@@AskDoctorJo no direct injury..just ergonomics throughout life..I'm a little taller than average and after an hour or two of sitting or doing anything my shoulder and arm start pulsing in like nerve pain..it is so aggravating..I bought a stand up desk but still have the nerve pain in other situations..I just bought a tens unit..hopefully this could help..but not sure what else to do or who else to see..a chiropractor, physical therapist..not sure.
I used to sell these things, I dont like how people say they dont know how to use them. I was hoping to get more clinical studies or more doctor lingo rather than the practical application. I have been wanting a tens unit for a while for the space between my shoulder blades. Long story short car accident and I was side swiped in my chevy Cobalt by a guy in a truck 6 years ago. I use the smaller device. In fact, I had a high volume squat session three days ago. The DOMS was bad because of the new stimuli to the muscle group. Last night I put the large pads on the leg in the direction of the actual muscle pattern. I had it on the second highest setting to really dig in. It felt amazing and after I felt so much better. Today, the quad pain is almost non-existent, I also added it to the calf area, not as effective but a bit better today.
Great, explanation of the difference between TENS/EMS - thank you. I have peripheral neuropathy arising from repeated and ongoing chemotherapy. Would I be correct in thinking an EMS treatment would be more suitable for 'reconnecting' the affected nerves in my feet and fingers to the brain. I accept that a full recovery is unlikely but some improvement would be wonderful. I'm not asking for a definitive statement just your 'opinion' - without a full consultation, on the potential value of using an EMS treatment for peripheral neuropathy.
Dr Jo, I want you to know your videos have made it easier for me to speak to my PCM. For instance, if I went to my PCM, I might say, "I'm in pain, help me", and with DRs being overloaded, I was always given the standard physical therapy and pain pills. Of course, I do need these items, but, after watching each of your videos, I am able to say, "Hey Dr ABC, with my conditions would be it medically sound to try an EMS unit?" You give your fans a new and proper language to use in our own care. ---- Plus, I am out of gel, and new pads, and my TENS provider went out of business, I had no idea where to purchase any. Dr. Jo, could you please, please make a video on units that have massaging functions to relax the muscles? Not to exercise, not for pain, just muscle relaxation. The info available online, as well as the standard settings on the units are often contradicting as to how many microseconds should be used depending on which part of the body it’s applied to as well as which frequencies are best (5Mhz< and such). Your opinion would be greatly appreciated.
Is your unit one you can actually change those specific settings? Wavelength, pulse duration, frequency? Many of the TENS units that have presettings will work for relaxation as well.
@@AskDoctorJo Yes, it says I can change the settings for massage and for EMS. But even the preset settings change a lot, mine has 3 modes for massage and one goes up to 60Mhz and others just below 5Mhz and also the manual says the legs need 400 microseconds and I have read elsewhere 250 is fine.
There are a lot of variations to it (usually anywhere from 50-250 microseconds, with 50 - 100 Hz for the "normal" setting), but I would stick to the manual since each unit is different. It really is to your specific tolerance, so that's why it varies so much. What might be comfortable for you, might not be for someone else. So I would really try different settings and see which one relaxes you the most. As long as you don't have the intensity too high, it shouldn't hurt anything.

If a muscle is too tight, can it be relaxed without reaching the point where there's a contraction? I read somewhere that frequencies lower than 10Mhz send oxygen and restore the fibers by removing "waste", which then relaxes the muscles. I'm afraid of seeing a contraction or using the EMS function because I don't know if that could cause a tear or something.
Hz or Mhz? I have only seen ones with Hz, which is also pps (pulse per second). Usually a lower Hz is for a different setting, which sometimes does cause a quick contraction. But again, all units are pretty different, so I am not sure with yours. With that said, a unit can not produce a contraction stronger than what you normally can (without injury), so unless you just leave it on with a painful contraction for a long period, it is not likely to tear anything. It might make it sore, but it shouldn't be anything bad.
Hello! I have a question please. I am suffering from full body atrophy. Doctors are clueless but I know it's from a spinal injury I've had years ago. Anyways, I am wasting away so I have been trying to combat it with weightlifting but I can only workout certain muscles at the moment due to lack of workout equipment. Will the ems unit help my muscles to stimulate and grow? Thanks.
It really depends on how much of a spinal cord injury you had. It can help retrain muscles, but it will not grow muscle if they are not able to grow on their own. Since they are fairly inexpensive, it might be worth trying though. Good luck.
Hello Dr. Jo, I really like your videos. May I ask Why did you have a Hip Surgery? I am 44 years old and I had my Hip replacement 6 years ago, it helped a lot with mobility and it extended my right femur improving the length of my leg, it was shorter since I broke my femur when I was 9 years old in a car accident. Usually I can walk without pain but as soon as I walk more or try to exercise, I start feeling pain. I just bough this machine to improve my legs muscles for strength and better appearance. Can you recommend legs exercises for hip replacement? Thanks

