    Im a 41 year old man and I've used Retin a for years and years. I use it on my face, under my eyes, on my neck and my hands. As my body has aged, my joints worn out, my near vision failed, my skin has got younger and younger. After years of Retin a use, my skin look like smooth stretched rubber, not a single pimple, no wrinkle or spot. Nothing. It's become pale and freakishly smooth. People ask me all the time my skin looks so nice. It's a life commitment to me, I've not been in the sun for 25 years and I wear a Japanese spf 50 every day. I love Retin A!

    This is a fabulous video! There is so much misinformation floating around the internet and TH-cam! One area that really needs clarification is the differences in efficacy of tretinoins versus adapalene and tazerotene. Hopefully people will get the message that sunscreen is not negotiable anytime but especially when using a retinoid. So grateful to have your expertise. Hugs!

      Hi Dr. Dray, I have been using retin a for three months now every other night. I have not had any side effects whatsoever. Is my skin maybe just tough skin or something. I feel like it’s not working. Really searching for answers online with no luck. What do you think, should I just be thankful I have no peeling or irritation? Any advice would be wonderful. Thank you!       @@aizamahinay5710 Actually tretinoin is safe to use forever unless u r planning to get pregnant of u r breastfeeding. You can use salicylic acid 2% cleanser it helps with comedonal acne as it goes into our pores and unclogs it. I am also a Tretinoin user and I also use other actives esp benzoyl peroxide as it helps the trwt works best.

    Thank you! Very valuable info. I appreciate you taking the time to educate us and making us informed consumers. Now when I see those expensive face creams that advertise their retinol content miracles, I just skipped them knowing that the effects of it might not be as good. I do, however, use a prescribed retinoid, but the skin peeling continuous to be a problem and I have been using it at least twice a week since April of this year. I guess I have to be patient until my face adapts to it. Also, I live in Vegas where the climate is very dry, so it could be a contributor to this side effect. Thanks again Dr. Dray. Happy Holidays!     I purchased a Clarisonic when they first came out. I started having problems with my skin becoming very sensitive and easily irritated. I have very normal and balanced skin and this was a dramatic change. I went to my dermatologist and his recommendation was to stop the use of the brush and go back to my normal skincare routine. In a matter of a week my skin normalized and my brush sits in a drawer. My dermatologist also said he’d stated seeing an increase in cases such as mine from overuse of the brushes. This was a number of years ago. Thank you for another informative video.

    As far as I know (and most published dermatology journal articles) thinning of the epidermal layer is temporary. It eventually goed back to baseline. Overall after some time tretinoin actually increases the dermis by various means (some of which include collagen and glycosaminoglycan aka hyaluronic acid etc).

    I've been using Retin-A under my eyes for 10+ years with no problems, BUT, my skin is not terribly sensitive and I DON'T put it right up to the eyelid, but a little further down. I do not use it on my eyelids. I can't see how that would help anyway... And like someone mentioned, it can seep down into your eyes. I prefer to moisturize my eyelids. I just put a dab of Vaseline under my eyebrows, a little on the lids, and at the corner of my eyes. Old school I know, but it feels nice and doesn't have chemicals that can irritate. 😊

    1. There's no retinization on the 0.05% tretinoin, does one continue irrespective? When does one increase the dose to 0.1 % ?
    2. Can one layer moisturiser-->tretinoin-->oils?

    I started doxycycline and retin-a in September for acne. Now, December, I’ve seen a difference in the texture of my skin. It’s a lot smoother versus how bumpy it previously was. Now I’m struggling with the acne scars that are left behind...but I’m still pushing through! I go back to my dermatologist the 19th!

    Been waiting for your opinions on Retin-A and retinoids. I have seen a number of the mature ladies on here putting very tiny amounts of the Retin-A on their necks as well, so was surprised to hear that you are against this method. I have used a very small amount, mostly what is left over on my fingers from applying to my face, and do rub it down my neck. I do not experience any irritation and have noticed some positive results on my neck. Of course this is just my own experience. Thanks for posting! Maria

    Can the increase in cell turnover potentially cause cancerous cells to increase production as well? I understand when you say it can be helpful with prevention but I've read it can speed up existing cancer cells. Can you explain all this on a more molecular level? Thank you.

      I recently started tretinoin for 0.05 which is the one next to 0.1%. I didn't feel anything yet. I have oily skin btw.

    This video was so helpful! I just got started on a 0.5% tretinoin cream at night with a clindamycin gel in the morning. I’ve been super nervous about starting but this made me feel so much better!

      hi! i know this is 2 years late but i was wondering how that pairing worked out for you? i got prescribed the same things by my derm last week, clindamycin in the morning and tret at night, except with a 0.025% concentration and along with antibiotics.

