All ranks do the same total amount of healing, higher ranks just do more upfront healing in exchange for a shorter heal over time. The main reason to always use it at rank 1 is because the situation that would make you want to rank it up is healing burst damage, but if you want to heal burst damage you have way better tools to do that like Spiritbloom or Lifebind.
If you are spamming a ton of living flames during down time, either hard casts or procs from your tierset, you could run yourself oom simply on too many living flames that don't really do anything But also going oom could come from bad spell selection or not using your mana resources apropiately like missing EC casts or not potting.
