The middle finger has been used as a vulgar gesture for centuries. There's evidence it was a well known gesture in the United States from at least far back as the 1920s. You can easily tell from the smile on his face that he knows the gesture has another unintended meaning.

I believe that's universal. there's a tendon that holds your ring finger to your middle finger but not vice versa. So digit 3 can extend, but digit 4 can't without help.
