Supposedly Moth Into Flame was about Amy Winehouse. I doubt it's meth, but he struggled with cocaine and alcohol in the past. I think he may have relapsed which is why he didn't join the band for the writing of Hardwired. James made some cryptic comments about Kirk that led me to believe that he may have fallen off the wagon and needed to get straight.

He has that sorta stutter/shakiness thing. Nothing to do with drugs and whatnot, he's had it since childhood. Sometimes it's better, sometimes it's worse. In some 80s interviews he just gets anxious and zones out completely for a few seconds.
Kirk is a damn fine guitar player who plays in a style that is very raw, but very soulful, in the vain of someone like Jimmy Page; compared to polished and perfect players like Friedman and Malmsteen. I'll take raw and soulful in contrast to polished and perfect any day. Kirk is the man!
I think Kirk was talking about the enter Sandman Riff at the end - he doesnt say, and then he says he got the idea from Soundgarden Riffs on Louder than love. So you can thank Soundgarden for Metallicas biggest song. I guess Kirk was feeling Outshined.

I'm not wild about they're entire output but I really like two most recent albums, Death Magnetic and Hardwired. I have to hand it to them, most rock mega-stars would be dying their hair and getting facelifts but Metallica seems to have really pulled it together and is keeping it all genuine. Not bad at all for 35+ years!
