    That was an important note at the end of this video about requiring an "Enhanced Ground Proximity Warning System" in 2002 and could have given flight 965 that extra time needed to get over that mountain. The traditional GPWS flight 965 was equipped with had a blind spot. Since it can only gather data from directly below the aircraft, it must predict future terrain features. If there is a dramatic change in terrain, such as a steep slope/ mountain, GPWS will not detect the aircraft closure rate until it is too late for evasive action.
    All planes after 2002 with 6 or more passengers are required to have the "Enhanced Ground Proximity Warning System". The system is combined with a worldwide digital terrain database and relies on Global Positioning System (GPS) technology. On-board computers compare current location with a database of the Earth's terrain. The Terrain Display gives pilots a visual orientation to high and low points nearby the aircraft.

    Just goes to show that no matter how much experience you have, whether being a pilot or car driver or similar, we all react differently in a life threatening emergency, both pilots "forgot" about the speed brakes.

    I have no idea why I’ve been binge watching this channel when I have a fear of flying. But it’s been weirdly satisfying knowing either a) what airlines to avoid (I live in Australia and won’t be flying Ethiopian anytime soon) or b) the changes they’ve made on the better known airlines over the years.

    I remembered seeing this, a little more detail on this version. The realism at the beginning of the video is absolutely fabulous. It's been many years since I messed with FSX, these flight Sims have come along ways.     I just read the American Airlines Flight 965: CVR/FDR Transcript, The captain sounded like a great person, but at the same time is sad to read:
    2129:23 Captain (cabin PA): Uh ladies and gentlemen, this is captain Tafuri. We have begun our descent for landing at Cali. It's a lovely evening as we had expected. We'll pass a shower or two on the way in but uh, at the field right now it's uh, good visibility, the temperature is two three, that's twenty three degrees Celcius, and if you prefer Fahrenheit, that's seventy two degrees on the Fahrenheit scale. The winds are ten miles an hour from the northwest. It's a very very pretty evening. I'd like to thank everyone for coming with us. Again, I apologize for, being late tonight. These things do happen sometimes, very frustrating but there wasn't very much we could do about it. Again I appreciate your patience in the matter. Like to wish everyone a very very happy holiday, and a healthy and prosperous nineteen ninety six. Thank you for coming with us.

      @Leus Reit - Two of the causes cited in the investagation after the accident was 1.) The FMS-generated navigational information that used a different naming convention from that published in navigational charts. 2.) FMS logic that dropped all intermediate fixes from the display(s) in the event of execution of a direct routing
      I hope they learned a lot from this accident and if the condition were the same, they could avoid such a tragic loss of life.

      A similar crashed occurred in the early seventies involving an eastern FLT to Miami, I believe it was a L10-11 . Three experienced flight crew got distracted by a nose wheel indicator light, as a result they didn't realize the auto pilot became disengaged ; despite audio alert , the plane flew into the everglades.

      I often react that way to some of the "probable causes" in the final report. Why leave out things like that? But an approach and landing could have been done safely without radar at Cali. Yes, it is then more demanding on the pilots. More so, it removes a double-check on their position and situational awareness. The pilots, knowing there isn't any double-checking by ATC's radar, should be as prepared as possible and willing to, say, get into a holding pattern to figure out any confusing details.

And "The Wall" is a stupid idea for very many reasons. As to your challenge: 10 of my favorite international cultural sites: Louvre, British Museum, The Great Wall, Forbidden City, Rodin Sculpture Garden, Uffizi Gallery, Galleria dell'Accademia, Sistine Chapel, Great Zimbabwe, and various cultural centers (Icelandic, British, Welsh, French) where medieval villages have been recreated. Admittedly, I haven't gotten to lots of places, including South American (unless you count T&T), SE Asia nor North Africa. But if by "icons" you meant pop-culture personalities, yeah, I'm pretty clueless on TV stars and contemporary musicians from the USA or internationally.

      @Sabrina Dugan Prisons have armed guards every few hundred yards for when people climb over the walls as people have been doing for thousands of years. Most illegal immigrants are in the US on over-stayed visas and arrived through US airports. Airplanes fly over walls. Most drugs arrive at US ports of entry in shipping containers or commercial vessels. Maintaining the existing walls where they have been deemed most effective would help more than new walls in very isolated locations.

      When I first learned to be a pilot, it was drilled into me “fly the plane, fly the plane”. A pilot’s number one priority is to fly the plane, and flight 401 is often cited as an example of that.
      1) Aviate (i.e. fly the plane)
      2) Navigate
      3) Communicate

    A cue to all aviators, do't be overly reliant on the FMS, when in doubt step it down and dial the numbers and into VOR's manually to be sure.

    Another amazing video and man it kills me so many of these crashes end in "PULL UP , whoop whoop, PULL UP" "TERRAIN, TERRAIN, PULL UP!" I wonder why so many pilots don't take them seriously? YOU'RE ABOUT TO FLY INTO A MOUNTAIN.. OR SOMETHING! FN PULL UP!

      The latest 'terrain awareness' aids offer something better like "and bank left/right" too. This crash was caused by the rushed entering of an incorrect waypoint into the Flight Management System when the landing approach was changed from that originally intended. Simple human error.