      @@kykywawawa hey! so this didn’t actually end up working for me at first, my acne was pretty sever, so my derm ended up putting me on acutane which DID clear it up, and im actually back on this combo to maintain, basically. it seems to work for most people though! mine was just too tough haha

    Hello, I love watching your vids! I am 31 and have been using tretinoin for years to treat acne and now melasmas. I read that clinical trials had to be ceased because of the increased risk of death from lung diseases! Is this true? If so why aren’t we told about this when we’re prescribed it? Also, I would love to hear something about the small broken blood vessels that have appeared on my face a I age. Any way to reverse them or make them less obvious?       thanks! this should be evaluated as it could be a variety of things and so it is hard to say for sure; don't attempt to self treat without finding out as you could complicate the problem/make it worse/difficult to evaluate

    1. What is your opinion in hydroxypinacolone retinoate (Granactive retinoid) - I wrote to deciem to tell me to which retin a receptor this active binds (as claimed by the producer Grant Industries, conversion is not needed) and they wrote that it up-regulates the rar-gamma in the dermal fibroblasts. As far as I know retin a binds to three receptors (alpha, beta and gamma), tazazatorene rata beta and rar gamma and adapalene only rar gamma I think.
    Could you please elaborate what does that mean? I was reading different views of dermatologists, some claiming that adapalene does not work on wrinkles specifically beacue it only binds to the gamma receptor. There are some studies out there in which adepelene did seem to benefit the skin against wrinkles. I would like to understand what functions do these diff receptors perform and diff forms of retinoids in their ability to upregulate these receptors - what does that mean for anti ageing. And do you think that hydroxypinacolone retinoid has an antiageing effect.
    Secondly - I would like to understand in what way does the peeling effect of a retinoid differ from the peeling effect of the acids? I mean - do the acids hust dissolve the top layer while the retinoids basically not only dissolve the top layer but also make the lwer cells mature more faster because they influence the dna in such a way that they restore their youthful ability/speed to regenerate?
    I can understand that the acid dissolves the upper layer by the pH - but the retinoids dont have that acidic pH - so in what way do they cause this peeling?
    Some doctors claim that peeling is a side effect - because we dont have enough receptors for vitamin a in the skin (we loose them through sun exposure) and because of that not all vit A can be taken up into the skin so it sits there and we get vitamin a poisoning of which the side effect is skin peeling. Is that true?
    Thirdly, I have seen 10-15 years of studies done on Seletinoid G, a retinoid that is proven to work on wrinkles and also being non-irritant. But there seem to be no products available for the user (oc or with a prescription). I wonder why? On the other hand there is hydroxypinacolone retinoate being sold as reint a alternative but it has not been proven by indepedent studies that it actually works on wrinkles...

    Great Video!! At what point do you decide it’s best to graduate to the next strength of Retin-A. What indications should we look for. I also follow an Asian skincare routine that involves sheet masks. Can I use a sheet mask with the Retin-A and at what point?

      Marie cerave pm cream has 4% niacinamide in it. I use it 15 min after I apply my 0.025% tretinoin cream.

    When starting off, every other night, how many nights and for how long?
    So it’s best to moisturise on wet skin first?? Then wait for it to dry and apply moisturise?
    Can you show us your retin a routine so we see how to apply it?

      Use 2x a week for the 1st month, for the 2nd month 3x a week, and for the 3rd month every 2 days. That's how I did mine. Btw, I'm using 0.025% Tretinoin pair with 5% niacinamide + HA, lowph facial wash, moisturizer with ceramides and Sunscreen. Do not forget to always double cleanse every night. Use cleansing balm of ur liking or cleansing oil and cleanse ur face for 1 minute before doing the 2nd cleanse with low ph cleanser. At night after u cleanse ur face, while ur face is still a little damp apply the niacinamide + HA wait for it to dry a little then you moisturize. Once you are done wait for another 20 minutes before you apply your PEA-SIZED amount of Tretinoin. Once it dries apply moisturizer again (it's called sandwich method to lesen the irritation). In the morning do not forget to wear sunscreen. Hope this helps.

I am currently using Curology for my acne and it has 0.009% tretinoin (as well as 1% clindamycin and 9% Azelaic acid) so I'm hoping to learn if this is enough to help with my acne and aging, as I'm in my 30s now and developed acne the past few years.     Do you recommend using 0.04 and 0.1 at the same time. I use retin a 0.1 every three days. I have a tube of 0.04 just sitting there. I was wondering if it’s ok or beneficial to use that 0.04 on the days in between.

      If you mix the tretenoin with moisturizer then there is a great chance for the tretenoin to be spreaded unevenly on skin.
      Some parts get more tretenoin and some less.

    I was prescribed tretinoin and Clindamycin (with benzoyl peroxide). I was instructed to use the Clindamycin twice daily so can I combine them at night before bed and if so does it matter which I use first? Thank you :)

    About a month ago you posted in regards to perioral dermatitis. At the time I thought I was suffering from that, turns out I used a cream with a retinol as a moisterizer and did not apply sunscreen on top of it so that is what caused the swelling around my eyes as well and dry peeling. I read the ingredients and sure enough there it was. So strange, I got this cream at the dollar store and never planned on using it just grabbed it while I was out and needed something at the moment. Uggghhh definitely reading ingredients thoroughly from now on before applying!!

