Because of puffiness under my eyes that seemed to be getting worse over time and having seen your first video on this subject, I decided to return to my Dermotologist and ask to dissolve the filler that was injected over 5 yrs prior. Never would I have imagined that the filler was not only still there BUT it was the cause of my now bubbles under my eyes. After the procedure the bubbles were completely gone! I was in disbelief that the filler I assumed had long ago dissolved was not only still there but cause a disfigurement. I was putting off plastic surgery assuming that was my only option. Thank you for your original video (and this one) it saved me from plastic surgery. Needless to say I forwarded both to my dermatologist 😉.
What an amazing story! Thank you for sharing. But unfortunately, your story is not unique, and I have seen multiple patients in the same situation as you. It is one of the reasons why I stopped filling tear troughs and decided to make this video. Thank you!
Thank you for sharing your story. It sounds so similar to mine. I had the tear troughs filled before successfully with a small about of a Jevuderm Hyalaronic filler a few year ago and was quite happy with the result. As time went by and the filler naturally dissolved I went back to my dermatologist. She recommended a new filler called Ellanse (none hyaluronic) as it apparently last much longer than the Jevuderm. I trusted her and allowed her to inject. Immediately my eyes started burning and by the time I got home was eyes were so swollen I could barely see out of it. After icing and going back to her the next morning se injected something to dissolve some of it. Fortunately the swelling went down and but it left tiny little lumps in the corners of my eyes. It was not Immediately noticeable but I could notice it but lived with it for over a year. (The original dermatologist sadly passed away) and I found a new dermatologist whom I asked to help dissolve tiny lumps in corner of my eyes. After reinjecting celestone and hyalase to dissolve it, to my horror it made it worse. Those tiny little lumps seemed to have increased in size have now become big and puffy and has spread all the way down my tear troughs. Its so unsightly that I've lost all confidence looking people in the eye and wear specs to hide it. I consulted a plastic surgeon for his opinion and he said it could be corrected with surgery or by waiting for it to dissolve naturally. He also told me to massage the area daily to help speed along thr dissolving period. This has had mixed results as I found that in some instances it just made it even puffier. I felt like I was the only person in the world whom experienced this until I came across this video and thought my only option may be plastic surgery which is not the route I want to go.
@@viccosmeticinstitute thank you very much for replying to me. Yes, unfortunately that's the feedback most specialists have told me. I would never have allowed Ellanse to be used if I knew this information upfront sadly. I am based in Cape Town, South Africa and a local dermatologist did it.
You have single handed bought so much transparency in the industry. I now see so many high profile plastic surgeons acknowledge that fillers don’t dissolve but move. The same ones who used to say the body just absorbs it.
@@drluizpires If only it did. The research shows it does not. For the most part it just moves. Well known plastic surgeons are acknowledging this in the US.
@@Aceofcups111 hyaluronic acid is a sugar and is naturally produced by the body thus is PROVEN that the body have the ability to dissolve it. It does it every day. Every person is different. Some ppl are able to fight a common flu other don’t. We can’t say that ppl are unable to fight a common flu bc some ppl can’t fight it. Same with HA. Metabolism change from person to person thus some will be able to process HA much easier than others. The human genes are not the same for everyone thus some ppl will show complications and some won’t. This will be like this with anything. Some ppl even react badly to perfume and this is more common than ppl think. Well known plastic surgeon are acknowledging this bc it is true but is not an absolute truth. Some ppl show problems and others don’t. It is very difficult to know before hand if the filler will give you problems or not.
I couldn't agree more! I feel so lucky of being exposed to this kind of content… I wish all my loved ones could have access to this awareness before they do this kind of procedure and then make their own decision...
As an avid consumer of cosmetic procedures, I couldn't agree more. I have been in this field for 15 years and do understand the associated risks, so I would say I am well informed. It is rare though to see a level of transparency such as the one you exhibit. Thank you!
I'd say not admitting a mistake as much of a discovery. Pharmaceutical companies thier marketing plus FDA research and our depending on their approval says it only last 6 mo to 18mo. This Dr has discovered once again the big pharma companies are lacking truth. Thank you for the couriisity and persistence of this Doctor
Normalize unfilled faces. I’ve ended up with thyroid issues and thyroid eye disease after fillers. My face swells randomly. Filler lasts up to 10 years and floats. The filler hydrates and dehydrates for life. You will look different everyday depending on what you eat. Some days you’ll look empty, other days you’ll look deformed. Don’t do it.
Thoughtful Pro Absolutely right..also, few doctors know where to inject. They choose the easy option that is the nasal folds area and after 2 treatments the shape of your face has changed. I see so many women around followed the same procedure. Ban the Kardashians and promote self acceptance...
