      Yes, pictures would have been great to see what he actually means. Because I had my marionette-folds filled and it looks really great. Perhaps they haven‘t been too deep.

      It makes the lower face heavier looking from my experience. A mid face lift is better

      I agree. I'm 54 and I have the marionette lines, and have always had a downturned mouth. My entire life I have heard, "What's wrong? Why are you upset/sad?" And that assumption couldn't be farther from the truth. I hate the fact that my face says one thing about me when I feel completely different. Arrrg!

      @@robinlynn2503 what I've learned is to keep a very small smile going, just enough to "lift" the corners of my lips which helps. I must disclaimer that I'm 49 and my marionettes are about an inch long and not that deep. Hopefully it'll stay that way. I'm not ready to age yet so I've taken up using Olay microsculpt and their 7 way face cream and hope it'll help some.

      @@viccosmeticinstitute Sorry, but I disagree from the perspective of the model you use. Why not use an older woman who ACTUALLY HAS MARIONETTE FOLDS!! Then we could see before & afters. This use of perfect young faces is ridiculous. If you are so sure of your technique, which is interesting for sure, use a model who has the physical 'defect' you are speaking of. Thanks.

    Love the education. Also very happy to hear of that there are still ethical doctors out there turning down patients because it’s not in their best interest to just fill the area. I always wonder how there’s all these people particularly celebrities but also just regular people walking around with their faces full of filler and looking really weird, almost disfigured and definitely not looking younger. It’s good for both patients and medical professionals to be educated about this. Thank you for the content, very helpful

      i pay $400 for one syringe, works great.. plus adding another syringe to fill lip corners help eliminate the marionnetes

    Wonderfully informative. I’m someone who went for the face lift (at 66) and I am so very pleased. I’ve never had fillers or Botox - I’m just one of those people who believe that sometimes the most extreme solution is the best - and in the long run less expensive and gives best results. Mind you - I did my research and made sure i was in the hands of someone highly respected and with a great track record - in Halifax, Nova Scotia, Canada ( my hometown) . It was a 4 hour surgery (had dermabrasion and fat from tummy used for volume and my neck tightened, all at the same time) and cost 15,000 Canadian - that was in March 2016. I was kept overnight in a beautiful waterfront clinic , had a private nurse and was given one year free follow up with the clinic esthetician - face massage, vit oils and facial ultrasounds. Would happily add a photo but can’t se how. :)

      I agree with you! The most low-maintenance option is accepting the aging process but after that, it's having a facelift. Better than years of fillers and botox I think and a much more natural effect if you go to the right person.

    I love his honesty. I have marionette lines and I can see while playing with my face that it may not work. A facelift would be needed for me at age 57. However,
    Ouch!!! In the wake of awakening, maybe we should let this crap go and just be happy from within. I look at these a bit then realize that this is living in the 3d earth and this is not what any of us are here to do. To be insecure and care so much about vanity. That is insanity. I will grow old gracefully and with acceptance and will honor the body that I choose to carry me on my path. I am grateful.

      Getting older is no picnic. I watch these videos thinking that I wish I could afford to do these things but I can’t afford them. I asked my Husband about getting some fillers in my neck and face and he said no way! He said I don’t need it and that there’s nothing wrong with how I look. I’m 48 and I’ve noticed my neck getting more horizontal rings and a bunching, creepy area in the center of my neck that I don’t like. My neck bothers me more than anything. I really don’t have that many bad facial wrinkles. It’s hard to not feel inadequate compared to women who can afford all these treatments to look better. Guess I have to just work on my poetry and guitar and feel fortunate that my husband loves me the way I am and so do my kids and grandkids. There’s nothing wrong with aging, it’s something we’re all destined to do if we’re lucky enough to live that long. Good thing I don’t live in California!

      Some people look older than their years because of stresses out their control, and although youth is king, I do not think a lot of, especially women, seek procedures out of vanity or insecurity; though society has always pressured women to fit a particualr image.
      I applaud those who just want a little correction, treating themselves as a reward to gain back just a few years for the ones they lost for whatever reason.
      Aging gracefully is easier for some, depending on their circumstances.

    I couldn't agree with you more. Marionette lines.folds are a very common complaint in my practice, but fillers in this area don't look good over time. I agree with your approach. Thank you for sharing your vast knowledge!

      I had botox to the downturn corners I couldn't pronounce words properly it was horrible now I have had filler and they are still not better. I don't know what to try next?

    Thank you for helping me save money. 😊 I'm smart enough to realize that it takes a facelift to improve. I am 57 but with hair, makeup and good skin care, people think I'm my 40's. That is my most obvious aged spot. Not ready for a facelift yet, maybe never! Peace, love, good health and happiness to all!