    Hi, thanks for your detailed explanation on the benefits and uses of retin-A. Our dermatologist advised my teenage son (14 years old) to use tretinoin cream to combat his acne (mild) and to apply it daily on a long term basis, at least for 10-18 years. She said it can help repair the collagen as well. Do you seriously think young people with acne should use tretinoin for such a prolonged period? Kindly advise.

    I have been using tretoin 0.01% and don't think it's strong enough. I am in Canada and that's the strength my dermatologist prescribed. I am using it for aging skin. I really want it to work. I was using hydroquinone 4% but I don't need to lighten my skin any more than it is naturally. I had this Asian beauty idea in my head and I just went and bought foundation from 3 different companies and I had to buy *the* lightest shade each company made. That's pretty darn light! I am good with just the Retin-A now.

    Great video! My derm began using retin A when she was 20 and now in her early 40's and her skin is so awesome (of course religious with sunscreen too) and I only WISH I had stuck with it years ago but sadly didn't really use it correctly and fell away from it.     I was using tretinoin cream .025..but my fave always looked soooo red and peeled. I switched to Differin ( I used this years ago as a . my derm prescribed along time ago when I was a teenager.) The redness went away almost overnight of using Differin. BUt I wish it had anti-aging benefits:)

    I am in need of ideas of when, in a skincare routine, to use my Benzamycin gel. I am 65 and have been using Benzamycin and Retin A very successfully for acne for 20 years.       Cindy Miller I dont think there is a stronger concentration unless you have a chemist compound it for you with a prescription. I have been using 0.1% for more than 30 years. I just use it everyday and use more product now.

      This is a bit late, but you must use sunscreen every single day if you're using retin A. Retin A makes your skin more sensitive and vunerable to sun damage so its necessary to use sun cream and hats/sunglasses. Also it takes many months to see the results of Retin A especially with a lower dose like 0.25. Try it again for a good 7-9 months

    can you do a men's skin care, baby and elderly. seems they have different concerns. men shave everyday and cut get cuts & rash. babies have delicate skin and the elderly with tissue paper skin bruise easily and have their own concerns. thank you.

    Greetings from Italy! Thanks for the extremely informative videos. They really have had a deep impact on my concept of skincare. I have a huge question: I've been suffering from acne since my early twenties (I'm now 31) and it seems like I've never been able to get rid of it once for all, it seems to change over the years, but it just stays. I've been recently prescribed an on the counter treatment containing glycolic acid, clyndamicin and tretinoin (0.02%) and it has worked fantastic. Here's the question: my dermatologist will not prescribe me any long term treatment with tretinoin, because as several doctors I've seen before, she doesn't believe in its prevention and scar healing benefits or at least its side effects seem to scare her, but I don't feel like I can carry on for long with this on the counter treatment as it contains many actives and she suggested to use it up to 12 weeks...       Hi Anna, I just came across your post. I’m not an expert, but if you’ve had acne for so long, I would suggest you to perhaps look at certain foods that may be breaking you out. You may be allergic to gluten or dairy.

    So excited for this series as I have been kicking around the idea of starting tretinoin (after consulting a dermatologist), but am anxious about possible side effects. Now there are sites where you can do an online consult with a dermatologist and they can prescribe the retinoid. I'm of the opinion that in-person appointments are safer. Any thoughts on that Dr. Dray?

    Yay! I respect your professional opinion on this topic. :) I started Retin-A in the spring and have been working through a routine. Can you speak to the “controversy” over layering acids (glycolic, Vit C) when applying Retin-A? I have started washing with CeraVe (love it!), glycolic toner, Hada Labo gel, then Retin-A (three times a week) then CeraVe pm. It’s worked but I read there are no-nos v. myths on layering. TIA! 🙂

      you should not use those "layered" (apply along with) glycolic acid/ with retin A...you just degrade the retin a and exacerbate irritation..i don't know if the ferrulic acid effects it either, but probably could

    Could you please give us your thoughts on Sebaceous Hyperplasia regarding maintenance products to help keep it at bay? I've had my dermatologist do an "electrical needling" treatment that zapped most of them on my forehead and I use a BHA in the morning plus tretinoin.01% at night. Thank you!

      retinoids like tretinoin can help, yes, but there is nothing to prevent them

      Nole Girl I just had several Sebaceous Hyperplasia zapped. It looked like I has acne foe a day or two but after a week scabbing has cleared up and it looks fine. I have used Retin A for many, many years and my derm says this has keep them from getting larger and does discourage formation- some. She says there are no other topical products that really make any difference. Cheers!

      Nole Girl yes the needling worked great but it's not a procedure covered by my insurance so it cost over $100 to zap them. Tretinoin in the evening and bha in the morning seems to help.

    Would you recommend differin as a general anti-aging retinoid, or is it specifically formulated for acne?