@@misstinatm how many times have you had them done? I had less than a mL and only done once I was happy with it but it pretty much disappeared after 4 months
Thank you for sharing your story with us all. You have been through so much with your tear troughs and hopefully your story will help prevent others from going through the same situation.
Hi Alexandra! I saw some of your older videos before you dissolved and I was wondering if you ever did dissolve. Would you please mind directing me to more information about your dissolving experience such as how many sessions, what brand of enzyme, how many units, etc? I am having the hardest time dissolving . Thank you!
@@viccosmeticinstitute Are the tear ducts and the veins near the eye connected to the sinus? I ask because I was on prescription eye drops and anytime I put them in I always tasted the eye drops in the back of my nose/sinus and it was unpleasant.
@@marsxSIN that is very normal! If you press near the inner corner of your eye for a minute or 2 after applying drops, it’ll help the eye to absorb the drops instead of running down the back of your throat 👍🏻
@Mani Bear💋 A true doctor will continue studying and training throughout their lives to keep up to date with current knowledge and medical breakthroughs. There are some who don't deserve the title that rely solely on what they were taught years ago in med school and believe that they know better than younger doctors with more up to date teaching. There is also a definite hierarchy in the medical field, and some doctors willfully ignore any mistakes they may make.
I love this. People admitting that they were doing something based on their education at that point but through experience have changed course of action. More practitioners should have this attitude! Don’t let pride get in the way of solutions, science is an ever-evolving process.
His honesty is shocking!. I learned to never trust plastic surgeons as I found the profession in general to be lacking of integrity. This is so refreshing.
from what i’ve heard (dad is a physician) i honestly believe a vast majority of doctors in all fields are pretty lacking in that area. obviously not every single doctor but generally, i’ve heard of (and seen myself) a lot of unnecessary procedures being offered and performed. disappointing for sure but general advice is get a second and third opinion before any procedure (at least that’s what i do myself)!
Thank you for this. I'm a guy and my tear troughs were a source of insecurity for me for a long time, but after watching this I will no longer be pursuing tear trough filler. Maybe my grandmother had a point by saying if it's not broke, don't fix it. There's nothing wrong with having tear troughs
From a woman’s point of View...........I would not even notice/ care abt your tear trough.........every face is so unique......I love crows feet on Men, lol.....to me , it shows a life of laughter & love & an active life.......Embrace them..........
Exactlly. You dont need it! I LOVE natural beauty. What people call flaws are actually beautiful to someone like me. I cant stand model/ mannequin/ robot looking " beauty". Its unattractive to me. Even alot of makeup and contouring is unattractive to me. A oersins soul and character is more appealing and makes someone attractive. Theres nothing unique or beautiful about a bunch of clone looking people with no unique flaws to love. Be you 💛💓 fuck the world and its idea of standard beauty. Its bullshit and unrealistic and screams shallow
I’ve had deep, dark hollowed tear troughs my whole life. I’m 25 and got tear trough filler for the first time 5 months ago. The doctor who did it said, “There’s literally no risk whatsoever.” 🙄 Ever since then, I haven’t been able to breathe out of my nose. Not joking. When I bend over, snot just pours out like a faucet. I’m constantly congested and with a runny nose. It makes it nearly impossible to sleep, exercise, and even speak comfortably. I’ve never had this problem in my life until the day of the procedure and every day since then. My under eyes are puffy and sore now every day when I wake up. They hurt often now due to any little thing-weather change, my period, whatever. Although they do look better aesthetically and it’s easier to put on makeup, I feel unhealthier than ever. It’s really impacted my quality of life. I want to get them dissolved, but I’ve also read nightmare stories about that as well. The hyaluronidase can break down your body’s own natural HA as well as the filler. I wish I had never gotten the filler in the first place. Learn from my mistakes y’all!! Stay safe!
This is a powerful life lesson. Anytime we are trying to hide, rather than accept, anything about ourselves...it will shift and cause symptoms elsewhere. The truth will come out eventually. To love oneself, wholly, and to reject the societal pressures...is to stop betraying oneself to fit a certain mold. Our self betrayal will always come out, whether in snot...or bad relationships..whatever. I wish you healing and wellmess
I have deep tear throughs and I always thought one day I want to fill them. However I had read already earlier of some concerns regarding this procedure. This changed my mind for good and I feel like I can start to accept them now. I'm glad there's more of a culture of natural looks and self-acceptance these days. Thank you so much for the video.
@@R1tu4lz Thank you. That's the kindest thing I've heard in a while. 🥺 This video was really a turning point for me to accept this part of me. I haven't thought of it since.
Just looking at the title makes me respect you. Doctors are meant to protect us, many doctors now r in for the money no matter how harmful the procedure is
