    I'm a manual therapist that does a lot of myofascial manipulation. I liked the video but there's a part of rehabbing a musculoskeletal injury like a labral tear that I never see addressed in anyone's videos that I think is worth mentioning. That is muscle guarding and the compensatory patterns that develop post injury. With a labral tear there will often be an increase in muscle inhibition with some/all hip rotators, flexors and extensors. In addition to the strengthening exercises like in this video, I've found it very helpful to restore some hip balance by reducing the hypertonicity in the muscles especially the tfl, glutes, deep hip rotators and going just above the hip, the QL. If you can get the hip to release from a muscle guarded pattern some pain levels generally drop and rom increases so strengthening exercises will be beneficial even more. Just thought to add to the discussion of your great video. Cheers

      I have both a posterior and an anterior labrum tear in my hip. The muscle guarding, and resulting muscle spasms, make this a debilitating injury for me.
      I couldn’t agree more on strengthening. It has helped with becoming stable and mobile, but I’ve only ever been able to get temporary relief from getting the muscles to relax and release before the cycle simply starts all over again. What sort of muscle release exercises can be done to better address this muscle guarding component?

      @@clintcollins7366 I’ve literally never heard of muscle guarding but I can feel my hip locking up and constantly popping. I’ve tried strengthening as well. If any muscle guarding techniques are mentioned, I’d be so thankful!

      @@CultureIsKey muscle guarding is a term for when muscle and connective tissue stiffens and under performs as a way to protect an injury. Surgery is often performed for the kind of labral tear you describe. Hopefully you've talked with your doctor/Ortho/PT about the issue. Maybe there's non invasive ways to manage it. You might look into prolotherapy, prp and stem cell treatments too but I couldn't assess your particular condition through posts like this. Best of luck to you.

    I had a labral tear in both hips at the same time. I did stem cell injections into each joint (at the same time) in February 2022 and it worked wonders for me. Not 100% healed at the moment, but probably close to 90% and I think I’m still healing, so hoping for 100%!

    It happens frequently when people have hip dysplasia. I am 38 and I have two dysplastic hip joints. I found it out just a few weeks ago, after seeing a hip specialist. My labrum looks very fine though, but my orthopedic doc said that I am more vulnerable in this area. I would advise people to always get a front x-ray (pelvis front view, both hips need to be visible) and look for the LCE angles. In Germany this is all covered by general healthinsurance but it is often overseen by a lot of orthopedics, because the symptoms are so unspecific (people have sore hip flexors, lower back pain, piriformis or deepl gluteal syndrome..) symptoms, because the dysplasia is often the cause for constant functional overload of the muscles around the hip.
    PT & Training helped me keep my labrum intact, but to fix HD, a surgery is usually the way to prevent any further damage of the joint (PAO).

    This was very helpful. I used to do yhe splits all the time especially growing up, my mom thought it was amazing and would have me show every one; now as an adult I have extreme hip pain, popping and catching. I go to the chiropractor all the time to have my hips adjusted but it just doesn't seem to last very long or even help at all as of the past 2 years. I've been doing more research the past 6 months or so and it seems that this explains more of what I'm experiencing than anything else. Joint pain seems to be the main character in my story the past 2.5-3.5 years including two acdf c5-c6 surgeries the past year and four months. 40 years old feels a lot older than what I expected 🤔😒😉 (funny because it's true)

    I’ve successfully torn both of my labrums. The right in 2020 & the left in 2021. Mine are both results from falling due to Multiple Sclerosis.
    My legs have pain 24/7 but the pain I experienced from the tears was much worse!! I couldn’t straighten my legs and the pain would go down the front of my thighs. The pain never ever changed until I was able to get cortisone injections. My hips have become so stiff that I can’t pick my legs up. My right side rotates completely outward. Unfortunately, I think it might be time to go down the surgery route!! Never would I have figured that almost 20 years of falls would affect my hips!!

    on april 4th 2016 i tore my right hip labrum and shredded my left hip labrum. it took me 16months to get mris done because doctors REFUSED TO HELP AND TOLD ME IT WAS MY BACK THAT MUST BE INJURED. i truly have no idea how my legs even work still, yet they are STILL REFUSING TO HELP and unfortunately anyday im going to be unable to stand. my right hip needs to be replaced ASAP but there is not even any drs working on my hips, trying to work on my hips or going to work on my hips. ive been doing PT for months but cannot do PT any longer because it has only made everything worse. love your channel and show unfortunately for me it has not been able to help much either. my right leg has collapsed or caused me to fall over 25times since 2020 and is only causing more injuries to other parts of my body and definitely getting worse each and every moment? i am at a complete loss because ERs refuse to help and DRs continue to refuse service, any help, ideas or suggestions would truly be appreciated?! thank you both truly such wonderful and beautiful people, love what U do for the world.