I'm a 45 years old male and an avid cyclist for nearly 25 years, and as of a year ago underwent cervical diskectomy c5-c6 and now how having issues with the facet joint region, possibly arthritis, will find out from my pain management doctor this week. Anyhow it appears the symptoms are affecting my left mid trap/rhomboid area casing muscle knots compressing the medial nerve down the left arm at times. When the pain subsides there the inflammation pain moves across the top shoulders into the back of the neck. Wondering if you may have experienced any of your patients with similar issues and the post results? I'm hoping an EMS FlexTone will help with the knots, thank you in advance! Vic
Most chiropractic offices seem to use (EMS) when people are injured (car accidents). When you’re injured, you feel pain right…? So seeing that EMS is for muscle rehab, why does EMS seem to be used more often in a chiropractic setting than TENS? The way I see it, is that people feel pain regardless; and anyone looking into an EMS / TENS unit, is probably doing so because they feel pain somewhere. So it would seem as if TENS should be used in any case. How do you know if you should use TENS (block the pain) or use (EMS) stimulate the muscle. TENS sounds like a temp solution, but what will EMS do for the long run? And to clarify that question, I guess I’m basically just asking, how do you know if you need a pain management solution vs a muscle rehab solution…? I can only assume that you have to determine whether or not your pain is a muscle related issue or a nerve related issue…
I don't really know what most chiropractors use, but many of my patients talk about some form of TENS being used if they see a chiropractor also. They might call it "e-stim" which can be a number of different currents, but if you are not getting a muscle contraction (or at least trying to), it's not EMS or NMES. EMS can be used for pain control as well since technically retraining a muscle should help decrease the pain. So it is really injury dependent. The best way to find out what you need is to go see a health professional first, even if it's just for an evaluation.
Great video. I had abdomen surgery about 10 months ago and the surgery caused some nerve damage. I believe the nerve damage is causing my abdominal muscles to weaken. I thought I would use the TENS to hopefully stimulate the nerves and the EMS to re-strengthen the muscles. What are your thoughts about this approach and is it possible to cause damage to the muscle with an EMS if its in a weakened state?

I do strength training with resistance bands & strengthen with my core. I believe I have a herniated disc in my neck & lower back. Nerve pain happens a lot along with my muscles get tight. I do stretches too. For instance, I felt fine when I went to bed. I was laying on my back. I woke up with nerve pain from my knee to my ankle. Sciatica is flaring up. Same goes for my neck. I'm guessing TENS unit since I strengthen my muscles already or a combo of TENS unit & EMS.

Dr Jo I had knee surgery 4 days ago. I had a tear on my lateral and medial meniscus. I'm going to be doing my own home therapy I have everything I could possibly do a therapist place I was wanted to know when is it the best time to do the tens either before therapy or after
I purchased a TENS unit to help with endometriosis pain. I'm having a hard time figuring out where to place the electrodes (mine are the four common patches like in the video) so that they cross and cancel out the cramps/pain. Any advice on placement of those will be super appreciated. Great informative video too! Thanks!
Thanks, and you're welcome! Is your pain pretty general in the lower abdomen, or do you have specific spots that hurt more? If it's general, you can go wider with a larger area, like go out to right inside the hip bone area on the front (not on the bone), and place them at the belly button line, and then about an inch or two below those. Some people also just place them on their lower back, which also helps with the cramping in the front. Good luck.
I have an self powered electromagnetic metallic deflector inserted into my sternum by Stark industries. every time I connect the TENS or EMS machines and turn them on an electromagnetic pulse seems to fire out of my palm, could you please help me by telling me how to use these machines properly as I keep destroying my expensive furniture every time I use these? PS I'm considering the option of fabricating a suit for these to control the energy pulse that keeps discharging from my palms, but am not quite sure if this is a good solution or not?
You're welcome! TENS is for pain reduction and increasing circulation to help the healing, so that would probably be best as long as you are cleared by your doctor to use it on those areas.
I had knee surgery back in December. I guess because of the pandemia they didn’t send me to therapy. My does not respond normally and even it locks and pops. I’ve been feeling my leg muscles too weak. If I walk for long periods , my knee starts to hurt bad. Do you think a TENS/EMS unit can help me with this?

You're welcome! The original ones (the type II that you needed a prescription for) had red and black wires for positive and negative charges. The red (positive) would be placed closer to the body/torso, and the black (negative) would be placed farthest away from the body. For most of the ones now that don't need a prescription, that doesn't apply anymore (unless it's indicated in the manual), and it's not a big deal if it's not placed that way, but if you do have two different colors, it's probably worth putting the red closer to the torso.
My PT is using both of these types of therapy on my knee ...sadly the quad muscle is not contracting...I allow the PT to increase the intensity nearly beyond what I can tolerate so we can get this quad to contract but still no contraction
What kind of injury did you have? Maybe the PT just needs to move the electrode placements around. Are you using Russian current for the muscle stimulation?
@@AskDoctorJo thank you...I'm supposed to have tkr but the dr wont do it now he said my quad is to weak...but the pain in the patella is so bad it inhibits me from doing certain exercises like leg lifts...I have been this way for over a year ...I really cant take it anymore....I see the dr again Wednesday..not sure what to expect...thank you for your suggestions...