About 20 years ago I saw a nurse who did injectables and asked about having the tear trough on one side of my face injected (I still only have an issue with one side). She said she wouldn't do it because the risks were too great. I was disappointed at the time, but after watching this, I feel really grateful that she made that call.
I love how this doctor is so honest about his experiences both negative and positive - and he's not afraid to say, "this just isn't working anymore''". So refreshing.
Learning about this makes me appreciate and love my natrual self more. I don't feel like I need to do these fillers ect anymore, thank you, this filler standard is just way too much.
I am a consultant dermatologist who does under eye fillers .. with the same baffling questions .. finally understood why today . Thank you - please have this published . It is one of the best explanatory videos ever seen . Continue the good work .
Oh wow, I was considering getting this done as my first cosmetic procedure ever (very soon, in fact), as my long tear troughs have always bothered me aesthetically since childhood. I guess this is a sign I should leave myself alone.
Its been a year since i am fighting the complications of filling this area with a filler, it took me more then 20 treatments with collagenase and hyaluronidase. I am now sending everyone your videos to educate. In Ukraine everybody does this and noone knows its gonna stay with you forever. Thank you for information
Could you get rid of the filler? Are you ok? I hope you are. I was botched with under eye filler and 11 months later I’m still weighing if I should get it dissolved or not :(
@@WickedRxqueen i look much better already, but still fight fibrosis and old fillers in whole face that migrated. Thanks to this video channel i found out my face has fillers still present. After doing multiple injections of collagenase+ hyaluronidase i am almost at the point where i can see my face that i had when i was 27. Last cpl years i had looked all puffy and botched. So dont be scared, its always a good plan to dissolve!
Alexandra Chemerichenko thanks darling! I hope everything turns out well for you and you can leave all this nightmare behind. Thanks for answering, it gives me hope. All the best for you, beautiful!☺️
20 treatments???? Be careful with dissolvers!!! They rip of your own body tissues that your body can't replace anymore and make you look older!!! There are horror-stories about that!
I had fillers in my tear trough 10 years ago and it was one of my biggest regrets. A short time (days) after I had the fillers I developed bags under my eyes that weren't there before. He dissolved the fillers but the damage had been done. I felt looked like a monster. I went to plastic surgeons and dermatologists for help but no one was able to and was advised to go back to the original dr who put in the fillers. I did find out from one that the fillers attracted water and that's probably what happened. I was 50 at the time and my skin was thinner and the fillers plumped up and stretched out the skin under my eyes. I've worn glasses since then and gained weight and the bags aren't as noticeable. I wish I had seen something like this before...actually, in all my research, I've never seen anyone address this issue. Good on you for doing so!
This couldn’t come at a more perfect time. I’m currently researching my options for under eye lids. I’ve had many cosmetic practitioners say that tear trough fullers are not an option. I’m relieved to hear I’ve been given sound advice. Thank you for been so honest in your approach and sharing your invaluable discovery & knowledge with us. 👍
Look into dermastamping/dermarolling (by a qualified professional!!) If you are looking to get rid of dark circles. I've used at home dermarolling before at slightly below where the cheekbones end and eyesockets start (high cheekbones and deep set eyes), and it has eliminated the darkness where I did it, making the remaining parts look more like fashionably upturned panda eyes instead of round dark blotches. I'm sure it can be used to clear up almost all of it if you get someone professional to do the riskier areas for you, like closer to underneath the lower lid glands and above the eyelid.
@@raven-kn6lv short answer: yes. Long: Both the dermarollers and dermastamping tools are the same, but in different styles. The roller is obviously a roller of spaced out needles, but the dermastamping one is like a blocky brush except instead of bristles it has needles spaced out on a patch. The needles can be anything from 0.2(5) to 2-3 mm, and the longer the thicker skin you need to use it on. But when you're getting something as sensitive as skin right next to the lids done it's really best you have someone else do it for you. The 0.2mm doesn't really do much on normal face and body skin, and is mainly for enhancing the penetration of serums and such. I imagine it could be used on the dark circle parts immediately around the eyelids, but it seems too risky.
I ain’t planning on getting any of these surgeries anyways, cause I’m broke and not healthy. I just subscribed to you for you honesty 👍🏻 You are a great doctor with a kind heart! More people should be like you. The world exist thanks to kind people. You are the best
Self acceptance is the moral of the story. Aging is a natural part of life. Eat healthy, exercise, stay out of the sun, wear sun screen ,keep your cells hydrated with fruits and vegetables. Keep your teeth healthy. Have your hair professionally done. Look your best when you leave your house. Smile and stay away from toxic people and places. Get proper sleep. This is all doable. Having fillers doesn't stop aging. Your cells will age around it and you can't keep chasing anti aging procedures. We are not meant to have the same features throughout the life span. No doctor or fillers can prevent aging. Work or accepting you in every stage of life. Live in gratitude especially if you are healthy. That's the biggest blessings to be healthy. Enjoy life, travel. We only pass through this world once. Make it a good one.