    My dermatologist told me she couldn't remove them but she could smooth them out. She did a fantastic job. Down side, like with all fillers it was temporary and only lasted about 8 months.
    There's hope but it has to be understood they will not be gone for ever and it will cost a pretty penny.

    I can relate to this so much as an injector.I was never happy with my results there and have just started to explain to patients the difficulties in that area. I used to get frustrated expecting that I should be able to improve things. I guess it was about confidence for me. Confidence to say no to patients, not in my abilities. Thanks for the great video.

    I never really fill marionette folds unless patients really insist doing so. Instead, I reconstruct the lower jawline by local liposuction plus lipolysis agents along with thread lift to give a much better facial structure for the best rejuvenated look. Great video, Dr. Chan !

      I have often heard my patients tell me a similar story. Try the filler in the corner of the mouth. Makes a good difference without surgery

    I just saw a plastic surgeon who told me this exact thing! I didn't have any crazy expectations, but am satisfied by the appearance of a softening of my lines with the corner mouth filler, etc. I look like a slightly younger version of me!

    I had fillers in cheeks and around lips for marionette lines and the results were excellent! And the lines were distinctly formed but not extremely deep. Very happy with how it helped!

    I was trying to learn about marionette folds and have concluded that volume loss due to fat and bone loss contribute and sagging of the mid face and skin laxity as well and your sharing just confirmed it... I have not found doctors here talking about marionette lines like you do, well there is another wonderful Dr I follow who does...that's about it. That said, I can't agree with you that filler does not work. I think a certain amount of filler can work (at least for a while) if it is fat and bone loss (if filler is placed at bone level is ideal) and it is very important to then improve skin quality. Even if a patient does a face lift but does not have lost volume replaced, she is going to end up with a smaller skinny face and still can look aged. So either filler or her own fats should be used to compensate the volume loss. Thanks for the video, Dr.

    Fillers didn't work for me for the marionette lines but as he says at 4.32 Botox helps a bit imo. Microcurrent massage also helped. My face only has these lines on one side of my face, side I used to sleep on. Excellent video.

    Dr., You correctly mentioned hairdressers creating youthful appearances with specific hairdos. Very true & more time and money efficient and effective.🌹
    Another method is makeup - specifically bronzers, concealers, and lip liners - Especially for the over 65 mature woman.
    Not only makeup for eyebrows & eyes (to draw attention to that part of a woman's face), but also contouring/ bronzing gels & powders that create a tanned/bronze look below the cheek & below the jawline. This contrast in shades and tones gived depths to the face.💃
    Lipliners! When applied correctly, have the ability to hide wrinkles above the lip and create a fuller lower lip once again. The marionette lines are still there, but a woman has a much better chance of not being stuck with "balloon lips".
    With makeup, everyday or even every hour is a new beginning/ a new artistic drawing.💋
    Because many women over 65 don't seem to hold the benefits of a facelift for long, & then begin to overuse fillers; I've decided to give "Aging Gracefully and with Class" a chance.🤗

    Your content is so informative and I love your approach.
    I know people tend to prefer instant gratification but I have seen amazing results on women who have been consistent with doing facial excercises(face 'yoga').
    The volume loss is not just in the epidermis and bone but also the slackening of the muscles. When you start bringing back the muscle tone in the face it's amazing what it does to the shape and contour(just like the body obviously!). It gives an overall much more youthful appearance.

      Nah it does not necessarily work for everyone especially for those who already are very 'cheeky' can't add more fillers to that area or will look like chipmunks or have wider face. It is the cheeks fat pats sagging and pulling downwards plus skin laxity with ageing. I have seen so many people doing cheeks but the marionette lines still stayed.

      @@Dandelion_flight I didn’t say that it works for everyone.Fillers are for replacing the volume loss in these areas providing a more youthful look and definitely not making someone look fake or overdone.However if someone has a more significant skin laxity or sagging skin then threads or face lift might suitable for addressing these issues.Thank you

    I really appreciate that you openly discuss what you've done in the past, and why it didn't work. In the USA, doctors are hypervigilant about avoiding saying anything that could be used against them in court when they get sued.

    I’ve had that ‘smile lift’ with a pop of filler under each mouth corner just as the Dr described; it doesn’t erase all the marionette lines, but it provides support to the mouth corners and I found it made a big difference in my overall look. Now I just put a little concealer in the ‘shadow’ of the crease and it makes the marionettes almost disappear visually. also had Voluma done in the cheeks earlier this year and the injector put a little in my jawline near my ear, at first I didn’t understand why but this video explains it so well.

