  • Stroke nursing NCLEX review (CVA) cerebrovascular accident lecture on ischemic and hemorrhagic strokes along with nursing care, tPA, symptoms, treatments, and pathophysiology.
    A stroke occurs when blood rich in oxygen can NOT reach brain cells (due to either a blockage or bleeding) and the cells begin to die.
    Types of stroke:
    Ischemic (most common) is due to a blood clot within the blood vessel or stenosis of an artery feeding the brain tissue. This limits the blood that can reach the brain cells. This can happen due to:
    -Embolism: where a clot has left a part of the body (example the clot has left the heart due to a heart valve problem or atrial fibrillation) and has travelled to the brain, which stops blood flow.
    -Thrombosis: a clot forms within the artery wall within the neck or brain. This is seen in patients with hyperlipidemia and atherosclerosis
    Hemorrhagic: there is bleeding in the brain due to a break in a blood vessel. There will be limited blood flow to the brain cells and this can cause excessive swelling in the brain from the leakage of blood in the brain tissue. A ruptured brain aneurysm or uncontrolled hypertension can cause this.
    In addition, a patient can experience a TIA (transient ischemic attack) also called a mini-stroke. This is where signs and symptoms of a stroke last only a few minutes to hours and resolve. It is a warning signs an impending stroke may occur.
    Stroke symptoms include: facial drooping, arm numbness or weakness, slurred speech or difficulty talking etc.
    Treatment for stroke (specifically an ischemic type) is tPA (tissue plasminogen activator). This medication works to dissolve the clot that has caused th decreased blood flow. However, tPA must be administered within 3 hours of the onset of stroke symptoms.
    Nursing care for stroke includes: monitoring vitals (especially blood pressure, respiratory status), watching and educating the patient with neglect syndrome (this most likely occurs with patients who've had a right brain injury), diet and swallowing techniques, hemianopsia, and how to communicate with a patient with aphasia (watch the video for more in depth nursing interventions for stroke).

    Great video! I had my stroke 3 years ago. It left me with alexia, and hemianopia, among other gifts. Doing well these days and working very hard to prevent a repeat! I am so very thankful to the fantastic nursing staff that cared for me at the ER and in the hospital proper. Thanks for all you do!

    I'm a "fit and acitve" 52 year old woman who just had my 2nd stroke less than 2 weeks ago. My first one was an eye stroke in 2009.
    This time I was diagnosed with PFO among other things.. and still pending.
    Many follow ups to come.
    This is not my first rodeo being sick.
    I have stage 3 CKD, I had stage 3 cancer in 2005.. the list goes on, despite me being extremely proactive in my self care for decades.
    This is very interesting to me and extremely easy to understand and follow. I've watched many of your videos. You have a gift and are truly an asset to the world. You're a gem.💎

    Wow, what a great presentation Sarah. I really liked the slides you put up on the screen, very nice addition. You certainly have your hands full with stroke patients. I cant imagine how exasperating that must be for the patient. I take Eliquis 5mg twice daily due to an aFib event back in August. So when you mentioned that TPA could not administered with recent use of anticoagulants my ears perked up! lol Great job Sarah!

    Hello Sarah, I am a Registered Nurse too from UK, I look after stroke patients and was very excited when you uploaded this video, I am a fan. To be honest, it is very rewarding at the same time it can be very stressful and heavy at the same time looking after stroke patients. Our system is same as yours we also do thrombolysing depending the type of the stroke as you mentioned and we have a 4 hour window. I am planning to go back to uni to get my masters degree as Advanced Nurse practitioner. Thank you again thumbs up.

    I love your videos. I do know enough relative to this one to produce my own lecture. Some stroke patients may have smaller areas of visual deficits which may preclude them from driving, leaving enough of a minor blind spot to pose an accident risk.
    Smoking and oral contraceptive use=16x stroke risk. It is possible to see a user of both have a stroke in their 20s.
    A layer of the arterial wall is made of collagen and elastin. You need vitamin C for collagen fibers to stick together. Smoking depletes vitamin C, as well as damaging the endothelial layer. Cholesterol patches the potholes, and calcium will smooth the bumpy patch job, reducing elasticity while blocking blood flow.

    My father have a hemorrhagic stroke in December 2006 he is 77 years old in on Christmas Day.. He has a high cholesterol and other health problem too.. I missed my father since in March 14, 2014.. He was in his 80's . He was 84 year old passed away..

    THAN YOU VERY MUCH FOR THIS COMPREHENSIVE VIDEO. I HAD A RIGHT SIDED STROKE ALMOST 4 YRS AGO. I was a nurse for 22yrs. in 1995 I blew out discs in my neck and lumbar, then developed cfids and fibro. In ALL symptoms I developed ,occasionally falling, loss of balance I attributed to deconditioning. well I am 67 yrs old I am hoping to get some more help. thank you for this information

    I love your videos. You explain everything very well. I love the visual learning and all the mnemonics, they help so much. One thing I do think you should upload on it just pharmacology. It's so hard to wrap my head around and can't seem to get it down.