      They DID disengage the autopilot and start climbing immediately when the terrain warning sounded but they forgot to disengage the air brakes they had employed as the plane was approaching Cali. Had the air brakes been disengaged, they would have cleared the mountain. So sad.

    Airlines should stop flying to airports without radar system like Cali. In this video pilots error is apparent, but if Cali had radar the ATC may helped the pilots with proper vectoring. I stand to be corrected.

      Raviechandran Mariappan. That was 23 years ago and AA as well as the captain knew they had no radar assistance. As of 2015 many airports in the USA had no radar and many more around the world. Nowadays Colombia is one of the countries that have the best radar coverage in the world.

      I would never run an airline that had to stop landings on airports without radar. My pilots should be more than capable to land without anyone watching them on radar. If they were unable to do that they would never get a job in my airline.

      Reem Khattar And knowing that there are very high mountain in the general area why not get altitude until you at least figure out where the hell you are? That is the first thing going though my head if I am a pilot on that flight

      Because he thought the controller told him to go straight to Rozo but he still should not have erased the points in my opinion.

      this is an over-emphasized issue. It's not that time consuming or hard to bring them back up again. Better yet, just bring up the published STAR and fly the damn thing as published to begin with. The controller never told them to deviate from that and it's not clear why they assumed he did.

      Like so many crashes, there were a series of factors that alone would not cause a serious problem, but together they caused this unfortunate catastrophe. The one factor that was not mentioned in the captions, but that was obvious to me was FATIGUE. After a long WX delay, the flight departed late, and the crew began making tiny mistakes, like incorrect Nav fixes, and forgetting waypoint names, like when the Controller corrected the readback when the Pilot said "*T H E* VOR". The controller recognized what was happening and correctly specified the TULUA VOR. The Pilot again failed to acknowledge it.
      ATC corrected the readbacks several times and tried to reinforce the TULUA checkpoint,,, several times. Then the Captain said "We really got [f-ed] up..." that should have raised a red flag in the F/O's mind. Another thing... When offered the alternative of a straight-in on 19 instead of the planned circle to land on 01, the crew response was "We're too close," but then the captain allowed the Airline Schedule to fly the airplane and opted to accept the offer. Accepting such an offer after committing to the 01 approach led to a loss of total control over every aspect of the flight.
      All of these "little" factors are common symptoms of tiredness, fatigue and exhaustion... Brain fog had set in. Same goes for clearing the Nav Computer. Same goes for not being able to find the VOR frequency. Same goes for failing to just ask for the VOR frequency. Same goes for being confused by what the Nav was showing for their position relative the VOR. Same goes for leaving the slats out and trying to climb. Same goes for reporting his altitude as 9,000 when in the video that altimeter is clearly descending through 6,000.
      And lots more.
      This really reads like the crew was too tired to fly the airplane -- just when they reached the most critical point in the entire flight: A night landing in unfamiliar, mountainous terrain, and then accepting a dive-bomber approach in a hasty diversion to an unplanned runway. God rest the souls of those who perished and comfort their loved ones.

    This is a beautiful video w/great audio. These pilots really screwed the pooch here. God rest their souls. The accident ultimately improved advanced GPWS and computer navigation systems, which likely prevented similar accidents from occuring.

    I recently watched a documentary on this...there were actually 5 people that survived the initial crash. A 19 y/o man, a 21 y/o woman (12/20 was her 21st birthday), a young father, his 6 y/o daughter & his son (10-12 y/o...can't remember exactly). The son survived the initial crash but died in hospital the next day. All their seats were positioned within 2 rows over the wing (right wing, I think). The survivors were gravely injured & stranded at the crash site on a remote mountainside for 8-10 hours before rescuers arrived. I'm always astonished & impressed by people that survive overwhelmingly traumatic events like this...& still thrive throughout life. The strength of the human spirit never ceases to amaze. RIP to all the souls that were lost in this tragic event.

    Anyway, The Flight Channel had a big hit in how I look at air crash investigation. I used to always watch, The NTSB investigation videos, since finding your channel in early 2018 I never watch NTSB crash investagtions thank you for what you have created!!

      I do, they complement each other for me. I'm a n00b, interested in aviation but I have little knowledge of terms used in aviation. The Air Crash Investigation helps me understand those thing. There isn't much else as most articles on Google are still quite over my head sometimes.
      I am definitely not a tech/physics/science person. 😌

      Nice to know, I definitely got my attention to aviation from the NTSB channel that would play every Sunday. *IF* you want these tips here you go,
      -I'd recommend playing airliner games they get your hang (from me) and overall just drive you in the aviation stuff.

    Fantastic video, as are all of yours. One little difference in this video is that the crash takes place in 1995. As the American plane is taxiing to then runway it shows a United Airlines jet with the Continental livery on it, which of course was not the case back in 1995. Just a small glitch I noticed. I enjoy all of your vids and they are the most realistic and comprehensive on youtube. Thanks for doing such a great job on them. Much respect.