I have used a tens for years when my back is out of whack and it was suggested that I leave it on all day. What is your opinion on wearing it for that long? Obviously it is the small, portable unit.
+Kristi Campbell I wouldn't recommend it, but that is just my personal opinion. It is really meant to get you through bad pain moments. If you use it long term, then your body almost builds a tolerance to it, just like long term medication use, so then it become less effective. Good luck!
Surgery was March 12th 2018. I am out of my cast and into the CAM boot which I can take on and off but can not weight bear unless I am wearing it. I have started with PT and I am doing exercises without the CAM boot while at PT to regain mobility and strength. I am currently at a 1 inch calf difference and PT said that will only get worse. I am a very fit person who would really like for it not to get worse so I'm hoping EMS with as tolerable seated calf raises might help. Thank you for your response.

They all do very similar things, but in very different ways. TENS and EMS are actual currents that not only help increase circulation to an area, but it also help relieve pain. The Thermotex infrared heat uses light therapy to help "heat up" the area which increases circulation and helps relieve pain. So it really depends on your specific needs and how far out you are from your injury.
AskDoctorJo my injury occurred over a year ago. I haven't been committed to rehab, until the last four months... in the water, for starters. Are there a few tools/straps that I should purchase and exercises/physical therapy that will be essential to get me back active? I dare not think it is a lost cause. :/

I am not familiar with that particular machine, so I don't know specific numbers for it. For the EMS portion, does it have a specific name, like Russian current, or are they all pre-programmed?

Hi there Dr Jo, Back in September 2nd 2015 I had a heart attack and ended up having an emergency operation. I had a triple heart bypass and the vein used was from my left leg, cut from ankle to just above the knee. Since then I have had knee aches and pains and back aches and also a lot of gout too. One good thing about it is that I am still alive though. Would the TENS benefit my aches and pains? It is painful when I walk now on one side of my hips but I do my best to plod on. I am 67 now but still young at heart and I want to be able to get around without knee and hip and back pains.

I have a question about the portable unit. I received a very similar TENS/EMS unit today, but am curious which functions are actually TENS and which are EMS as they are all labeled as different forms of massage... The manual is a bit sketchy at best... Can you identify which are modes are TENS and which are EMS since the manual doesn't do that?

I was just wondering if you could tell me if there is any way my knees will heal so i will be able to hike again. Im wondering if you know how long it takes to heal from knee pains from hiking. Unfortunately, without getting to evaluate you, I really don't know what it could be. If you just irritated your knees, then you should be fine with some gentle stretches and exercises. Good luck.

hi , nice video.. i ve had a hip replacement surgery on left n right is waiting n have strong pain due to the bone deterioration on right (Still to be operated ). couple of questions: 1. what is the best machine to use for operated hip ? also is it safe with having metal implant? 2. Thanks. If you are looking for pain control, the TENS unit is best. It is okay to use with metal implants as long as it's not a pacemaker or something like that. If you are still in therapy for your first surgery, your physical therapist will be able to help with the placement. Good luck.
it would be helpful to show us which "one" has the contraction. I ordered a similar device which says its ems but there is no clear indicator how to do that.
What do you mean by "one?" The muscle that contracts or the unit that does the contraction? Unfortunately, each unit is usually pretty different with it's settings.
I have been bedridden for a few months with an impinged nerve in lower back which means can't stand or sit for no time at all hardly. I ordered a tens machine off amazon to help with my lower back muscle that has been basically locked on for these past months due to the impingement. I forgot about my legs wasting away until today they are aching real bad, I never had developed leg muscles to begin with (chicken legs) so really didn't have much leeway I guess for being laid up this long. Would the EMS unit help me to rebuild these atrophied quads? Or would I be ok with using those exercise bands to work the muscle and then use tens to make the ache afterwards disapate?
If you are able to contract your muscles on your own at all, that's what you should try to do. You can do something like a quad set or a straight leg raise with the EMS, but you always want to try to actively move your muscles as well. Good luck.
Chris C you need good chiropractic adjustments once a week or so until you're able to stand straight again. I know they can be pricey but im sure you can find a good affordable & i promise you won't regret it if you find 1 who knows what thwy like are doing. for the pain though i dont see why the unit wouldn't help.

I have severe diabetic nerve pain in my calfs. I just got a TENS and EMS unit. I am assuming I use the tens for pain muscle relief and my goal is to get off of pain killers. How. do you determine what # level setting to you start at (1-150) and there are four categories (Burst,Normal, Pulse With Modulation & Plus Rate Modulation). Which of the 4 is best to set it on. One last question. Can these units be used a preventative, like starting of the day or afternoon with no pain present to help in eliminating severe pain coming on. Thank You
+William Snyder Yes, you would use TENS for pain control. It really doesn't matter with the setting a whole lot. It depends on what setting you like and what feels best for you. You want it to be strong, but not painful when it is on. I don't think it can work like a preventative, but it is definitely worth a try. Good luck.

