Abdomen X ray- May shows stone in Pancreatic duct

Endoscopic ultrasound- Rosemont criteria. #Best test for evaluation for chronic pancreatitis
Fecal elastase
5. How do we treat chronic pancreatitis
Non dilated duct- Enzyme supplement
Dilated duct- ERCP
EUS guided CPB
SURGERY
6. Is there any cure ?
If underlying cause is stopped disease can be cured-After stopping alcohol and smoking, operating, and with steroids in autoimmune pancreatitis, disease can be cured in many patients. Idiopathic chronic pancreatitis is life long illness and requires life long treatment
7. COMPLICATIONS
Pseudocyst - EUS guided drainage is best treatment
Malignancy - 4% risk of cancer
Bile duct obstruction: Can occur due to fibrosis, pancreatic head edema, inflammatory mass, malignancy. ERCP stenting is the treatment of choice.
Duodenal obstruction: Can occur due to fibrosis, pancreatic head edema, inflammatory mass, malignancy. Medical therapy and surgery.
Bleed: Splenic vein thrombosis can cause fundal and esophageal varices. Esophageal varices are treated with EVL. Gastric varices are treated with glue injection.
In few patients, splenic artery pseudo aneurysm may develop, and cause GI bleed. This requires angiography and embolization.

Hello Doctor. My brother's son is 20 years old suffering from chronic pancreatitis for past 4 years, CT & Ultrasound shows multiple calculi seen in body & head region, largest one measuring about 8mm, density about +800 HU, Pancreatic duct dilated 7 mm. Random Plasma sugar 104 mg/dl, Serum lipase 90 U/L, symptoms acute abdominal pain, loose weight due to non apatite. Do you think ERCP a best option or pancreatomy? Please guide.