I am so enjoying finding plastic surgeons and dermatologist online lately who are so honest and transparent. This is so refreshing. I’ve never had any cosmetic procedures but suddenly I’m totally into these videos!
As an RN working in the OR for many many years I can tell you with authority that you guys are a beautiful breath of fresh air! I’ve never seen a humble surgeon...ever! You’re kind and willing to say that some product or techniques didn’t work as well as expected! Thank you so much for your kindness!🙏🏻🙏🏻🙏🏻
I love how this channel doesn't promote or demonize anything, just gives the facts and updates as more is discovered. It's great. I'm honestly much more likely to consider small plastic surgery or botox now, and less likely to desire fillers. I'm no stranger to body modifications, and I wish fillers weren't seemingly so widely treated as no more permanent and inconsequential than hair dye.
I haven't got fillers and I'm not planning to but I really love this channel as I am interested in the body's workings and understanding aging, including facial aging. I'm just so impressed with the knowledge and integrity of dr Chen... it's such a pity men like him are a small minority in the medical field.
The quality of this video is amazing. Pulling on other experts, explaining why you've changed your method and breaking down our anatomy in a way most of us can understand. I always have been nervous about getting that spot filled but genuinely thought about it. But now, I think I'll stick to my color corrector and concealer lol.
I've had dark circles and hollow undereyes since childhood. I'd been looking into tear trough filler for a couple years now, umming and ahing over getting the procedure as concealer doesn't hide the shadow caused. After this video I am not looking into it any more. Wow. I'm so thankful for your ability to share your experiences so truthfully. Seeing this kind of honest growth from a professional is lovely! Thank you so very much!
I’ve spent the last hour watching under eye filler videos and seriously considering doing it myself for dark circles. Thanks for creating and sharing this video because I absolutely will not be doing it / any fillers in my lifetime now.
It is absolutely outstanding to have a genuinely honest colleague who is able to admit his regret of a certain injection technique. I would like to thank you so much for your video and explanation. The injection industry seems so biased and money driven. Honest reviews and opinions like yours are very rare. Best regards from a plastic surgeon from Germany
I feel like lots of those cosmetic procedures haven’t been tested properly. There are so many new procedures all of the times and no real long-term experiences with it. What you’re doing is really important work. And it’s so great that you’re learning from your own experiences and changing treatments accordingly.
Im so glad you made this. I had this problem and I thought I was the only one experiencing the “post-filler dimples”. I was so insecure after getting the fillers, it was my biggest regret
I just wanna say THANK YOU for doing the right thing, despite knowing it may affect your business. A few years ago I watched the video you did on fillers and it is burned in my mind - I hate my lips but now I would never think about touching them. I'm starting law school soon and I hope to practice law with as much integrity as you practice medicine ❤️
Thank you so much for your integrity. I was seriously considering getting this done, but I’d rather accept my slightly hollow under eyes over a puffy look that will cause me major issues down the road.
Tear trough implants may be a good alternative. As of a few years ago, I located a physician down in Texas (believe it is Houston) who performed these. I want to say that each implant was customized to the patient. I.e. Using a putty-like substance, the staff would take an impression of the patients' tear troughs, and send off to a lab so that the implant was of the accurate size and thickness needed to properly fill the trough. Not sure whether this doctor still offers this surgery, but perhaps look into it. I can tell you from personal experience that tear trough fillers are NOT WORTH their exorbitant cost. I've done them twice and the results are barely noticeable, and they quickly dissolve. Very disappointing. This approach to correcting dark circles is nonsensical. It's also a money pit. Three trips to get filler and you've likely paid a good chunk of what it would cost for a permanent solution - either with trough implants or via undereye blepharoplasty.
I am so grateful for your wise words and I wish you were on more. I have been a holistic master esthetician for almost 4 decades and the issues I see from fillers, botox, lasers, and cosmetic procedures is quite honestly unbelievable. Hard spots, lumps, scars that didn’t heal properly and muscles that have stopped working. Most women cry in my chair because the procedures didn’t do what they expected and and or changed how they looked so dramatically ( not in a good way) that their self esteem was even worse. The fact you show so many young women is so disheartening. We need to love ourselves, we need to stop trying to change who we are. We need to change the story on what is beautiful. Aging is a gift and imperfections are normal. Leave your face alone and just take care of it. Blessings. 🍑🙌🏻🧡Lisa
Hi Lisa, I’m a big fan of yours, for some reason TH-cam unsubscribed me to your channel! I will address that. Do you have any natural remedies for reducing masseter muscles. I clench my jaw and it’s giving me a really square face ;( I don’t like it.