The TENS unit might help control pain some, but honestly, the best thing for arthritis is exercise. If your mother has multiple spots that hurt, aquatic therapy in the water might be great for her. Good luck.
+Kenny Ho I would say using both. However, I really don't think the EMS will strengthen your muscles more than what you can do on your own. It is really for rehab and recovery when your muscles aren't quite engaging on their own yet, but it might help some. Good luck!
Hello dr Jo. I am a recently trained Doula and Childbirth Educator and I have a couple of questions. One would you advise using a TENS unit for mommies with back pain or hip pain? And two, I am wanting to simulate contractions to demonstrate labor contractions and how to properly count them. Would you recommend this EMS for this demonstration?
EMS is contraindicated for women during pregnancy. TENS can be used if cleared by their doctor, but you do have to be careful where it's placed since some studies show it could induce contractions.
Hi Jo, I have had a persistent chest injury for about 2 years caused in the gym. Long story short, I did not give it enough time to heal in the weeks after the initial injury. Now there is a weakness in the muscle fibres which causes it to re-injure very easily and was wondering if using a TENS machine will aid recovery once I have worked my chest in the gym? Your advice would be most appreciated!
TENS is really more for pain control, and some to increase circulation to the area. I'm not sure if it will help, but it might be worth a try. Good luck.
Dr. Jo, do you know how one can find the motor point on a muscle with EMS? I was informed on it, but wanted clarification. I don't want to get an electric shock to my heart. My understanding is that you put one electrode down on the Pt. and then one in the palm of your hand. You then turn the machine up to a light intensity and run your finger down the muscle until you feel the electricity at its strongest point? Is that the (safe) way to go about it? Thanks.
If you don't have a pen-electrode, you can do it that way. You might have to wet your finger slightly for the current to go through, but it won't hurt you either way. Good luck.
Hi Dr Jo firstly let me congratulate you for your magnificent job that you do with this channel. I practice Kendo which requires a lot of strength on the knees as there is a lot of power and also landing after very short and intense movements,sadly I injured my knee ligament a couple of years ago and know I've been told that I need to strengthen my knee if I want to continue practicing. How would you position the electrodes to make it work?
If you are looking to strengthen, EMS is best. I would use two electrodes over quad muscle belly, maybe 3-4 inches apart. You want to be able to get a muscle contraction. Good luck.
I have a Massage mode on my TENS Sports 2 machine, and I really like having it as close to max intensity as I can handle. It does of course contract the muscle, especially on my quads after a long run. Is this ok to get a intense massage in? Or am I fatiguing my muscle too much?
Yes, since TENS is for pain control, you can use it several times a day 3-5. EMS is actually working the muscles so you don't want to over work them, so probably just 1-2 times a day. Good luck.

I have MS and need to avoid atrophy of right leg muscles. I swim every day but I don't think the muscles are getting enough stimulation. Could I use a TENS machine to help? I tried the Walk Aide that shocked the peroneal nerve when the knee was bent but found it pretty distracting and painful as it jerked my drop foot with the shock. But I liked the idea of stimulating the nerve- just not the price tag of $5K. The TENS units are much cheaper and would they do the trick?
Dr. Jo, I suffer from OA, especially my hands, I use a TENS on my chronic low back pain and absolutely love it, but I cannot get any relief for my hands. Most of the pain in the base of my thumbs. Any suggestions?
Depending on the electrodes, you can cut them to be smaller as long as you don't cut the part that has the wiring. Then you can get it closer to the area since it's smaller. Do you have the square shaped electrodes?
Yes, I have the 2" square electrodes, I place one on the palm of my hand and one on the joint above the thumbs base, Set it to 120pps and width at 50. I have very large hands, most gloves I have to buy are X-large. I also use the compression gloves at night.

I have knots in my shoulder and mid-back. I have been using a TENS unit on my shoulders and I do get some relief. The TENS doesn't release the knots. I just ordered an EMS Device, where would you place the electrodes using the EMS?
One is an alternating current and the other is a steady current. It really depends on what is going on, and is different in every situation. Good luck.
Hi! Thank you for your video. I have pathelofemoral pain sindrome in my left knee and now I have pain in my right knee too. (I guess it is from being over used) I started using TENS machine on both knees 30 min each, once a day and I feel a little better. I wonder how many days in a row can I use the TENS? If you think it's coming from your back, I would still focus on treatment on the back. If you want to try it on your foot, depending on how big the pads are, maybe the top or bottom. Sometimes it takes a few placements to find the best spot that works for you.
Correct. You really shouldn't get a contraction with a TENS unit. Sometimes on smaller muscle groups you will get occasional ones, but it shouldn't be constant.
I always love your videos-very informative! I do have a question: is EMS good for muscle knots/spasms? Would the electrical contraction from EMS reset the already over firing of the muscle fibers or make it worse by contracting more? I use a TENS-it helps, but I'd really like to relieve knots so I can exercise to build up weak muscles in my lower back due to scoliosis.

