Throughout the movie its revealed that he doesn't know what is real or a dream anymore. And in the end he doesn't care, he chooses the happiest story. But if you notice - several things hint it was all a dream.

That's not really true though - the movie hints he is still in a dream... 1) The chemicals they used for sleep were too heavy to wake up normally. 2) There is no way to escape limbo other than waiting out the time (100 to 500 years of waiting) 3) Cobb never had a reliable totem - he was using Mals totem and his wedding ring totem could be put on or taken off. 4) Everyone was telling him to "wake up back to reality" (could be his own brain hinting to wake up) 5) Mr. Saito was always waiting for him 6) His kids barely grew up even though it was supposed to be years that he hasn't seen them... This all shows he is probably still in a dream, but chooses this dream over cruel reality. This theme parallels the lie they gave to Fischer as well - who believed the lie in the dream that his father loved him.
That wasn't his totem though... And throughout the film people constantly say "you need come back to reality, wake up". There's a lot of clues that the ending is a fake dream ending.

Christopher Nolan movies always have a dead wife connected to them. In some cases, several. The only one I'm not sure of is "Dunkirk" but technically the story was about how the allies were evacuating because the German Blitzkrieg was annhilating everyone so I'm sure there were some dead wives there too. Along with dead men and children probably.