I have actually gotten tear-through fillers 3-4 times and I haven't seen any bad results and I was actually going to book a new appointment very soon. But then this video popped up and I'm so grateful that I have been educated in this matter and will NOT continue this treatment anymore and I'm so thankful that I haven't gotten any bad results
It is so refreshing to see someone this serious about an industry that is taken so lightly my not just patients but professionals as well. Thank you! I have not had any filler done yet, but have strongly considered it for a time. However, your content is helping me to make an educated decision.
So nice to see a good doctor telling you what you need to hear even when it doesn’t benefit him. In fact it takes away business and he still wants the best for us
Fillers are just a band aid solution to cosmetic issues. If a person wants natural, long lasting results, surgery is the way to go. It may seem more invasive from the outset but at least you won’t have any foreign substances floating in your body. However, the best thing IMO is just to leave it the hell alone.
@@augusto7681 I would get a free consult with a board certified plastic surgeon. That's what I did. Also, sometimes the cheek area can be filled and it helps the eyes.
Just got mine that was injected in 2007 dissolved yesterday! I can’t stop staring to my deep hallowed eyes now! I think it’s the most beautiful thing I have ever seen (comparing to the puffiness). I’m middle, eastern so that’s my natural anatomy. The younger me hated people thinking that I’m exhausted! Now I don’t care. Life is not about perfection, it’s trade off and I would take what my genes gave me over what I’ve caused to myself (You end up blaming yourself). In my defense, 17 years ago they told us that it will only last 3 months! I had to endorse it for 17 years! sigh!
i’m sicilian mixed with middle eastern and we have that too. that mediterranean look! it’s gorgeous. it took me a while to fully embrace it but when you remember that it’s a feature of your ancestry, you can wear it with pride. i don’t care what beauty standards say i love my people so i love my features. ❤️ love and blessings to you!
This guy is an example of how considered ethical practice should be run. He is the only doctor I have ever seen discussing and investigating the long term use and proper management of fillers I have seen anywhere. He is a top doctor. Forget those other doctors who would never look this deep into their work. Its all pretty basic really. And the fact that he is willing to admit he was wrong to do this is a testament to his ethical overview.
I totally agree with you. I either do a lower bleph or nothing at all. I’m also oculoplastic surgeon. My mentor actually stopped all fillers all together after a mild complication... he simply didn’t want the stress and was loving his surgeries anyway!! Great video and gutsy too !!
You have so much medical and professional integrity. Doctors are meant to help people not mislead them and perform unhelpful or even unsafe procedures to make money. You are a fantastic example of that. THANK YOU for the transparency and for educating us!
Wow the timing of this video is INSANE! I was literally considering getting my tear troughs filled but now I’m 100% turned away... what non-invasive alternatives would you recommend? Edit: sorry I need to clarify that I’m not looking to reverse or prevent the effects of aging. I’ve always had under eye bags and I’m only 23 so I don’t think just skincare would help. I was looking more into PRP possibly?
@bHoD gYaLLo As somebody who has hereditary eye hollowness and puffiness at the age of 22, who exercises 5 days a week, drinks enough water and uses retinol, vitamin C and SPF constantly for years, I can assure you that not everything lies in skincare. Some people have hereditary, aka genetic eye hollows, and skincare sadly will not fix it. Either you accept your differences from others or you fix it with surgery - I chose to accept myself and treat my skin nicely with skincare anyway, but it doesn't mean it's going to fix my "imperfections". It is what it is.
@@lehedonist I wholeheartedly agree. I also understood that I couldn't do much about it at an earlier age and stopped purchasing unnecessary beauty products ever since. Saved me lots of money thankfully.
Tear trough implants may be the way to go. Few surgeons specialize in this, but when I researched it a few years ago there was surgeon in Texas (Houston, I think) who performed those. I think the trough implants were customized to each person as well - i.e. they'd take an impression using putty (similar to what orthodontists do to create retainers), and then send it off to a lab.
Gua sha?facial massage for lymph drainage, also avoiding toxins like processed sugar, caffeine, tobacco& alcohol, eat & sleep well and drink lots of water, won't fix all things but definitely helps to be healthier
I’ve had hereditary, extremely dark circles my whole life and I just started saving up for under eye filler. I’m so glad I came across this video.. I found that the safest “treatment” for dark circles is concealer. It works better than any filler has
I think we are about to see a large reckoning in the cosmetics industry, where people are realizing how much fillers do spread and maybe should be used as a last case option rather than go-to solution
I've honestly been waiting for this. So much filler work looks awful. The wealthiest people in the world (i.e. people who can surely afford to go to the best doctors) look objectively worse after a certain amounts and placements of fillers. Some of them, on video or irl, look like they have cement in their faces. It's so strange that people have forgot the whole point of any of this is to look BETTER. If removing every last wrinkle makes you look like an alien with cheeks full of marshmallows, that's not a desired outcome!
Thanks for this video. Some doctors think they can't change their minds about certain subjects. You demonstrate true integrity, and your patients are lucky to have you!
I had my very deep tear troughs (genetic) filled with a ‘soft’ form of filler via a cannula which was inserted just below my cheekbone and the filler was laid deep onto the orbital bone. That was done in 2014, the result was great and still looks perfect.
Thank you so much for sharing this video. It takes supreme courage to say "I was wrong." You are truly helping others by constantly learning and letting the science guide you.
I saw the title of this video in my feed and panicked because I just had this done about a month ago. The doctor did the mid cheek filler as you described at 15:00. Maybe he’s aware of the Jan 2021 article on the tendon attachment or else he saw this video, but thankfully it worked! Thank you for your integrity.
@@CamilaSaurus My doctor advised against tear trough filler. I have decided not to repeat the mid-cheek (or any other) procedure. I’ve been doing facial exercises instead and find that they give me a better, more natural result.