Hi, I have spinal stenosis in the lumbar spine as well as sciatic pain. I donated a kidney 19 years ago and part of the incision herniated so my muscle tone in my abdominal is not good. The sciatic pain is always on the side of the weakened muscles. I have used deep tissue massages and Vinyasa yoga for the last 3 years to manage and keep things flowing... However lately it doesn't take much to set it off.. Am wondering if TENS is an option?
It might be. With spinal stenosis, extreme extension of the back usually flares things up, it might be to a point where the Vinyasa is irritating it instead of helping it. Good luck.

It really depends on what is going on in your shoulder. Did you have a specific injury?

I used to sell these things, I dont like how people say they dont know how to use them. I was hoping to get more clinical studies or more doctor lingo rather than the practical application. I use the smaller device. In fact, I had a high volume squat session three days ago. The DOMS was bad because of the new stimuli to the muscle group. Last night I put the large pads on the leg in the direction of the actual muscle pattern. I had it on the second highest setting to really dig in. It felt amazing and after I felt so much better. Today, the quad pain is almost non-existent, I also added it to the calf area, not as effective but a bit better today.

I have peripheral neuropathy arising from repeated and ongoing chemotherapy.
Would I be correct in thinking an EMS treatment would be more suitable for 'reconnecting' the affected nerves in my feet and fingers to the brain. I accept that a full recovery is unlikely but some improvement would be wonderful. I'm not asking for a definitive statement just your 'opinion' - without a full consultation, on the potential value of using an EMS treatment for peripheral neuropathy.

For instance, if I went to my PCM, I might say, "I'm in pain, help me", and with DRs being overloaded, I was always given the standard physical therapy and pain pills. Dr. Jo, could you please, please make a video on units that have massaging functions to relax the muscles? Not to exercise, not for pain, just muscle relaxation.
The info available online, as well as the standard settings on the units are often contradicting as to how many microseconds should be used depending on which part of the body it’s applied to as well as which frequencies are best (5Mhz< and such).
Your opinion would be greatly appreciated.
Is your unit one you can actually change those specific settings? Wavelength, pulse duration, frequency? Many of the TENS units that have presettings will work for relaxation as well.
@@AskDoctorJo Yes, it says I can change the settings for massage and for EMS. But even the preset settings change a lot, mine has 3 modes for massage and one goes up to 60Mhz and others just below 5Mhz and also the manual says the legs need 400 microseconds and I have read elsewhere 250 is fine.
There are a lot of variations to it (usually anywhere from 50-250 microseconds, with 50 - 100 Hz for the "normal" setting), but I would stick to the manual since each unit is different. It really is to your specific tolerance, so that's why it varies so much. What might be comfortable for you, might not be for someone else. So I would really try different settings and see which one relaxes you the most. As long as you don't have the intensity too high, it shouldn't hurt anything.

Can I ask you something else? If a muscle is too tight, can it be relaxed without reaching the point where there's a contraction?
I read somewhere that frequencies lower than 10Mhz send oxygen and restore the fibers by removing "waste", which then relaxes the muscles.
I'm afraid of seeing a contraction or using the EMS function because I don't know if that could cause a tear or something.
Hz or Mhz? I have only seen ones with Hz, which is also pps (pulse per second). Usually a lower Hz is for a different setting, which sometimes does cause a quick contraction. But again, all units are pretty different, so I am not sure with yours. With that said, a unit can not produce a contraction stronger than what you normally can (without injury), so unless you just leave it on with a painful contraction for a long period, it is not likely to tear anything. It might make it sore, but it shouldn't be anything bad.

Hello! I have a question please. I am suffering from full body atrophy. Doctors are clueless but I know it's from a spinal injury I've had years ago. Anyways, I am wasting away so I have been trying to combat it with weightlifting but I can only workout certain muscles at the moment due to lack of workout equipment. Will the ems unit help my muscles to stimulate and grow? Thanks.
It really depends on how much of a spinal cord injury you had. It can help retrain muscles, but it will not grow muscle if they are not able to grow on their own. Since they are fairly inexpensive, it might be worth trying though. Good luck.
Hello Dr. Jo, I really like your videos. May I ask Why did you have a Hip Surgery? I am 44 years old and I had my Hip replacement 6 years ago, it helped a lot with mobility and it extended my right femur improving the length of my leg, it was shorter since I broke my femur when I was 9 years old in a car accident. Usually I can walk without pain but as soon as I walk more or try to exercise, I start feeling pain. I just bough this machine to improve my legs muscles for strength and better appearance. Hello Dr. Jo! I'm a 45 years old male and an avid cyclist for nearly 25 years, and as of a year ago underwent cervical diskectomy c5-c6 and now how having issues with the facet joint region, possibly arthritis, will find out from my pain management doctor this week. Anyhow it appears the symptoms are affecting my left mid trap/rhomboid area casing muscle knots compressing the medial nerve down the left arm at times. When the pain subsides there the inflammation pain moves across the top shoulders into the back of the neck. Wondering if you may have experienced any of your patients with similar issues and the post results? I'm hoping an EMS FlexTone will help with the knots, thank you in advance! Vic
Most chiropractic offices seem to use (EMS) when people are injured (car accidents). When you’re injured, you feel pain right…? So seeing that EMS is for muscle rehab, why does EMS seem to be used more often in a chiropractic setting than TENS?
The way I see it, is that people feel pain regardless; and anyone looking into an EMS / TENS unit, is probably doing so because they feel pain somewhere. So it would seem as if TENS should be used in any case. How do you know if you should use TENS (block the pain) or use (EMS) stimulate the muscle. TENS sounds like a temp solution, but what will EMS do for the long run?
And to clarify that question, I guess I’m basically just asking, how do you know if you need a pain management solution vs a muscle rehab solution…?
I can only assume that you have to determine whether or not your pain is a muscle related issue or a nerve related issue…
I don't really know what most chiropractors use, but many of my patients talk about some form of TENS being used if they see a chiropractor also. They might call it "e-stim" which can be a number of different currents, but if you are not getting a muscle contraction (or at least trying to), it's not EMS or NMES. EMS can be used for pain control as well since technically retraining a muscle should help decrease the pain. So it is really injury dependent. The best way to find out what you need is to go see a health professional first, even if it's just for an evaluation.
Great video. I had abdomen surgery about 10 months ago and the surgery caused some nerve damage. I believe the nerve damage is causing my abdominal muscles to weaken. I thought I would use the TENS to hopefully stimulate the nerves and the EMS to re-strengthen the muscles. What are your thoughts about this approach and is it possible to cause damage to the muscle with an EMS if its in a weakened state?

