    Even if (1) biometry is within normal range and (2) growth velocity is normal (i.e no dropping of centiles), Doppler can still be abnormal (MCA, umb Artery, CPR and uterine artery). And this can be labelled as FGR. Therefore, Doppler MUST be an integral part of every growth scan.

      Not true. The article first talks about SGA Vs FGR. And the doppler criteria are applicable to such fetuses.
      Deranged doppler in a normal sized fetus will either predispose it to FUTURE growth retardation or will lead to fetal mortality but wont allow fetus to be termed it as FGR on same day.
      Article looks nice as long as its not misunderstood and used only as reference for certain situations.
      Based on guidelines, the article promotes more and more usage of doppler, hence more antenatal visits, might lead to more confusion and possibly lead to more apprehension among patients and ultimately more financial burden only on the patients.
      Till the time such articles become part of standard USG books like Callen or Rumack, it may be good idea to follow them only as reference for certain cases.

    I have always loved Indian Radiologist lectures. Thanks a lot for another great one.
    It would have been even better if you could have said what the abbreviations means. For someone who is not an OB expert and trying to learn OB US, the alphabet soups are kind of confusing.

    The problem is usg centres don’t mention centile values of various doppler parameters, rather they give numerical values.

    I was born small, and the nurses assumed I was premature when they saw me, but I was only one week early. I don't have access to any information about my growth in the womb. I am on a waiting list for Genetics, I suspect I have Temple Syndrome. I wish records were kept for everyone, to make it easier to find clues like being SGA for conditions later in life.

    So for the percentile chart you can only follow up a patient if you use 1 specific machine through out the whole pregnancy?

    practically its quite difficult to interpretate whether its 3rd centile or 5 th centile..the machine shows just below 10 th centile .in late FGR few articles say to look for MCA instead of umbilical artery as it may be normal in late trimester..give your feed back on this ?

      Established percentile or LMP percentile. Never AUAU percentile. All depends on first growth at dating scan.

    Very simple method for assesIUGR
    1st trem- less than one week of LMP
    2nd trem-less than two wks of LMP
    3rd trem- less than three wks of LMP
