It's most likely because, at times, the notes feel like they're 'dangling'. A stray note appears in a place where it shouldn't, albeit still sounding melodic, ofc. It's really good at inducing 'madness' into songs.

I have a theory that the leitmotif at 0:43 ans 1:23 is part of the Pianist’s song that made people turn into notes. The fact that so many battle themes in the game use this leitmotif and how it appears in both Roland and Angela’s theme makes it feel like all of these themes are connected in a metaphorical or poetic way.

Phase 1 [3 Dice] It's Roland and all the other librarians. This is your starting chance to deal true damage. It uses 2 pages to gain light before using 'Start Score'. Phase 2 [Locked Dice] All librarians are turned into 4 music notes. There are two variations, 'Flesh Quaver' and 'Flesh Crotchet'. Both are ranged, And cannot be moved. Quavers are sharp, Crotchets are blunt. One all four are down, Pianist uses a card reading 'Semibreve'. Phase 3 [6 Dice] The librarians are NOT returned. Roland fights alone, Pianist uses ranged sharp and slash damage. All of the pianist's cards relate to note names, 'Beam note', 'Double Sharp', 'Double Bar'. When at 50%, Pianist force ends the turn and uses 'Sixteenth Rest'. Phase 4 Roland has one card, Pianist uses one card, 'Timeless Finale'. Roland's card is 'Staccato'. You can choose to use it or not, But not using it will end in roland taking more and more damage. Letting roland lose ends in a short cutscene where he laments (Haha) letting this cycle of hate repeat. However, The card will let roland win. Using the card over and over, Roland will addess his anguish over what happened, Talking to noone. Once he finally accepts what happens, He uses 'Diminuendo', A 4 dice blunt card. It deals immense damage and instakills the pianist by the final dice. End Roland visits Angela, And admits that he cant get over any of it, But understands that it'll keep haunting him until he, too, dies.

그런데도 어째서 내 몸은 그 어느 때보다
온 힘을 다해 피아노를 연주할 수 있는가?
따위는 지금, 이 순간의 환희에 비하면 사소한 것이다.
멈추지 않은 채 빌어먹을 내 몸뚱이를 피아노에 두드리며 연주한다.
피아노는 진작에 망가졌어야 마땅함에도
모양새를 갖춘 채 오히려 더 커져 있다.
건반이 늘어가며 더 커진다.
늘어난 건반에 따라 내 팔은 늘어난다.
연주를 위 한 새로운 팔이 솟아난다.
음악은 더욱더 감미로우며 아름답게 울려 퍼져간다.
눈앞에 악보가 펼쳐진다.
사람들이 나와 같이 찢기며 음표가 되어간다.
해결사들이 검을 뽑는다.
하지만 이윽고 그들도 연주의 일부가 되어간다.
오선지 위에서 소리가 보인다.
비명소리, 살이 터지는 소리, 뼈가 부러지는 소리, 내장이 뽑히는
소리...
분명 소음일 터인데 나로 인해 선율이 되어간다.
점차 아름답게 연주되어간다.
이것이야말로 내 재능이다.
지하에 처박혀있던 나와 피아노는 하나가 되어 높은 무대로 향한다.
이제 누가 굽어보며 연주하고 있는가…
...이제 누가 올려다보며 선율에 몸을 떨고 있는가.
난 그저 도시에서 태어난 비루한 피아니스트일 뿐이다.
