay off of the triceps exercises until it’s finished.

    Not sure if I have this - my elbows often crack/snap when I flex my wrist and extend my arm, but I almost never have any pain or discomfort associated with the movement; sometimes I do get an ache around the top of the forearm (sort of like tennis/golfer), but never from the cracking itself. What do you make of this?
    Any recommendations on which type of doctor I should see?

      I’d see a shoulder and elbow surgeon. Painless cracking or clicking usually isn’t an issue but if unsure it’s something usually quick to check out and reassure the patient.

      @@SliceofBread123 with ulnar nerve subluxation patients often get discomfort at the elbow and sometimes tingling in the little and ring finger in the hand due to the nerve irritation.

    Hi dr, i have a question if you dont mind.
    Can ulnar nerve compression or entrappment cause temporary subluxation until the compression or entrapment is resolved.
    Is it possible that the nerve is only subluxating because it's irritated/enlarged and will stop subluxating once the underlying issue(irritation or perhaps a tendon issue) is resolved?
    Or is it that once it starts subluxating, it never stops without surgery because once the issue presents itself its permanent?
    Basically, are there Ulnar nerve conditions that cause subluxation but once those conditions resolve, the subluxation goes away.

    Hi, I'm 15, my ulnar nerve started snapping after I fell on the ground during football practice, and the orthopedic says that they don't recommend it because I'm too young to do this surgery, and it really makes me stressed rn :( I'm not sure if I need to do the surgery; right now, it is just snapping with some soreness around the elbow, which I'm afraid will cause further nerve damage due to my being active. Should I do the surgery, or should I wait?

      @@trenediX I can feel it sometimes when I bench but most of the time I don’t feel it snap but sometimes I have some soreness or random pain around it

    Hey doctor can u pls help me . The ring finger in my left hand is twiching for 2days but I have noticed somethings . It also twiches when I touch my thumb with index and middle finger.and when I touch the right side edge of my elbow it also twiches.These are the things I have noticed till now.Can u pls tell me how wto get rid of it although the twitching is very little and painless it is a bit annoying and conserning for me

    Hello doctor I have struggled with this for years now, I am a very active person and developed this condition during Covid possible for lifting too heavy or from horrible working and sleeping posture. It used to be much worse a few years ago now it’s only a partial subluxation if I put a lot of pressure on my elbow in a pushing motion such as bench press or pushing up off the ground and whatnot… I wonder are there stretches and natural ways for me to heal this? I never had this condition before 22 years old and I used to do a lot of crazy lifts like 310 lbs on bench press and dips with 90 lbs hanging off of me. Now I can’t do any dips with just my body weight I can do maybe 10-15 pushups only until 90 degrees elbow bend until I feel a snap in the elbow over the medial epicondyle and I can’t even do 135lbs on bench anymore unless I do super wide grip so my elbows don’t bend too tight - any recommendations other than surgery sir? PLEASE!!

      @@66boram there isn’t a reliable non-operative treatment for this that I know of as it’s a mechanical problem.

      @@ShoulderLondon do you have any papers or resources you can recommend me to read so I can understand how the mechanics work to cause this issue. I’ve weightlifted for the past 12 years with no issues at all until the last 2 years. There was no specific trauma or event I can recall that caused this as a result.

      @@66boramHey man i feel ur pain had the same issue, asked my doctor the same thing she said only thing i can do is therapy but that only eases the issue it doesn’t stop it from snapping, i did the surgery and can assure so far its worth it, its painless and fast and the sooner u do it the better i should make a full recovery within 7 weeks

      @@66boramThe mechanics part is basically what’s demonstrated in the video it supposed to pass through the back of the elbow but was dislocated upwards and is passing through the front thats the snapping sensation