I do strength training with resistance bands & strengthen with my core. I believe I have a herniated disc in my neck & lower back. Nerve pain happens a lot along with my muscles get tight. I do stretches too. For instance, I felt fine when I went to bed. I was laying on my back. I woke up with nerve pain from my knee to my ankle. Sciatica is flaring up. Same goes for my neck. I'm guessing TENS unit since I strengthen my muscles already or a combo of TENS unit & EMS.
I would get a combo unit mainly because they are about the same price, and then if you ever need the EMS, you will have it. Good luck!

Dr Jo I had knee surgery 4 days ago. I had a tear on my lateral and medial meniscus. I'm going to be doing my own home therapy I have everything I could possibly do a therapist place I was wanted to know when is it the best time to do the tens either before therapy or after

I purchased a TENS unit to help with endometriosis pain. I'm having a hard time figuring out where to place the electrodes (mine are the four common patches like in the video) so that they cross and cancel out the cramps/pain. Thanks, and you're welcome! Is your pain pretty general in the lower abdomen, or do you have specific spots that hurt more? If it's general, you can go wider with a larger area, like go out to right inside the hip bone area on the front (not on the bone), and place them at the belly button line, and then about an inch or two below those. Some people also just place them on their lower back, which also helps with the cramping in the front. Good luck.

I have an self powered electromagnetic metallic deflector inserted into my sternum by Stark industries. every time I connect the TENS or EMS machines and turn them on an electromagnetic pulse seems to fire out of my palm, could you please help me by telling me how to use these machines properly as I keep destroying my expensive furniture every time I use these?

You're welcome! TENS is for pain reduction and increasing circulation to help the healing, so that would probably be best as long as you are cleared by your doctor to use it on those areas.
I had knee surgery back in December. I guess because of the pandemia they didn’t send me to therapy. My does not respond normally and even it locks and pops. I’ve been feeling my leg muscles too weak. If I walk for long periods , my knee starts to hurt bad. Do you think a TENS/EMS unit can help me with this?

You're welcome! The original ones (the type II that you needed a prescription for) had red and black wires for positive and negative charges. The red (positive) would be placed closer to the body/torso, and the black (negative) would be placed farthest away from the body. For most of the ones now that don't need a prescription, that doesn't apply anymore (unless it's indicated in the manual), and it's not a big deal if it's not placed that way, but if you do have two different colors, it's probably worth putting the red closer to the torso.
My PT is using both of these types of therapy on my knee ...sadly the quad muscle is not contracting...I allow the PT to increase the intensity nearly beyond what I can tolerate so we can get this quad to contract but still no contraction
What kind of injury did you have? Maybe the PT just needs to move the electrode placements around. Are you using Russian current for the muscle stimulation?

It might just be the electrodes need to be moved around some. Sometimes it takes a few tries in different spots to get on the right area.

I have used a tens for years when my back is out of whack and it was suggested that I leave it on all day. What is your opinion on wearing it for that long? Obviously it is the small, portable unit.
+Kristi Campbell I wouldn't recommend it, but that is just my personal opinion. It is really meant to get you through bad pain moments. If you use it long term, then your body almost builds a tolerance to it, just like long term medication use, so then it become less effective. Good luck!

Surgery was March 12th 2018. I am out of my cast and into the CAM boot which I can take on and off but can not weight bear unless I am wearing it. I have started with PT and I am doing exercises without the CAM boot while at PT to regain mobility and strength. I am currently at a 1 inch calf difference and PT said that will only get worse. I am a very fit person who would really like for it not to get worse so I'm hoping EMS with as tolerable seated calf raises might help. Thank you for your response.