    I know you've heard this 1000x, but your channel by far is the best channel on here!! Not to mention my anxiety for flying increases during each video lol, but just the absolute quality of your work is what keeps me watching. We all can tell that you take pride in each and every video, and it is truly appreciated!! But just as I was confused trying to understand the conversation and locations, I could understand why the pilots were confused. It doesn't matter how many flying hours, or how professional you are, when you're not used to it, or it's your first time, it is easy to get confused. It's so sad for all the lives lost, and I couldn't even imagine those who survived!! Can't wait to see what great video we will have next week!!

    I was very glad to read the dog survived. I always feel bad for the animals in the cargo hold (or, indeed, in the passenger compartment in carry-on bags) who just get left to a horrible fate whenever an airplane ditches, even when the ditch is kinda survivable.

I also recommend the podcast Black Box Down for something to fall asleep to that is sound only. Super interesting to watch/listen to, and I've been spend way too much time the past year binging those.

    Sometimes we as pilots the higher we go up the success ladder and the bigger machines we fly just makes us forget the 4 Cs of basic aviation. Climb,Call,Confess and Comply. We also forget to Aviate, Navigate and Communicate.

    I flew from Boston to LAX on a 757 in 1994 on US Air I believe. Both pilots were ladies and I was a little apprehensive. After landing safely in Los Angeles, I realized I had one of the most comfortable flights I ever had in the 757 and with this flight crew. I don't know why these experienced pilots could get so messed up and confused but with all of the NAV Aids out there, you have to wonder. The fact that Cali did not have a radar due to the bombing, why wasn't that listed as a contributing factor to this crash? Anyone know?

    By 1995 with all the technology we had at the time, I never thought at the time that a US jetliner would fly into a mountain any more. Those things were common in the 1950s & 60s. The newscast of this was really shocking. And then later we learned that it became a case of technology being their worst enemy. So sad that two experienced pilots simply forgot to disable the speed brakes and they would have cleared the mountain.
    Another thing in that the ground proximity warning only gave them 12 seconds to react was a major flaw.

      Mark B Fensler the GPWS was limited to its design back then and back then it primarily relied on the radar altimeter which would have suddenly detected the ground rising up. Now EGPWS has a map of the ground and the altitude so it knows in advance if the plane is too low. It’s basically like being blind and only knowing a car is about to hit you when you hear it

    I don't understand why an airline would be permitted to fly into that area that had no ATC radar and mountainous region. Okay, so they had an FMS but did anyone ever think that it could go inoperative? Then what. Perhaps at the time only daytime flights should have been authorized. Crazy...

I can't believe the professional Colombian investigators failed to bring up this crucial detail in their extensive investigation, when you revealed the crash's culprit in a single paragraph of text! If only they had you, TH-cam commentor, to aid them in their research, they would've found out this obvious cause! Unfortunately, the only aid they had was the US NTSB, the FAA, and representatives from Boeing and American Airlines, the poor ill-suited souls. As soon as that dastardly Colombian air traffic controller started spewing Spanish all over the radio, those poor pilots were doomed! How very third-world of him.

What was he thinking when he erased the way points in the computer knowing that they were flying through mountainous region with no radar as a back up ? To my knowledge Jeppesen was 30 percent at fault for the crash, Honeywell was 10 percent at fault, and American Airlines was 60 percent at fault. Finally keep your racist beliefs to yourself.

      Some airports don't have towers technically most airports don't it's not an absolute necessity they had one better a controller but no radar more than enough to land safely it's not like the situation was a surprise

    Please make a video on the india airlines flight 257(boeing 737-2A8) that crashed on august 16 1991, crashed in manipur, india. The crash killed all 69 people on board. Please make a video of it

    This is the one where the more moves the pilots executed, the more they sealed their - and almost everyone on the plane - fate. How utterly sad for two pilots with such heretofore impeccable records.
    Wonder if the survivors are alive today? Would love to hear their stories. And the dog. Wonder how long he lived afterwards.

      If you watch the TV series on this crash, which everybody loves to denigrate, you'll see an interview with a young lady whose mother was trying to play matchmaker with the young man sitting next to her, making her so uncomfortable that she moved to sit with her father. Her parents both died but the guy survived and they got hitched. This is from memory so it could have been another flight, but I don't think so. Only 4 survived I believe.

    I remember it so well. My cousin Michelle was one of the survivors along with her dad Gonzalo. My cousin Gonzalito and his mom didn't survive. I was 5 years old when this happened and we got the call. Couldn't make it to Buga but Michelle got flown into Cali so we ended up seeing her there. In the wake for Gonzalito and his mom along side other victims. There were no open caskets.

      @Mark Lacey: This happened in 1995. The GPS system only went live that same year and it was exclusively for military use for several more years after that. No commercial airliner could have had GPS in 1995.

    It's such basic airmanship to identify the station. Had the pilot listened to the identifier after selecting Rozo, all would have been well. Great video. I'm presently a student pilot and your videos are a great help.

    Thank you for all the hard work you put into the videos. I know its grueling work, I do some video editing and it could take countless hours of tedious work. This video is an example of very experienced pilots with thousands and thousands of hours as captains and First Officers making a simple mistake causing many, to loose their lives.
