BC woman says she can’t get transplant because of cost to stay in Vancouver for post-op care

  • A B.C. woman says she can’t access the life-saving transplant she needs because she can’t prove she has enough money in her bank account, and cannot afford to uproot her life to Vancouver for post-operative care.
    “I don’t have enough money to save my life. In B.C., in Canada,” Christina Derksen-Unrau said.
    The Princeton, B.C., resident suffers from asthma, emphysema and lung cancer, and a lung transplant is her “last option” to survive.
    In December 2022, Derksen-Unrau was told she was a candidate for transplant, but the approval came with a catch.
    Global's Aaron McArthur reports.

    I also need a transplant and have been taken off the transplant list despite having been cleared by surgeons and nephrologists (kidney transplant). Despite these things, not only have they failed to activate me on the transplant list, they have also given me underhanded comments while in communication with them implying that my disease is my fault (I have lupus nephritis, a genetic disease) despite me being a 30 year old athlete who is medication and treatment compliant (I never miss appointments or treatments).
    I have gone through the training to do home hemodialysis essentially taking over the work that other healthcare workers would be doing and ostensibly lightening the load that the system takes.
    I do 26-30 hours of dialysis a week at home, more than double the amount of dialysis that the average kidney patient does. According to the doctors this is a good thing that should make the transplant easier/more likely to succeed.
    Again, despite going above and beyond (to the point that I provide my own medical care), I am still not on the transplant list and have not been told why. The only thing BC transplant has done for me is tell me that I don't deserve to live.

    • @jasonsheath3294

      Just want to respond and clarify why this is not the same at all. The Alberta woman who passed away after trying to force the AB transplant agency to list her despite not taking the vaccination - is a sad case and she had my sympathies. But what you need to understand a couple things:
      A. Organs are extremely scarce resources - too many patients not enough donors;
      B. As such there are strict rules about who does and does not qualify for a transplant;
      C. Those rules revolve around one central premise - who is going to best care for this scarce gift and protect it;
      D. This is why active alcoholics, morbidly obese and smokers do not get put on the list until they quit first / lose weight. Sometimes they have to show they can be healthy for at least 6 months (sometimes longer) to prove they won't be damaging this scarce organ with their lifestyle;
      E. Before the Covid vaccines were made available - mortality for transplant patients from Covid 19 was approximately between 40-50% !!!!
      F. The reason is that we (yes, we, I am a kidney recipient myself) take immuno-suppressant drugs which while keeping our bodies immune system from attacking our transplanted organs - leave us with seriously compromised immune systems;
      G. The risk of "clotting" from getting Covid with no immunity for health compromised persons is about 10x what it was from the vaccines;
      H. With multiple vaccinations the risk of death is now down to 10% for transplant patients(kidney and lung are highest risk);
      I. That poor woman didn't trust Big Pharma and their (as you put it) 'experimental vaccines' but would have been required to take numerous daily medications to keep her transplant from failing;
      J. Given the risk of severe damage or death from Covid was so much higher for her with no immunity on immuno-suppressants - how she could be given such a scarce organ over others who would be doing everything they could to protect it?
      How do I know all this? Not only did I get a kidney in 2016 but I have been on isolation (Work from home) over 4 years due to severe risk from Covid. I have had multiple vaccines but even still Covid finally got me Christmas Eve. I was able to get the IV treatment Remsdevir (Big Pharma to the rescue) which likely saved me (only emergency treatment that still works for transplant patients).
      However, after it was all said and done - Covid STILL did serious damage to my kidney. I went from 28% function down to 21%. At 15% its back on dialysis for me and the lower you go the faster the kidney fails (less tissue same workload).
      Even with all my care and isolation and vaccines I still lost multiple years of freedom from the marathon meat grinder of dialysis. If not for my vaccinated partial immunity (vaccines only provide some protection for us - not like healthy people )- also due to our transplant meds, and Remsdevir I likely would have died.
      In the end the risk she wanted to take with a scarce organ was arguably even higher than those who smoke, drink or are obese.
      I am truly sympathetic to someone who was so fearful of 'Big Pharma' when she would have been reliant on them anyway for her transplant meds for the duration of any transplant. Very sad she passed away so needlessly.

    I think we have a problem in Canada - not just BC - with expecting too much from our public healthcare system. The reality, no matter where you live, is that there will NEVER be enough money to fund all healthcare costs for everyone. In some countries that do not have public healthcare plans, the population accepts that only the wealthiest people can afford the most expensive treatments. We all have seen the reports of former middleclass USA citizens who are homeless after becoming bankrupt due to medical costs.
    In Canada and other countries with 'socialised' medical systems, many treatments and medicines are much cheaper than in places were only or mainly private medical care is available. Many people in Canada, including among my family and friends, have received medical treatment that would not have been affordable in a privately funded system. But sometimes treatment is not available, as in this woman's case, in the area in which you live. In that case, is it really expected that society as a whole is responsible for the costs of not only the medical treatments but also her living expenses? Some people will argue that it is, that since we pay into a publicly funded system, then ALL costs to save a life should be borne by all.
    No one wants to put a price on life - but since more and more life-saving and life-prolonging procedures are discovered and perfected, and many of them cost thousands or hundreds of thousands of dollars, at some point we have to admit that there is not enough money in the pot to cover all the expensive treatments for everyone. Instead of facing this, and having some kind of national (or provincial) public discussion about it, hopefully leading to some well thought out decisions about how we want to spend our public healthcare dollars, we will continue to have these heartbreaking and emotional stories about the people who can not get the treatments they feel entitled to receive. Is it fair that a person who lives in or very near to a large metropolitan area can get a lung transplant while a person who lives in a distant rural area can not? I do not have an answer, except that I would not trade my small town life for life in Vancouver or Calgary etc. even to prolong my life by several years.
    I hope this woman either manages to access the treatment she needs, or to make peace with her shortened life expectancy.