They all do very similar things, but in very different ways. TENS and EMS are actual currents that not only help increase circulation to an area, but it also help relieve pain. The Thermotex infrared heat uses light therapy to help "heat up" the area which increases circulation and helps relieve pain. So it really depends on your specific needs and how far out you are from your injury.

Back in September 2nd 2015 I had a heart attack and ended up having an emergency operation.
I had a triple heart bypass and the vein used was from my left leg, cut from ankle to just above the knee.
Since then I have had knee aches and pains and back aches and also a lot of gout too.
One good thing about it is that I am still alive though. Would the TENS benefit my aches and pains? It is painful when I walk now on one side of my hips but I do my best to plod on.
I am 67 now but still young at heart and I want to be able to get around without knee and hip and back pains.
Unfortunately, I don't know for sure without getting to evaluate you, but I think it's worth a try. Good luck.

I received a very similar TENS/EMS unit today, but am curious which functions are actually TENS and which are EMS as they are all labeled as different forms of massage... The manual is a bit sketchy at best... Can you identify which are modes are TENS and which are EMS since the manual doesn't do that?
Actually, I bought the Tec Bean which is listed in your online store. I'm just looking for direction on which settings are truly EMS versus which settings are TENS, as that is incredibly important for me to know before I even attempt to use it.
If I was diagnosed with extensor tenosynovitis of the foot, which settings should I use in a manual TENS machine? Thanks doc!
They are all pretty different, but with TENS, it's really about what is most comfortable for you since it is for pain control.
hi, i saw your video and it was very helpful. I currently came back from a very hard hike and ever since then I have been having bad kneed pains. I was just wondering if you could tell me if there is any way my knees will heal so i will be able to hike again. Im wondering if you know how long it takes to heal from knee pains from hiking. I have never had it before.:-) thank you.
Unfortunately, without getting to evaluate you, I really don't know what it could be. If you just irritated your knees, then you should be fine with some gentle stretches and exercises. Good luck.
Our unit works great it is both tens and ems. However the setting for ems says to use chinese acupressure chart..is that true? For back shoulder and back issues.
I'm not sure what you are asking. EMS should be used on the specific muscle(s) you are trying to retrain/strengthen.

It might help some, but usually EMS is for when you really can't activate the muscle much yourself, like after a surgery or serious injury. Good luck.
Can you please explain the difference between a NMES (Neuromuscular Electrical Stimulation) unit and a TENS unit I have a dual unit that offers both modes.

hi , nice video.. i ve had a hip replacement surgery on left n right is waiting n have strong pain due to the bone deterioration on right (Still to be operated ). couple of questions:
1. what is the best machine to use for operated hip ? also is it safe with having metal implant?
2. Would either of the machines help for my unoperated hip bone pain ? Also do i need to be careful on where the nodes are placed ? i am not great with biology :)

Thanks. If you are looking for pain control, the TENS unit is best. It is okay to use with metal implants as long as it's not a pacemaker or something like that. If you are still in therapy for your first surgery, your physical therapist will be able to help with the placement. Good luck.

it would be helpful to show us which "one" has the contraction. I ordered a similar device which says its ems but there is no clear indicator how to do that.
What do you mean by "one?" The muscle that contracts or the unit that does the contraction? Unfortunately, each unit is usually pretty different with it's settings.
I have cervical injury....it helps me or not?
I have been bedridden for a few months with an impinged nerve in lower back which means can't stand or sit for no time at all hardly. I ordered a tens machine off amazon to help with my lower back muscle that has been basically locked on for these past months due to the impingement. I forgot about my legs wasting away until today they are aching real bad, I never had developed leg muscles to begin with (chicken legs) so really didn't have much leeway I guess for being laid up this long.
Would the EMS unit help me to rebuild these atrophied quads? Or would I be ok with using those exercise bands to work the muscle and then use tens to make the ache afterwards disapate?
If you are able to contract your muscles on your own at all, that's what you should try to do. You can do something like a quad set or a straight leg raise with the EMS, but you always want to try to actively move your muscles as well. Good luck.

Chris C you need good chiropractic adjustments once a week or so until you're able to stand straight again. I know they can be pricey but im sure you can find a good affordable & i promise you won't regret it if you find 1 who knows what thwy like are doing. for the pain though i dont see why the unit wouldn't help.

I have severe diabetic nerve pain in my calfs. I just got a TENS and EMS unit. I am assuming I use the tens for pain muscle relief and my goal is to get off of pain killers. How. do you determine what # level setting to you start at (1-150) and there are four categories (Burst,Normal, Pulse With Modulation & Plus Rate Modulation). Which of the 4 is best to set it on. One last question. Can these units be used a preventative, like starting of the day or afternoon with no pain present to help in eliminating severe pain coming on. Thank You
+William Snyder Yes, you would use TENS for pain control. It really doesn't matter with the setting a whole lot. It depends on what setting you like and what feels best for you. You want it to be strong, but not painful when it is on. I don't think it can work like a preventative, but it is definitely worth a try. Good luck.

The TENS unit might help control pain some, but honestly, the best thing for arthritis is exercise. If your mother has multiple spots that hurt, aquatic therapy in the water might be great for her. Good luck.