      Hi Scott, I'm truly sorry to hear about your experience. The best thing I can do is recommend seeing a new orthopedic doctor to try to get help and surgery you feel you need. Please know that not all physical therapy is the same and just starting with breath work (I do have some videos on this page for this) can be very helpful to at least reduce pain in the moment. You can also do some gentle nerve glides or mobility for the hip, as well as some basic strengthening that doesn't require you to load or place much pressure on the hip. I hope you find the relief you are looking for soon 🙏

      That’s terrible! It took over a year for me to get labrum tear diagnosed. It will take another year for me to get it replaced. If you are under 40 years of age there is orthoscopic surgery available. That’s how it works in Canada. Good luck! Try the steroid injection into the joint. It helps with the pain a great deal! Go to a pain clinic for now until you get it fixed.

    I had excess bone growth on my right hip which tore my labrum. I could barely walk at times and had clicking in my hip when I would raise and lower my right knee. I tried PT and Ibuprofen, but moderate menstrual cramps would undermine the progress I was making with PT and it was like back to square one--severe pubic bone pain and inability to walk. So In November 2023, I decided to have surgery to have the excess bone shaved down and the labrum repaired. For me, I had to choose between a poor quality of life that would last who knows how long and a temporary recovery/short term disability.

    I had a labrum tear, that pain and movement issues lasted almost a year. After that year of rest, I went to PT for therapy. Then after much rebalancing practice on cussions and thigh and calf strengthening, they put me on an upright cycle, and within 3 minutes, I got sudden pain in the same labrum. Bertolotti connects the sacrum to the spine (with a bone that should not be there), in my case it's rubbing and not fully fused.
      Anyway, the past 1-2 weeks I've been able to walk a 5K per day. I tried pushing it and going into a jog for 10 seconds at a time and I still get muscle spasms in my hip. I might just be a walker, if I can walk a 5K a day I won't ask for more in my life relative to how I couldn't move most of the last year.

Although I still have a long way to go, pain levels have already dropped. Much better than before surgery. My suggestion - find a really good hip orthophedic surgeon that has tons of experience and that works with athletes and older people so you can get a good diagnosis. FAI arthroscopic surgery success rates are actually quite high. It just takes time to recover. About 6 months to 8 months to full activity.

However, depending on the severity, there is a lot you can do to continue to regain full function and minimize any long term symptoms. Hi, I have been suffering from the same issue for my left hip. I had a surgery in July 2022 i had FAI in the surgery the tear has been repaired and femur head has been shaped. Unfortunately my pain never let me go off i had 3 cortisone injections at the end in February 2024 I had another MRI bang there is another tear in the same hip in different positions which is anterior. And also i have been dealing with psoas tendon clicking. It looks like i will be having another surgery. Do you think does it worth it?

    Is "core control" favoring towards posterior pelvic tilt? Like the hips, pelvis, ribcage more stacked?

    • @docjenfit
      @docjenfit  ปีที่แล้ว

      I guess core control is more about finding dynamic support through our core regardless of positioning. Everyone has a slightly different pelvic tilt, so rather than focusing on that, try focusing on feeling that core support and control through other dynamic movements.

      Not a doctor- but literally in the same situation so here's my story if it helps you: my hips were crooked and had to be adjusted back to normal. So many things could have caused this, but it basically meant my leg bone was jammed up into the socket and shifted my hip. I got my hip adjusted and my legs are now the same length. Currently going through treatment for the tear though. Hope this helps but definitely see a doctor or physical therapist

    • @docjenfit
      @docjenfit  ปีที่แล้ว

      Yeah our labrum isn't even nearly two inches thick, so it must be something else functional going on. I would recommend working on hip mobility from side to side to see if there are any major restrictions that could be causing you to guard differently on that left side.

