Ok, after some testing I finally figured this guy out. If you're trying to squeeze a specific item out of him (Diamond, Odd Medicine, etc), here's what you have to do. He will never offer an item that is included in your offer. That means that if you offer him a Blue Flower for example (even if it's just 1), his offer will never include Blue Flowers. So in order to push him into offering what you want, you can simply trade for the minimum amount of the materials you don't want (700-800k should be enough to make him offer his top tier items) and then include them in your next offer. Give him a bunch of Supreme Whiskeys and/or rare materials that you have in spare, and then make a new offer with Blue Flower, Red Flower, etc (things you don't care for). This will rule out those items from his list, and he'll be forced to offer other things like Odd Medicine, Diamond, Dynamite, etc. Alternatively, you can get Blue Flower and other stuff by exchanging Buggy Points, thought that's probably more annoying and time consuming than trading for them.
I'm a little late but I just started playing this game. Thank you for this. This comment saved me a lot of time and the method you suggested worked to perfection.

How much trade value are you trading with? It doesn't matter what you trade its the value of your items that matter. Sapphire is one of the highest grade materials so to have a chance for it to appear you need a lot of trade value not sure what the minimum is but I say start with 500,000 then work your way up from there. In the video I trade a platinum ring worth 1,250,000 trade value that number is guaranteed to eventually get a lot of sapphire but its chanced based on the material he offers so you see something you don't like trade for anything with a low trade value like 300,000 then trade again for a high amount again he will usually offer something different to trade and eventually he'll offer sapphire.