+Kenny Ho I would say using both. However, I really don't think the EMS will strengthen your muscles more than what you can do on your own. It is really for rehab and recovery when your muscles aren't quite engaging on their own yet, but it might help some. Good luck!
Hello dr Jo. I am a recently trained Doula and Childbirth Educator and I have a couple of questions.
One would you advise using a TENS unit for mommies with back pain or hip pain?
And two, I am wanting to simulate contractions to demonstrate labor contractions and how to properly count them. Would you recommend this EMS for this demonstration?
EMS is contraindicated for women during pregnancy. TENS can be used if cleared by their doctor, but you do have to be careful where it's placed since some studies show it could induce contractions.

Hi Jo, I have had a persistent chest injury for about 2 years caused in the gym. Long story short, I did not give it enough time to heal in the weeks after the initial injury. Now there is a weakness in the muscle fibres which causes it to re-injure very easily and was wondering if using a TENS machine will aid recovery once I have worked my chest in the gym?

TENS is really more for pain control, and some to increase circulation to the area. I'm not sure if it will help, but it might be worth a try. Good luck.
Dr. Jo, do you know how one can find the motor point on a muscle with EMS? I was informed on it, but wanted clarification. I don't want to get an electric shock to my heart. My understanding is that you put one electrode down on the Pt. and then one in the palm of your hand. You then turn the machine up to a light intensity and run your finger down the muscle until you feel the electricity at its strongest point? Is that the (safe) way to go about it? Thanks.
If you don't have a pen-electrode, you can do it that way. You might have to wet your finger slightly for the current to go through, but it won't hurt you either way. Good luck.
Hi Dr Jo firstly let me congratulate you for your magnificent job that you do with this channel. I practice Kendo which requires a lot of strength on the knees as there is a lot of power and also landing after very short and intense movements,sadly I injured my knee ligament a couple of years ago and know I've been told that I need to strengthen my knee if I want to continue practicing. How would you position the electrodes to make it work?

If you are looking to strengthen, EMS is best. I would use two electrodes over quad muscle belly, maybe 3-4 inches apart. You want to be able to get a muscle contraction. Good luck.
I have a Massage mode on my TENS Sports 2 machine, and I really like having it as close to max intensity as I can handle. It does of course contract the muscle, especially on my quads after a long run. Is this ok to get a intense massage in? Or am I fatiguing my muscle too much?

Yes, since TENS is for pain control, you can use it several times a day 3-5. EMS is actually working the muscles so you don't want to over work them, so probably just 1-2 times a day. Good luck.

I have MS and need to avoid atrophy of right leg muscles. I swim every day but I don't think the muscles are getting enough stimulation. Could I use a TENS machine to help?
I tried the Walk Aide that shocked the peroneal nerve when the knee was bent but found it pretty distracting and painful as it jerked my drop foot with the shock. But I liked the idea of stimulating the nerve- just not the price tag of $5K. The TENS units are much cheaper and would they do the trick?
TENS units are really for pain. EMS units will help contract the muscles, and might be worth trying.
Dr. Jo, I suffer from OA, especially my hands, I use a TENS on my chronic low back pain and absolutely love it, but I cannot get any relief for my hands. Most of the pain in the base of my thumbs. Any suggestions?
Depending on the electrodes, you can cut them to be smaller as long as you don't cut the part that has the wiring. Then you can get it closer to the area since it's smaller. Do you have the square shaped electrodes?
Yes, I have the 2" square electrodes, I place one on the palm of my hand and one on the joint above the thumbs base, Set it to 120pps and width at 50. I have very large hands, most gloves I have to buy are X-large. I also use the compression gloves at night.

You want the knot to be in between one channel (two electrodes).

If you think it's coming from your back, I would still focus on treatment on the back. If you want to try it on your foot, depending on how big the pads are, maybe the top or bottom. Sometimes it takes a few placements to find the best spot that works for you.

Correct. You really shouldn't get a contraction with a TENS unit. Sometimes on smaller muscle groups you will get occasional ones, but it shouldn't be constant.

I always love your videos-very informative! I do have a question: is EMS good for muscle knots/spasms? Would the electrical contraction from EMS reset the already over firing of the muscle fibers or make it worse by contracting more? I use a TENS-it helps, but I'd really like to relieve knots so I can exercise to build up weak muscles in my lower back due to scoliosis.
Thanks! I don't recommend using EMS on the back since some muscles cross the spine. It could cause contractions you are not wanting.

I have spinal stenosis in the lumbar spine as well as sciatic pain. I donated a kidney 19 years ago and part of the incision herniated so my muscle tone in my abdominal is not good. The sciatic pain is always on the side of the weakened muscles.
I have used deep tissue massages and Vinyasa yoga for the last 3 years to manage and keep things flowing... However lately it doesn't take much to set it off.. Am wondering if TENS is an option?
It might be. With spinal stenosis, extreme extension of the back usually flares things up, it might be to a point where the Vinyasa is irritating it instead of helping it. Good luck.