I found your video through NYC surgeon Dr. Larry Linkov. Thank you for sharing your research publicly so people can make better informed decisions. Your candor is refreshing and informative.
Thank you so much for your thorough explanation of tear trough fillers. I had this procedure done in my mid-20s and saw a significant improvement. Now just a few years later, I have terrible swelling/bulges under my eyes that age me by 10 years. I had been told that my filler should have dissolved by now, but the information you provide has confirmed my suspicions that it is still there and causing the disfigurement. I now wish I hadn't gotten the filler done in the first place as it significantly affects my appearance and I can't afford to have it dissolved.
This is fascinating! I am so glad the findings have been published. I suspect it will take several years for thinking to change regarding the semi permanence of fillers!

This video was sooo good!! never did l expect to learn about a ligament that's connecting SKIN to bone and not bone to bone like the rest. I can't explain how much this channel and your content mean to me as l'm currently on the path to go into dermatology/surg myself.
I got my Orbicularis Retaining Ligament released almost a year ago due to tear trough deformity - the results are fantastic, fixed my problem 100%! (That was all I had done. I'd had a lower blepharoplasty about 10 years ago and did not need further fat removal or anything.) Got it done because I was afraid of fillers there due to the complications, and am so glad I did. Thank you for your informative and honest videos.
You have taken a good pathway to fix your concern. Unfortunately, many are unwilling to take the surgical pathway, and take the 'easier' filler pathway only to be disappointed. Thank you for sharing your story.

