The ones stated in the guidelines. Diploma, TOR and birth certificates. Yes that's true, but my uni can issue another copies of those documents so I just submitted the original one..

@@jessicawijayasr just saw your response, I labeled only the first pages and then used a paperclip to keep papers under that label together. But for certificates I used a single plastic file and labeled on the file only. I hope my response is not late for you!

Naguguluhan lang Po kasi ako dun sa form 1 Yung (page 21 ) Po na nakalagay Po is ( HIGH SCHOOL GRADES* {only for terms or semester completed } ). Mula first year to 5th year high school Po ang nakalagay and 1 to 3 semester each column Po sa bawat year. Sorry Po nalilito lang Po talaga ako sir, Kasi Po Diba dito sa Pilipinas 4 years Po Ang high school at 2 years Ang senior high. And then sa senior lang Po Ang may 1st and 2nd semester. But sa junior Wala Po, only first grading to fourth grading each year.

You can ask them to make 4 sets of the reco letter. All in different white envelopes and with sign on each envelope

Seal it in an envelope. Glued with signature. You must follow the instructions precisely or you will be disqualified.

@@jessicawijayasr just saw your response, I labeled only the first pages and then used a paperclip to keep papers under that label together. But for certificates I used a single plastic file and labeled on the file only. I hope my response is not late for you!

No need to remove. Your recommender can attach his/her own reco letter in the form. But the form from the NIIED has to be filled out.