    • @terivansteel3425
      @terivansteel3425 หลายเดือนก่อน

      Spine, sacrum, etc., can be out of alignment, and with adjustments, it can be corrected. Most people who think they have uneven leg length have this type of problem. Your muscles will want to pull everything out of alignment again, so physical therapy and rechecks would be needed until stabilized

    Im not sure what is wrong with me. Can a labral tear refer pain to the nerves in the inguinal canal? My right testicle always feels sensitive. And it always scales with the pain in my groin. I'm told I don't have a hernia. And I've already done physical therapy and it did nothing. I've also gotten blood and urine tests and nothing was found. Standing for a while always seems to agrivate my groin as well.

      Hey there! I have this exact same experience right now! I have no idea what it is. I’ve had ultrasounds, CT scan, reports came back with no findings, urologist says doesn’t see anything, another dr. Who specializes in Varicocele Says he see a small inaugural hernia. I have a really good massage therapist coming over tomorrow. Whom I spoke with today and explained to him what I’m experiencing and he thinks it could be a labrum issue in addition to the hernia.

    • @docjenfit
      @docjenfit  ปีที่แล้ว

      A labral issue could definitely contribute to the tightening up of muscles around the hip and inguinal area. So there's definitely a chance it could be contributing to this. Maybe try working on some hip mobility that is tolerable and opening up some stretches in that low abdominal/inguinal area.

    I’m really hoping and crossing my fingers that I get an answer. So my pain does not go to any part of my leg it doesn’t go to my back it is literally a single point right on the top right hand side of my pelvis area. Right on top of the pelvis bone may is where I feel excruciating pain. I used to run and walk and rollerblade a lot and do peloton.
    When Covid I pretty much stopped everything and gained 30 pounds. I had this pain five years ago but I got into great shape and the pain went away but after gaining 30 pounds it has come back with a force and I don’t know what it is because it doesn’t fit the description of anything I’ve seen so far because it doesn’t extend on any lower region. It’s just the tip of my pelvis. I have ice to ice put electric shock on it. Nothing really helps any idea of what this could be? I’m really considering going to a doctor to get a diagnosis? Do they do an x-ray or an MRI or what other tests would they perform? Sorry for the lengthy post but I am in pain and don’t really understand what it could be.

      This same thing happened to me about 3 mths ago dancing to mexican music very scary feeling then I stopped dancing and tried to walk back to our table as I walked I was limping and with a weird feeling. I not knowing what to do decided I'm going to self test and stretch to see how bad it is 😵 I'm glad I was fine I did a squat and no pain, I bend down and no pain but I tried to lift my foot and I couldn't even do 5in. off the ground back kick wasn't as bad but still not normal. Walked upstairs no pain but as soon as tried to lay down the pain was almost unbearable stayed on my back used a back massaging heated pad wrapped it on my thigh doing 15mins every hr. Until I fell asleep. Next morning I tried to go downstairs and it was painful made it to the couch wrapped it and turned on the heating pad and next day I made it into work, I been doing the butt muscles exercises and they've worked so far. Got back on the dance floor not long ago and I did ok still concerned because if I walk a distance I feel a pinch in my hip off and on but not severe, circular motion exercises while sitting has really worked for me. Hope your doing better now

    Mine started with a fall. That was 6 years ago. Mine are now on both hips lower back like you made the C. It actually feels like the muscle is tearing. Sometimes it migrate down into my groin. My doctors have always been looking at my back bone for the problem because I’ve had trauma there before also.I’ve always known it’s been my muscle. Do you feel like this is labrum tear? Finally getting diagnosed but one doctor says labrum tear. The other doctor says chronic muscular pain disorder. Help!
    PS. Ever since it’s happened I’ve lost control in my legs. If they are straight I can only lift them about 12 inches off the ground. If I turn my ankle in side pushing towards the other foot it only goes about 3 inches. This is now in both legs.

    I find this confusing on how to apply. I apparently keep reinjuring my labrem tear. Did it during yoga and have never been right since 2017. And I have a tendon tear from sit bone to thigh. I can hardly do anything now without pain. I've done 2 rounds of rehab, but just doesn't really get better. It's hard to sit and do nothing as I'm so active. Everyone seems to have a different opinion on what should be done. I think it's a whole body alignment problem, but nobody wants to address that.. they only want to address a single area. Once everyone realizes everything is connected and treat the whole body, nothing will get better for good. imo

      Totally agree! We tend to have these issues pop up because of restrictions elsewhere in the body! I agree that everything is connected and we can't really create true change unless we find where our baseline restrictions are everywhere and start working on them!