I have extremely deep set eyes that cause permanent tear trough creasing and dark circles, and I had been doing research and thinking I wanted to get filler for them but this video changed my mind. I have also sometimes joked and called my tear troughs my “itachi lines” Bc they look like the lines on the face of the naruto character, and I have decided to embrace them. They are a natural feature of my face and they make me visually distinguishable. I don’t need to look like everyone else. My “itachi lines” are not a deformity, they are a defined bone structure lol, they are a beautiful part of my face. Thank you for helping me realize that.
Love you for that. I also wanted to do this treatment bcs mine are due to genetics. Retinols are working at all for me. I used to make fun bcs of my dark circles. A friend from highschool even said that i have the darkest eye bags in our batch🥲 that hurt me till today🥲 i mean i know? I see myself in the morror everyday?🥲🥲
i was going to get tear trough .. and this just stopped me .. thank you so much for your honesty and for trying to put things right for your patient and wider audiences
Really? If you believe upper and lower blepharoplasty is less risky than fillers, you haven’t done much research. Both are good options but have risks.
Yes, there are not many doctors who think this way, but after doing fillers for so long, I feel it is a real problem that more doctors need to be made aware of. Hope you enjoyed the video
An incredibly thorough video, taking a great deal of time to include 3 interviews and his own long term experience with tear trough fillers. Very much appreciated.
Thank you for your transparency, your honesty. Admitting a (mistake) takes a big person. I understand your thought process. You went with the standard at the time but learned from experience, and you're making a difference. Thank you.
This video was the best thing I could ever have watched... Have been thinking of having this tear trough injection for several weeks. Thankyou for being so honest ;))
I'm a estetic doktor from Kraków in Poland. I have the same observation like you and few years ago I stoped augmentation tear trough hialuronic acid....after years the most satisfactions for and my patients and the safest is augmentation with combain S-PRF and amino acid. Nice to meet you ,it was pleasure found your opinion. Best regards Roma Zielińska MD
I contemplated getting these for years! Then I found out my chronic under eye hollow circles were a warning symptom of thyroid and thyroid eye disease. Glad I didn’t get these fillers to silence my body when it was giving me clues something was wrong.
Omg. I literally am in the same potential situation but a few steps behind you. I even booked a consultation for tear trough in two weeks but I’m waiting for some test results for potential thyroid problem. I had no idea it could be linked! Thank you for writing this! Now I will definitely wait until I get my results back until I consider any type of injections! 🙏❤️
@@lauramay5361 The fillers doesn’t do anything to your thyroid, but if you’re suffering from hypothyroidism, you’re eyes can get hollowed out and you can get more dark under the eyes. That means that if you just cover up the symptom for an underlying problem, it can be dangerous for your entire body, not just for your dark circles. If you treat your hypothyroidism instead, you might not even need a tear trough to get rid of it!
Hope more people come across this video. 3 years ago I started developing huge puffy bags under my eyes, so large my skin was stressed and pink with irritation. Went to an allergist thinking I was having a severe reaction to something, never considering it was the filler I got just over TEN years prior. When that appointment yielded nothing, ended up at a pricey cosmetic dermatologist who specifically asked about filler, and suggested it could be the old molecules partially breaking down and taking on more water (regarding the fairly fast onset). Fortunately hyaluronidase resolved it, very relieved I did not have the under globe issue addressed here! Just don't do it. I don't even think the risk of placing just below the trough is worth it with the possibility of migration and these substances being so long lasting in some people.
Thank you for sharing. As an Oculoplastic surgeon in California, I agree with your assessment that peri ocular fillers are quite risky. I have managed 2 cases from outside, non-doctor injectors with filler in the orbit. One patient had worsening swelling after COVID vaccination (published reaction) and one with double vision after filler injection. I think if you got an Oculoplastic surgeon involved they would have been able to to treat without the ultrasound; in my two cases I was able to inject into the orbit without any special imaging guidance due to my experience of giving orbital and retrobulbar injections for other medical conditions. Any ophthalmologist is trained to inject into the orbit, so do not forget to involve you EyeMDs.
Thanks for your comments. I am a doctor myself and landed on this very interesting and eye-opening video. I noted that you do intraocular injections for various medical conditions. Do you have experience with Charlins syndrome ( nasociliary neuritis )? Thanks
this video is SO well done, so informative! i really appreciate all the research and transparency you're bringing to the forefront. thank you for this, you are truly an amazing doctor
Amazing pioneering work into this difficult to treat area. I learned lots as an injector and will not also reconsider my approach to treating this area. Thank you for advancing the field beyond "inject where it's hollow" to "know before you inject!"
Thank you so much for caring enough to tell people about this. You are truly interested in your patients welfare. That is so admirable. And appreciated.
Doc . I cant thank u enough!!! I was exploring to see more videos to encourage me to do under aye filler… I already booked an appointment with a very professional doc to do it ,, then this video popped out blowing my mind!!! NO WAY I would do it after seeing your video,,,, I canceled my appointment so happily……. Thank you soooo much
I watched a video of yours 2 years ago in which I doubted you over the opinion of my injector and another maxosurgeon. I would just like to say 2 years in I've made an appointment to discuss chin implants. I think the filler industry will get better the more you speak up but it's just so much to go yet. I feel embarrassed how much I just went with everything before.
As always, your honesty regarding fillers is greatly appreciated. I have a massive complex about my lips, they've bothered me my entire life, but with doing more and more research and watching more videos like yours, I've chosen to stick with botox lip flips and makeup techniques instead of lip or undereye fillers, for the time being. In a few years as I age and my face matures more, maybe I'll change my mind - but at this stage, fillers aren't worth it yet for me (I mean, I hate my lips, but do I really hate them enough to risk blindness, necrosis or migration? Not really.)
Ironically, with lips, the finer your lips , the harder they are to fill. There is less 'space' in finer lips to fill. Good to see I have made you consider all the possibilities. Fillers are not for everyone.
Just stick with fat transfer or do nothing. At this point nothing looks better than what we are seeing out there. Use bioidentical hormones when you get older and take saw palmetto to prevent collagen break down.
Thank you so much for stopping doing this procedure and bringing light to this issue. It’s kind of terrifying. I hope others follow suit and the company developing this and telling these lies stops, put out a warning and stop selling this.
I had to pay for an appointment with a plastic surgeon. He didn't inform me about anything, just tried to sell. I was disgusted and didn't agree to the surgery, had to pay anyway for the appointment LOL
@@viccosmeticinstitute do you have any experience with dissolving Ellanse?
@@sds2954 No, I don't think much can be done to dissolve Ellanse. Where did you have it done?
@@viccosmeticinstitute thank you very much for replying to me. Yes, unfortunately that's the feedback most specialists have told me. I would never have allowed Ellanse to be used if I knew this information upfront sadly. I am based in Cape Town, South Africa and a local dermatologist did it.
You have single handed bought so much transparency in the industry. I now see so many high profile plastic surgeons acknowledge that fillers don’t dissolve but move. The same ones who used to say the body just absorbs it.
It's great to hear that we have made a change!
It does absorb...
@@drluizpires If only it did. The research shows it does not. For the most part it just moves. Well known plastic surgeons are acknowledging this in the US.

