I went to a podiatrist once with pain in the area of my arch. He gave me exercises but warned me that with my line of work, being on my feet all day, and running around like a banshee, that eventually...my foot pain would travel to my knees and my hips. I quickly disregarded his comment and went about my business. It took almost a decade, but....he was right!!

This is such great info and I can personally attest to the significance of losing weight and changing your gait to significantly decrease pain. I have bone on bone OA in my right knee with awful pain for several years. After numerous doctors, shots and physical therapists, a Pilates teacher told me the way I was walking was probably increasing the pain. She worked with me on my gait and strengthening all of the muscles in my legs and core and three months later, a HUGE difference! Thank you for all of the details in your lecture!

@@rathindranathdassarma5560 Very long story shorter: after many physical therapists , doctors, injections etc for my bone on bone arthritis I met a Pilates teacher in Florida ( Gola at Pilates Haus in Jacksonville) who really paid attention to the way I walked. Like a lot of people I walked with my feet ever so slightly turned out. No one else had ever noticed and it was very slight. But if you looked at the wear and tear on my shoes you could see that! So she taught me how to walk so that I put far less pressure on my knees & we worked on core, leg and hip strengthening. I had to learn to literally turn my legs in -from the hip joint, NOT from the knee and then focus on walking very deliberately heel ,toe, heel toe all with my legs turned in enough that while it looked straight, it felt like I was walking pigeon toed. Then she worked with me on strengthening all of the hip and joint muscles, walking straight with my shoulders back and my hips slightly forward. And I practiced. In my 60’s and I had to retrain my body to walk the way we were built to walk;) Btw, Gola happens to be an especially gifted Pilates teacher. I’d taken yoga and Pilates off and on for years with no improvements. I just got lucky . I wish I could describe the process better; I suggest you try on your own a bit just make sure you ONLY move the direction of your feet slightly and from your hip /leg socket NOT from your knee or ankle. Your entire leg must be in that position as you walk , straight and heel, toe, heel toe. Good luck!
I am 39 with right knee OA. I have been daydreaming of affording Pilates for this reason! Pt is so impersonal and they just throw a tech at you. I hope to find an amazing Pilates teacher to help me too 🙏
I'm encouraged with Dr. Fredericson's comments about patients with bad X-rays being able to manage their inflammation, and those with good X-ray results having a lot of inflammation - which to me indicate there is a lot those of us with knee pain can do to adapt our lifestyles.
I am 76 years old. I was diagnosed with osteoarthritis when I was 60. My orthopedic doctor told me to do exercises to strengthen all my leg muscles that support the knees and to do them daily, for the rest of my life. I am a vegetarian but still have eggs and dairy. Staying low Carb also reduced my weight very effectively. I am monitored for osteoporosis and been on medication for decades. I did take Chrondroiton and Glucosamine for a few years but then it was ineffective after a while. So only the exercise routine has been effective. One has to be disciplined to do it daily. Thanks to this, I have been trekking in the mountains (four continuous days in April this year of 5-9 hours daily) and my knees did not let me down. I do use knee supports for protection. I am very grateful to my doctor for his sound advice.
@Maya Maker Some people might have the diagnosis of osteoarthritis on images and x rays incidentally but can be asymptomatic. Was that the case for you or did you experience any arthritic symptoms of pain, mobility issues, burning sensation etc like majority of symptomatic people?
@@tr6151 I had dreadful pain in my left knee. It could have developed because I was walking too much. My knee was swollen. I had to have physiotherapy. Once that swelling reduced, I was prescribed daily leg exercises and I have to do them for the rest of my life. I have added to them so now it takes about 20-25 minutes. When the exercises got easier (which means my muscles developed) I added ankle weights. I am now at 1.5 Kg on each ankle.
Very informative, thank you. This is proof that there’s a lot to try before going to surgery. My neighbor had the surgery. He has problems with range of motion. He is going back in for second operation. Good thing he is retired and has recovery time on his side.

I think the importance of dealing with ligament instability with the use of PRP and/or stem cells should be addressed as just injecting into the joint is still not helping the ligaments which is where the problem of arthritis starts. This has been used in treatment for decades but ignored by orthopedics.

So as to shift weight onto the lateral side ? Also, could said heel wedge be used in conjunction with an off loader brace ? Thank you again !
I feel when I was really young and missed up my knee . Later in life I twisted my knee , filled with osteoarthritis . And ended up in surgery for a torn meniscus. Every 3 or four months I get a cortisone injection and every 6 months Synvisc-ONE . I take a blood thinner for DVT , so there is some medicines I can not safely take . And the natural ingredients in them that may also be a blood thinner .
Glucosamine and chondroitin trash my stomach. I have talked to so many people who couldn't take it, as well as the black pepper with tumeric. Odd that you have no patients talking to you about it when I have been surprised how common these problems are.

Dr Stoner in SF has done this for one of his patients, one partial and then a year so later the other partial. I asked him your question - he said, two partials is very rare. He would not say why they dont like going this way - still dont know. Seems like a good idea to me.

Funny you brought him up. My Husband and Father follow his advice. My Husband has been able to heal a lot of old injuries through his exercises. He is 58 and has worked out since he was 19. I have watched him get in even better shape than I have ever seen him in just in the last year of following that guy’s advice!