      I agree I had the same and I'm very happy with results; the point of doing this is to look natural; I have a tiny weeny left but with a tap of concealer its looks great; the filler usually last a year; I feel that every year it becomes less and less probably because these muscle or area is being trained to do something different. I would think if you have very deep lines then a face lift is fine; but really when you get a certain age their is no denying it.

    I still had filler to this area even after watching this video. Initially it looked better but a month and half later it looks worse. I even had it done by a highly reputable doctor. I'm altogether done with fillers. Not worth the money for my face.

    So informative. Just what I have been looking for. Wanted help but fearful that I would go and have the wrong procedure by the wrong person. Young people around me having procedures at 25 years old by nurses (I am an RN not running them down but this is my face thank you very much)and never seeing a plastic surgeon. Thank you for your honesty. I can not tell you how grateful I am to watch your videos.Thank you so much. The one thing this young girl said to me when I told her I had had my naso-labial folds and my laugh lines done and it didn't work very well was that the nurse who does hers would fill my cheeks and didn't that make sense?? You have just helped me understand what the process is and why it didn't work. and why the cheek fillers won't work either.

    Not all marionette lines are caused from that movement. I have osteoporosis and lost my bottom teeth because of it and when I got bottom teeth replacements I'm still left with marionette lines now because of the Jawbone decrease. Plus, some is from just aging. I would like to see an example of an older women just like others have said.

    Thank you for being so honest and saying the best thing for the lines pulling the lips down is a face-lift. I understand a lot of people don't want that but it is the only thing that can help those lines. You either accept it or do that... not stuff filler in it.     I appreciate the in depth reasoning in this video of why fillers in the marionette lines don't work, and how it would take too much in the cheeks to get any lift, and would look ridiculous. I already have very chubby cheeks when I smile, so when someone tries to sell me on cheek filler for lifting of the nasolabial folds or marionette lines, I'm always skeptical. I've tried fillers in all 3 areas to no avail. And was not willing to get an excessive amount in my cheeks and look crazy.
    A little feedback: The girl in the video with what looks like blood is running down her face is creepy, it made me want to stop the video! But you are very honest and knowledgeable, and that I appreciate! I agree with others, use a more mature model. thanks! :)
    QUESTION: I landed here looking to see if lip fillers could help turn the corners of my mouth up a bit? Then I see that you put a tiny bit in the side for this. What type of filler, and how much on each side approximately? (I'm in Phoenix, Arizona, so I can't come see you) Would a little bit of plumping of the lips help distract from the marionette lines too? I have some drooping in the jowls, so I'm skeptical of any injections in the jaw line. Is there another treatment you'd recommend for that? I'm not ready for face lift, and a bit afraid of that too. Also...I have acne scars in the area where the marionette lines are, and I believe they contribute to the increased look of the folds, is there a fix for this?

    Just do buccal massage. It releases the muscles and gets the tension out of them. If you do it yourself it’s completely free, I’ve had great results doing this.

    Dr one thing you forgot to mention is inward pointing teeth. As we age teeth, and lip support gets worse… and people with inward pointing teeth it makes the downward smile worse. As my teeth get worse and more inward facing, my marionette lines and downward smile gets worse. Braces is an option for me and fillers until the braces. Also you said fill the jaw? Do you mean so it lines up with the already hanging jowl? So then that makes the jaw bigger. Not sure I get that.

    I love this so much!!! I use this "myomodulation" technique on my patients around the mouth and it makes such a beautiful difference. So glad that you talk about setting realistic expectations. So important for building a trusting relationship as well. Still hoping to come to Australia for a conference someday. Thank you for your work!

    Getting a lower face lift got rid of the marionette lines, jowls, and lines around the lips...done in office with only local anesthesia (afraid of general)..doesn't make you look tight like you've had work done...cost about $10,000...Goal was not to look 19 but to look good for my age (66). Highly recommend.

    What are your thoughts on perioral mound liposuction for those mounds that contribute to the marionette folds, if skin is otherwise firm? Some surgeons say it's very dangerous due to nerves, others say no nerves are in that perioral mound region. A video on this would be so helpful, as there's not much online about it!       I think I have perioral mounds that give the appearance of a pulled down mouth it's so irritating I'm 37 and it makes me look older. I have had a few consultations that said dermal filler in the area is best but watching this I'm not so sure.