It’s refreshing seeing drs admit medical mistakes they’ve made
Id say it's an act of heroism.

Thank you for sharing your story Thoughtful Pro. Have you had the filler dissolved by the way?
Thoughtful Pro Absolutely right..also, few doctors know where to inject. They choose the easy option that is the nasal folds area and after 2 treatments the shape of your face has changed. I see so many women around followed the same procedure. Ban the Kardashians and promote self acceptance...
Omg this is so true.
I’ve only had my lips injected and this statement is so true. Depending on what I eat or drink my lips swell or deflate..

The amount of medical integrity here!
I love this. People admitting that they were doing something based on their education at that point but through experience have changed course of action. More practitioners should have this attitude! Don’t let pride get in the way of solutions, science is an ever-evolving process.
This is why i dont trust people because of their education/credentials.

Stay safe!
Can’t believe he said there are no risks. The consent form must list blindness and stroke as rare but possible complications of under eye filler!
Thanks for letting us know, hope you heal soon xx
May I ask how much filler was used per eye?
This is a powerful life lesson. Anytime we are trying to hide, rather than accept, anything about ourselves...it will shift and cause symptoms elsewhere.
The truth will come out eventually.
To love oneself, wholly, and to reject the societal pressures...is to stop betraying oneself to fit a certain mold.
Our self betrayal will always come out, whether in snot...or bad relationships..whatever.
I wish you healing and wellmess

I regret doing it. Glad you found this video!
I had fillers twice & yes the last one lasted 5 yrs & pooled in my lower lid on the cheekbones. It looked horrible. I had no clue it was the filler.
@@Call-me-Al is it those dermarollers with needles?

I was one of the many people who was seriously considering this procedure, thank you so much for putting this out ☺️
I am so grateful for your wise words and I wish you were on more.

But then this video popped up and I'm so grateful that I have been educated in this matter and will NOT continue this treatment anymore and I'm so thankful that I haven't gotten any bad results
Same! I only got it done once and had no issues but now I’m too scared to get it done again!
May I ask how long ago you guys got this done?

In my defense, 17 years ago they told us that it will only last 3 months! I had to endorse it for 17 years! sigh!

I'm not a doctor or plastic surgeon, but surgery - either blepharoplasty or a fat transplant, depending on your type of eyebags or eye hollows
It works better than any filler has
What about micro fat transfer?
We need more doctors like you. I really admire and respect your dedication and integrity.
Thanks for this video. Some doctors think they can't change their minds about certain subjects. You demonstrate true integrity, and your patients are lucky to have you!
Love your truth , a True HONEST dr. Who RESPECTS his work......
Are you saying you didn’t do tear trough filler, just the mid cheek? Did it help with the hollowness, and has it traveled/caused any issues since?

Really? If you believe upper and lower blepharoplasty is less risky than fillers, you haven’t done much research. Both are good options but have risks.
Wow - thank you, doctor. This is far from what the mainstream has been telling us about filler, although it often looked like it just moved.
Yes, there are not many doctors who think this way, but after doing fillers for so long, I feel it is a real problem that more doctors need to be made aware of. Hope you enjoyed the video

What does it do to your thyroid?

Thanks
how about fat transfer?
How did you know it was in the orbit without imaging? Where in California are you located?

Amazing pioneering work into this difficult to treat area. I learned lots as an injector and will not also reconsider my approach to treating this area. Thank you for advancing the field beyond "inject where it's hollow" to "know before you inject!"
