My brother died in a car accident from not navigating a curve. He was drunk & had liver disease but kept it private. Personally I think he had confusion that night as well. Don't drive

@@MarkCooperMD i'm 35 and have all the symptoms of HE, for years, by symptoms I mean VERY severe confusion encephalopathy where I couldn't comb my hair, drive because visual processing issues, etc, to a degree of needing someone to help with those basic tests, like pealing a cucumber was an issue but what's the most significant is : sleep reversal. Which I have had for a long time severe since I was 18. I was reading, that in HE, because of cirrhosis, melatonin levels don't have a typical 'curve' in the evening, thus accumulate in the blood and that causes non-24HR sleep schedule. Makes sense, taking melatonin at 6-8pm in the morning in very low 0.5mg dose helped me fix the schedule, without it I am all over the place. - So basically, I DONT have liver issue - my enzymes are good and the ultrasound was good. I have autoimmune multiple issues, - including small fiber neuropathy causing probably low bowel movement, thus I have severe SIBO. this is where the overlap with HE comes: I see it's treated by treating SIBO , which is what gives me some relief, when I take antibiotics they can very much help my encephalopathy. never fully. But also I run into a paper saying HE have also autonomic neuropathy thus, that gives them SIBO which gives them neuropathy. is that the likely direction? I am just trying to figure out, there is so so little on SIBO and encephalopathy but mine is very severe. I took Ozempic and nearly died from encephalopathy, i'd expect to be hospitalized as I was falling a sleep more and more and couldn't stay awake. Never met anyone with such symptoms. Ozempic had very shortly gut side effects for me that went away, like for most other people, but the (likely) build up of bacteria in the gut due to low motility, is what gave me encephalopathy progression then (I guess). The another weird thing is my bowel movements are completely normal if I eat a lot then I have normal, sometimes slightly constipated if I don't eat much, but very regular overall. If you have any insights,please.
Thanks for this explanation. It’s the best one of HE I’ve seen. Although his family never said what his cause of death was, an ex of mine began acting very confused and angry on Facebook. He had posted that he needed a liver transplant, and that he’d lost a severe amount of weight. Also said he’d had problems walking and had to stop driving. Later photos revealed a dramatic change in appearance too. He looked like he’d aged 50 years. He’d been a heavy drinker when I was with him so I can only assume he had cirrhosis caused by alcoholism. He’d written to me shortly before he died, claiming he’d come all the way to Hawaii to see me (we had not seen each other in over a decade so this was alarming to say the least.) and was buying a house here! Never saw him because he Stayed two nights and left. It seemed so far fetched I was forced to conclude that he was deluded and experiencing severe confusion from his liver disease. Can HE be that severe? RIP, I’m sure this was an awful way to go.
Hey doctor my father is in coma like condition from hepat encephalopathy and even Hospital says no to my father, can you please tell how much days of pain my father had to feel lefts😢😢😢