    I love you and the positive attitude and bed side manner that puts patients at ease. I have seen brilliant work from you. I agree with you on this topic, in the tear trough. Different approach is essential. I do believe that filler is excellent as a strut effect on the ligament. Example, 6 point or 8 point lifts. Or MD codes. I have fabulous results filling marionette lines. Had my jaw broken and 19 teeth knocked out. only used .5 total. The idea of "so much filler " is ridiculous. I keep wondering why the most expensive procedures are promoted . Instead of a facelift, Sculptra is an outstanding product. I can't promote off label usage, although in a artistic hand and longer reconstitution time, adding 6 MLS of local anesthesia and bacteriostatic water is a great approch. I have witnessed results that are mind blowing using 3 vials of sculptra, once, properly massages for months and then adding the "struts" of H.A. strategically 6 months later. I personally don't fill lips with H.A. until the entire structure is revived. Microneedling, and light acid peels make the dermal canvas beautiful. Of course ethnicity plays into the approach. I personally would refer a patient who is trying to look European. That is Another issue never discussed, and it is sad. Personally, I have only ,5mL in my marionette.(not in this photo) I'm almost 44. Not trying to look 24. There are a million options. I love Rebecca Fitzgerald and anyone who doubts the power of injection should watch the techniques that are nothing short of a masterpiece. She doesn't use "mounds" of filler. What about acne scaring? Bellafill is the best choice. I'm not sure lasers help, instead they worsen them by tightened dermis I am living proof. I am not meaning any disrespect. But this is a non invasive channel correct? Expose the risk of facelifts. And fat transfer is the number one procedure to cause occlusion. With so many new applications and products , you are only taking about the bad effect of filler, and suggesting VERY Invasive procedures. So much is out there, why not make a video of other options without saying h,a, is so horrible. Quit bringing it up. I have witnessed the amazing transformation you have done with H.A. so that was an optical illusion? You are very talented but this channel is so redundant. Any approach in the wrong hands is a disaster. Exciting time for aesthetics. Move on if you like. But I can assure your audience not ALL fillers are bad.lets see a "ponytail lift", if you are inclined to surgery. Minimal invasive and fabulous results. Top off with plasma for the surface. That's would be great 😊 I volunteer, lol. Thanks for the content. You are loveable.

    I had lip and marionette fold filled a bit and am so very happy.. they also put some at the corners of my mouth and I did get some up turn of my lips... I think it’s all about expectations ... I did not expect perfection, just softening of the folds!!!

    The scariest part is, you enter the cosmetic clinic, as you want to correct one or more facial regions which you don't like, but end up walking out of the cosmetic clinic, looking & feeling 10 or even 20 times worse! Truly scary & depressing to think about.
    IMPO, always, always do plenty of prior research & check all reviews, etc;. Speak if you can, to "real" people who've been to the clinic that you're interested in visiting. This may well help you, to steer away from a potential botch job.

    I got 2 ampules of filler in each cheek. I’m 55 and had some sagging around my mouth area. It really helped and the new cheekbones were attractive! Personally it didn’t totally remove the marionette lines completely but it really improved my appearance. A little dysport around the mouth helped relax the muscles...added plus it plumped my lips a bit. Total cost...$2400. Ouch! Worth it? Yeah it was. Painful...not really.

    Hi i have been having Profhilo treatment for the last three years. I no longer have any filllers or botox as i find Profhilo just gives you a nice natural younger look. I have never heard you speak of this treatment and would like to hear your opinion. I am a 53 year old man.

    I noticed my downward turned mouth immediately after having my braces removed. I’m sure it was because I had 2 teeth removed prior. (I think this was even more detrimental because I was an adult.) Please get a second opinion if you’re thinking about getting braces and they want to pull some.

      Omg, this so true, I wore braces for 2 years and 2 teeth removed too to pull back my underbite. I loved my smile now but noticed the lines got worse. So sad.

    Thank you very much for this video! I got to the same conclusion as client. I am 32, I have great skin and no wrinkles, but I started to develop marionette lines several years ago. I tried Botox and fillers for several times, but nothing had an impact. So, I understood that the only solution is a face lift. But I am still too young for it. May be, I will go for it in several years.

    I had a face lift and an upper lip lift and also laser all around my mouth. But yes the gravity is still the winner! I go back next month to see what else my doc can do, other than going back into surgery to do another pull up of my face. I still find myself pulling back my face and neck, same as i did before my surgery. I just feel my doc has to try again. Plus my neck still has loose skin laxity and banding. I am 63-yrs old. The battle continues!

    My experience has been positive with dermal fillers to the marionette lines. I am 60 years old and have had them injected 3 times in the past 2+ years with substantial improvement. I've noticed that he does utilize a small amount at the corner of the lips, so maybe that accounts for part of it, but I have been happy with the results.
