    Senator Patterson is very good at exposing the shortcomings of the public servicer. the public service is a joke. This woman gets paid a small fortune to fuck up every day

      You are so right, Senator Patterson is so great at exposing shortcomings and holding corrupt Senators/Ministers to account, and bringing the PS State/Federal and Senate to task towards honesty and transparency.
      I resigned from the PS (State) a few years ago. The wrong under qualified, ineffectual people of such self importance and absolute arrogance, are put in management/ hierarchy positions, and they are so inept in their daily dealings of their positions. It is nepotism and patronage in its highest forms. This is so dangerous on so many levels. Good employees are being muzzled and constrained, injured, and threatened by upper levels. The State and Federal Departments need bl..dy big enemas from the highest levels down. They need to get rid of the corrupt, deceitful, dishonest elements from the PM position down. Watching Senate in progress, some Minister's are so corrupt throwing their underling Senators under the bus to save themselves is so heinous. It's repugnant.

    I don't believe that a series of bad immigration decisions is due to the Minister's incompetence.

    Hey Senator, please chase up with the appropriate ppl why the oil & gas companies aren't paying their taxes and royalties! Our small business actually paid more in taxes than some of these companies that are taking our resources and making billion $ profits! The amount they owe could legitimately fix Australia's housing crisis, roads, hospitals, uni debts, prisons etc. Who are these companies paying off in Government and the ATO?

    The Dept of Home Affairs has a staff of around 13.5 - 14 thousand and a 9.5 billion dollar budget, yet cannot organise someone / anyone to keep an eye on what the AAT is up to..seriously!!

    If I make a mistake 1/1000th of this gravity- there disciplinary action, I will undoubtedly lose my job- and probably have my reputation tarnished for the rest of my career- in a job that affects only one patients safety at a time, not millions of peoples' safety. There really should be no wiggle room for mistakes in her position. It is unforgivable- and absolutely atrocious that it takes the studious prying that Senator Paterson undertakes in order to unearth it.

    Firstly congratulations Senator Patterson on your persistence and surgical like intellect- keep up the great work (and the pressure!)
    One of the basic tenets of introducing an organisational change - ie ministerial instruction #99 for both the minister and the secretary is to firstly assess and plan its implementation (things like resourcing impacts spring to mind - just saying) and then post implementation to monitor the outcome ie “is this change doing what we thought- if not why not etc” From the answers delivered it appears this was not done at all - management 101! Is this really the standard of the skill set of both the minister and the secretary??
    Additionally in any other workplace if a management failure directly contributed to the death of another individual there would be legal consequences for those failures. No doubt there is some legislative ‘ get out of jail free card’ that will prevent any real scrutiny to be applied.

    Without implicating anyone specific, I sense that the general culture appears to have lack the drive or being committed to the job, to do the job correctly the first time, and to keep tab of what when wrong. If this isn't done, there is no chance to remediate incompetence or unsatisfactory performance under a process of the continuous improvement cycle in pursuit towards high quality job performance.
    Not specifically referring to this case but from other Senate estimate hearings I get the impression that part of the PS in Canberra is getting bogged down and entangled in interdepartmental processes such that decisions and information transfer to ministers is impeded or paralyzed. The excuse that "we haven't got enough resources" reminds me of the typical union culture cost+ mentality always screaming for more resources instead of looking at opportunities to work more efficiently.
    This type of behaviour would not be tolerated in the private sector and if a department manager (or whatever) comes whinging to one's manage about not having enough resources, one will be told to just work more efficiently with the resources available.
    The culture in the private sector is that the job that needs to be done correctly and must be done on time and excuses such as what we see in these Senate estimate hearing are not acceptable.
    It the private sector such oversights are not excusable and it will bring into question as to one's competence to do the job according to the job description. Mishandling of one's job responsibilities and not producing the quality outcomes required will commence a staged warning level that will lead to dismissal if not fixed.