      Simply put, we're in a population trap. We have allowed more immigrants and refugees into our country than our infrastructure and capital can support. Therefore, it is now impossible to maintain a basic standard of living for everyone without the average standard of living going down. It's similar to what happened in Japan and South Korea, except their problems are caused by refusing immigrants, while ours are caused by being too generous. Even if we stop all immigration tomorrow, it will be decades until our average standard of living starts to climb again.

    The transplant team is doing its job. They cannot let a valuable organ to someone who is not prepared for the recovery of such a major surgery. It is one thing to say that they must have a guarantee that they patient will be able to survive by being close the hospital, for routine care, and another thing to say that they cannot get a transplant because they don't have the money. The transplant team, by being very forthcoming and transparent, is requiring the patient and the public to take the necessary measures to ensure that the surgery is a success. As a result, the minister can be contacted, as well as social services, in order to put in place a housing arrangement before the surgery is done -- as it should be. One cannot mess around with a recovery that is not well prepared, and if this patient does not have the capacity to stay close to the hospital, a solution to that problem must be found.

      You keep using rhetoric to avoid the magic word "money". As someone who not only got worked up for a transplant in BC in 2006 but also was involved in lobbying the federal and provincial governments for financial supports for BC residents like this poor woman - I can tell youthis problem isn't new. She is being denied life saving treatment because she doesn't have the necessary money. Unless you already live or have family or friends on the Lower Mainland you are required to put up a massive amount of cash to qualify for a life saving procedure. This is just awful. There are many people like this woman who are too poor to do this. I was one of them. Fortunately I had family in Edmonton so I could leave my life behind in Kelowna and start over in Edmonton with the AB transplant program but for many people who live in many small and poorer communities across BC - their lives are not worth the cost to the BC Healthcare system.

On the contrary, I'm saying that the transplant team is bringing to light the need for a transplant patient to be well supported. Sometimes it can be filled by family, but sometimes not, which is the problem that needs to be addressed.

    I don't believe there is any place in Canada that will cover a person's costs for accommodation for 3-6 months, that's just a reality. Obviously the transplant team has to have strict rules in place to ensure a recipient of a rare and precious gift of life will be compliant with aftercare, for obvious reasons. Perhaps this couple will need to sell their home and property to be able to afford this? Many people have to do this in such a situation, you gotta do what you gotta do to save your life.

      You clearly don't understand how the transplant list works. If she changed provinces she moves to a whole new transplant wait list at the bottom -IF they even let her on their list. Provincial transplant agencies carefully guard against patients swapping orovinces to parachute on to their lists fir a faster chance at transplant. Scarcity of available donors means this happens all the time. How do I know? Because I was one of those patients that moved from BC to Alberta and the AB transplant agency put me through a lot of screening before finally approving me after several years. But I went to the bottom of their list. And for those waiting for organs more time waiting equals higher risk of death especially for lung patients.

    The hospital or the doctors aren’t asking her if she has the money but rather telling her that she needs to be monitored for aftercare & will have to be able to accommodate that fact. Sad to say, since there are many people waiting on a transplant list - but they do have to screen. They are telling her what she needs & making sure that she is ready otherwise the transplant will be a wasted opportunity.Also I would like to mention that there are all kinds of organizations that would & could help her with living accommodations.I noticed she amassed over $22, 000!!!! Insult the transplant team, Adrian Dix, but I find this reasonable & normal in a transplant situation..Btw, noticed it said she has emphysema - so a former smoker (hopefully former)? Why didn’t she listen to the healthcare workers back then? when they warned her this would happen? Always gets to me - the people that have asthma but continue to smoke knowing how bad it is for thier lungs. I work in healthcare, but luckily for the public, I am not a doctor, because I think I would refuse to treat someone whose decision it was to ignore my important warnings & medical advice.

      Let me explain something: someone on dialysis for example costs about 80-100K per year to treat at hospital. Many people live on diakysis 7 to 12 years or longer. A transplant costs about 50k-60k and the average life of a deceased donor kidney is about 15 years. For living donors its a much higher average. For a transplant to work properly those recipients have to be monitored extremely closely for the first 3-6 months depending on the organ but the cost of the initial surgery is often (like the obvious gains with kidneys) outpaced by the savings to the system. However in order for that to happen those patients need to have close treatment to make sure that "investment" to use your purely monetary perspective to work properly. If you are against this then what is the alternative? MAID for those with serious illness unless you have money? To go down that path is a dark road.

    This is not a political issue. This is. About the expectation we have for cost of treatment. If you choose to live in a place without medical care for your condition then should we expect to have tax dollars spent housing you? There are many conditions that can only be reasonably addressed in Vancouver. This government is well aware of these. The opening of BC Cancer Centres is one such condition. The expectation that the government will provide all services to all Communities is a non starter. That leaves the cost of temporary housing in Vancouver. A double lung transplant recipient requires nursing long after tje operation. They require specialist appointments on an ongoing basis.
    The social worker, that is a community health position is confirming that tjis women can come to Vancouver and live there. There are charities that provide housing, there are other facilities that provide longterm or permanent support. The social worker would have provided these options. I feel for this woman. She has to chose to upset her life or she can choose to die sooner but at home. There is no magic bullet. As for politics. The BC United party sold off land purchased to build a new hospital in Surrey. The Federal Conservatives fought healthcare transfers to the Provinces. I don’t care how you choose to vote. Please do make that decision on facts not rhetoric.
