Second was him fighting head on instead of killing zaraki like how he killed rose and kensei. Third is him underestimating his opponent. Lastly, he could have imagined himself getting miraculously healed from the backlash, he could have imagined time stopping for zaraki. There were infinite possibilities of winning tbh🤷🏻♂️

Gerard isn't actually immortal. He has a "core" that if destroyed, so is he. This was stated by Kubo. It likely will be revised in the Anime like other fights.
This is something really cool about Bleach, the powers are created in such a way that you can see and feel the potential on each one. For example, Izuru Kira, the liutenant of the 3rd squad, his shikai is so simple yet so strong, after activating his shikai, Wabisuke, his sword becames like a Hook, but with two 90 degree angles, having a Blade in the inner side, with the special ability to double the weight of everything it touches, and its acumulative, so basically after hitting your sword 3 times ir becames 8 times heavier. Tell me that couldn't be really overpowered

Gremmy stands no chance due to how weak of a vessel he is(his hax got over powdered by zaraki). Ichigo’s true Bankai alters fate. If he gets a clean hit he can kill anybody no matter if you had immortality or intangability. The reason it didn’t work the first time on Juha Bach is cause he has access to the infinite multiverse and can just choose a time line where he doesn’t get cut. It did work when Uryu used his sternriter ability to take away Bach’s reality power though.

I'd also put Jugram with his Balance reversing all damage back to him. He destroyed his pre True Zanpakutō Bankai with zero difficulty. So him fully using his Schrift, and possible Volständig (Which I believe he has because of Yhwach's trust with him with the Almighty of all things, that and Yhwach gives others powers through Reishi as well)

For example, Izuru Kira, the liutenant of the 3rd squad, his shikai is so simple yet so strong, after activating his shikai, Wabisuke, his sword becames like a Hook, but with two 90 degree angles, having a Blade in the inner side, with the special ability to double the weight of everything it touches, and its acumulative, so basically after hitting your sword 3 times ir becames 8 times heavier. Tell me that couldn't be really overpowered

The Almighty cannot make the impossible possible, if there is no possibility of something happening, he can't do it, hence why the Almighty could not be used to kill Aizen.

The reason it didn’t work the first time on Juha Bach is cause he has access to the infinite multiverse and can just choose a time line where he doesn’t get cut.
