When I worked at a pharmacy, the pharmacist always printed the cost of the medication, but then wrote down what the cost of the generic was. That way people understood and had almost a choice in the matter. It sometimes saved them 100s of dollars
PBMs have essentially become a massive kickback scheme. Just change the anti-kickback laws to no longer exclude PBMs or anyone else. There should be no reason for kickbacks to exiat.

This also makes referrals much more straightforward, since the doctor doesn't need to worry about a particular type of specialist is in or out of network.
In my country, Australia, the government negotiates with the drug companies and if the drug company asks for too much the government says "oopsy, looks like you can't sell that drug in our market", and whaddya know... turns out that they actually do want market access 😂 In cases where the drug is genuinely very expensive to create and where the consumer typically wouldn't be able to afford it, the government subsidizes it.

A big thing I don't see discussed often is that, the money paid to health insurance companies doesn't sit in a pool that then pays out when someone needs it. They're investing it. that's why they're so resistant to paying for things, or delay as much as possible.
Price gouging of pharmaceutical drugs makes no sense to me; if you as a company charge medications so much that many potential customers can't afford them, then how are you going to make money off those medications in the long-term with only the wealthy top percentage of the population being able to afford them? I think The U.S. Federal Government needs to have some form of price control to ensure that we don't pay an absurd amount for prescriptions compared to other developed countries. If a bottle of pills in say Germany costs $6.00, that same bottle of pills shouldn't cost more then 50% extra here in The U.S., which would be $9.00 in this case. I'm not saying pharmaceutical companies don't deserve to make profit for their research & development, but they shouldn't be able to charge insane prices for their life saving products that most uninsured Americans can't afford.

I just don't understand your health system. In Australia, if I'm sick I go to the dr, they write a prescription, I go to the chemist (pharmacy), they fill it, I pay for it, end of story. Medications are different prices but mostly under $20-$30. This system is just insane.
I pay $25 or less for prescriptions. My insurance, including one of the big PBMs mentioned, is thru my employer, who self-funds, and is also a large state government. These companies will be reasonable, if forced.
Here in Portugal my mother's last pharmacy bill total was 86 Euros. She paid 4 euros 27 cents, the state paid the rest. This is only possible thanks to my country's vast, unprecedented wealth and resources.

No. Patients & retail pharmacies are. Depending on the price of the drug health insurance companies decide how much of the cost they'll cover for patients & how much they'll pay retail pharmacies to fill it. If retail pharmacies are losing money on filling prescriptions because the reimbursement from insurance companies are low they end up going out of business. And if the patient's co-pay is super high & they can't afford to pick up their medication they go without treatment.
Sounds like it's time for the government to get into the pharmaceutical business. That way, they can control the cost of the drugs they research and develop and set prices accordingly.

Most people don't take into account that one reason drug prices are so high is that even after developing a drug after many failed attempts, it still has to pass the myriad of Federal tests that are required to bring it to market. Drug companies have to receive enough in profits from a successful drug they create in order to start the process all over again for the next one.
I would like to know what is going on behind the scenes with Medicare choices between original Medicare with supplemental coverage vs Medicare Advantage. I keep reading in the comment sections of different TH-cam channels and one long debate on the Nextdoor app, that even though Advantage appears to be the best choice, it is not. I have private coverage from the Marketplace but I turn 65 later this year. I take a medication for an autoimmune disease that is an expensive injectable biologic. The pharmaceutical companies offer copay cards to help pay for it, which in my case in 2024 (my first year purchasing coverage from the Marketplace) ended up giving me a copay of $0. But I just found out you are not allowed to have a copay card if you are on any government subsidized programs, including Medicare. Is this just another way to force a person to get the Advantage insurance?
You should not advocate for the government flexing its arm here. You should advocate for this: All drugs that have similar effects should be accessible with any prescription. So, if there is a name brand and a knock off and both achieve the same thing but you get a prescription for the name brand you should be able to turn it in for the knock off. This creates competition and causes prices to come down to a little over the cost to manufacture.
Suggest you do some actual reading on how intellectual patents work for pharmaceuticals. Drugs are defined by their ingredients (chemical compounds) and characteristics, method of use and formulation. Far more complicated than for what treatment share they market for.

Every year they get a 6-7% insurance premium bump, in 2021 all insurers that provide services to those with obama care got a tax break. The whole thing is corrupt.
