    mental health services provision remains paternalistic , users really consulted during course of services contact from referral , assessment , treatment and decision to being being discharged. Not because of clinicians lack of empathy the constraints of work in form of time to deal with case loads, skills around needs assessment that reflect users involvement in decision making and attitudes towards mental health service users not being able to contribute to objective formulation of needs and solutions.
    Example is the staff patients ratio on acute units where HCA's , hard working but used to meet only basic needs at large and the emotional, risk areas can hardly be seen to by the qualified nurses as they double a lot in administrative work.
    Staff outcomes measurements are low on the agenda and a protracted services users outcomes measurements collaboratively formulated will need some improvement to improve quality outcomes of patients journey.

    What's needed is institutions like the old days.stop the phsycos attacking the public with knives, and such,.....talking about phsycos....!
