Exactly what I theorized, as well as many others. The caretaker was likely a young vampire of lower generation, one who didn't know old peoples and childs typically cannot control themselves or the hunger frenzy when they first awaken. She acted in pity/compassion, most likely, a noble act in her mind. And it ended in tragedy.

Youger vampire are still very much human and really any clan could feel pity and mercy toward and old soul that always and only lived for the sake of other. - Brujah or Gangrel could be revolted by a life of servitude and be tempted to offer her a second life/chance to rise above her condition. - Ventrue or Tzimizie could see a perfect servant that would ever even think of betraying their master (long history of lords ruling above their better in those 2 clans). - Toreador or Follower of Seth could see such a beautifull tragedy and be curious what would happen if said tragedy would become eternal.
My guess based on what she said, what the scene looks like, and what some people said in the comments on the original video: The old woman voiced her regret about not having spent a day for herself, her caretaker (who's a vampire) pitied her and Embraced her to give her a whole eternity to live for herself, but didn't account for the fact that newly turned vampires are prone to going into a Frenzy upon their first awakening, especially if there are living humans around them. Thus the old woman killed her family to sate her unnatural hunger, while the caretaker either fled in fear/shame or was killed by the old woman (either by accident during the rampage, or deliberately when the old woman fell into a rage over what she had been made to do).
Oh. That is darker than what I though. My belief was that the caretaker killed everyone else and just left her around her dead family. I thought that was dark, but that makes more sense while being way darker

I wouldn't call it a coping mechanism because he really is insane. Most likely he acted the way he did in this short out of decency and respect for the deceased.
I think it's just Big D's personality, not a coping mechanism. I can relate to him because I whip myself into a manic frenzy of confused thoughts like he does. Where I think, reassess, re-reasses and reconsider what I might've missed. And I neurotically worry if I overlooked a threat or miscalculated something. He's just built that way. It's not insanity, it's mania, which has been reinforced by long habit because he's be in the game for an insanely _long time._ And unlike him, I haven't faced any real traumas or survivor's guilt. He values knowledge because any small grain of insight offers the possibility of outflanking and evading powerful enemies. And I get it. Fault and responsibilities are different things. Sure, the death of your loved ones is not your fault, but it is your _responsibility._ If not you, then nobody else. And all because you were lacking. One moment of unguarded carelessness. A moment of naivete. Ignorance. Always, you could've done better. Zigged left instead of zagging right. A neighbor of mine died in a gas stove fire. I did nothing because I thought as a civilian does. It was not my place. There was somebody better-trained. Maybe the backdraft was too dangerous to attempt a rescue. Firefighters arrive. But I never even attempted to explore whether or not that was the case. Maybe all that got my neighbor was prolonged smoke inhalation. Or the fire maybe was not so bad yet if we had acted sooner. It wasn't so hot you could feel it standing near. I could've pitched a rock through the back door to test the environment. Another man was on hand to lend me strength. I did nothing. So she died. It wasn't my fault. But I chose not to take responsibility. It hits hard once you come upon the realization that inaction is also a choice.

If you look closely, you can see that Carl has the same jacket on that big D always wears, (specifically at 15:36 )with the puffy collar and everything. And he was all serious this time, much like big D, no craziness for either of them, with a similar look of eachother, while maybe mourning a loss
Going out on a limb here, at the end of the video we see Karl just sitting there. No funny screams or jests. Just sitting there depressed. I think he lost someone recently. Someone who really cared. And considering how Big D helped her into the morning sun and how she wished she had died yesterday… I think he may have had to make the decision to pull the plug. Correction: It was someone else from the team.
This Big D moment shows off many of his abilities, he must’ve smelled the blood on the wind if, how else could he figure out a location of a vampire attacks. His ability to read the room, if we were to look through his eyes we would see him look at the room then the blood and the old lady. Calculating time of death, how it went down and then noticing how the radio should not be playing at such time. Finally besides his moment watching the sunrise with the old lady, is how he cradles the radio. It slams to turn it off but his hands stays on as he removes it, like he feeling the good dreams that were once played.
My guess: the caretaker was a young vampire herself and probably had a decent bit of humanity which motivated her but... my guess is that she was also a Caitiff since most of those tend to not be the brightest when it comes to Embracing. The sire might have had good intentions but... not everyone, even those who willingly seek out vampirism in WoD, have the best coping skills especially during their first night when their hunger is just that overwhelming.
The embrace and the first night is never fun times. It's one of the reasons Camarilla is so strict about embraces- they want to make sure the Childe doesn't break the masquerade. One of my favorite embrace stories from the group I've played with was my buddy Mathew's Lasombra character, Michael, who was given their own sister to feed on by his sire.
So many other media depict vampires as either nigh unstopable monsters or misunderstood sex gods. I think that the Life Well Lived short shows the honest and real tragic irony of being a vampire. You become a nigh immortal being that will never die and yet you still live on borrowed time. And when you die, all that remains is some unidentifiable ash and if you still have living family and friends, they will most likely never know your fate and thus can't mourn your passing.
For the record, the train they were on during the argument was the "smallest railroad" that was mentioned in the end of the police interrogation audiodrama. They commandeered it for themselves as they are renewing Guy's blood bond.
Then again this video could be connected to the story karl was telling as an after credits scene in the episodes? It could have been that "grandma" there is the old woman that got briefly turned here but when she was on a frenzy and anything she said in karl's story was any bottled up negative emotion she had for the character that was telling the story who might have survived due to grandma coming to her sense and realizing what she has done only after feasting on the rest of the family, and now that he is an adult he is living with regret, which is why you see him hunched or sitting at the end alone with the "Shine On!" Sign at the end? Note: this is just my theory and interpatation of both scenes together.
When it comes to Hunter, be it Reckoning or Vigil, one of the things that gets a lot of focus is always the why. Why does your Hunter Hunt? For most of them, it comes down things like rage or vengeance because one of the Night Folk hurt you or someone you care about. Other times, it's curiosity, the need to shine a light in the dark and see what's there. Between this short and D's interactions with Kevin, it shows his empathy and compassion. It makes me think that D is Hunting for lost souls. Not just for the people hurt by monsters, but for the monsters themselves. He's not the sort who papers over or forgets what they've done, but he wants to them to remember who they were and who they can be, not just what they are. When it comes down to it if they each killed a vampire, I think that D would mourn the person lost, Markus would mourn the chance for knowledge, and Door would mourn not at all.
Even with someone as unapologetic and monstrous as Pyotr, Big-D was considering making a diamond out of his ashes, "to show that even the ugly of us can be made magnificent". And as back-handed as it is, perhaps that was Big-D's attempt at giving the vampire one last redemption in death? Or, perhaps, an eternal reminder of the person they've killed...
A Life Well Lived shows that Big D isn't a hermit or a wayward, as hermits and waywards are driven insane by being in the presence of the supernatural. Hermits isolate themselves where as waywards are driven into murderous rampages.
The halloween video was released on 1st of november at least on swedish time. the 1st of November is All Saint's Day (though it's techically on the 5th this year) still. Its a day of remembernace of people who have passed away. Like grand parents and so on. So the theme sort of fits
