This song was made for the Batman Returns soundtrack. It makes more sense after watching the movie. I always thought it reflected Cat Woman's inner conflict, so you're correct about the mental health aspect.

This could also be about twin flames if the listen choices to interrupt it that way but it was directly inspired by Cat Woman from the Batman Returns soundtrack. She is in fact regarded as a legitimate legend and style icon who not only influenced many artists but a generation.
